A comparative evaluation of the GINI index and the modified Glasgow prognostic score in predicting postoperative sarcopenia in rectal cancer patients undergoing NACRT.

OBJECTIVE: Postoperative sarcopenia following neoadjuvant chemoradiotherapy for locally advanced rectal cancer negatively affects recovery and oncological outcomes. Both the Global Immune Nutrition and Inflammation Index (GINI) and the modified Glasgow Prognostic Score (mGPS) are derived from routine laboratory data. There are no studies comparing the performance of these scores and indices in predicting sarcopenia in patients with CT-confirmed sarcopenia. The aim of this study is to determine which of these two indices is a more specific predictor of postoperative sarcopenia in patients who u

Dosimetric evaluation of patient-side range shifters using two materials for superficial region treatment in spot scanning proton therapy: impact of material, air gap and setup distance.

This study aimed to evaluate the dosimetric characteristics of patient-side range shifters (RS) made of polymethyl methacrylate and high-density polyethylene for superficial region treatment in spot scanning proton therapy. The impact of air gap (AG) size and RS setup deviations on dose accuracy was investigated. A virtual water phantom and a simple volumetric beam arrangement were used for dose calculations in a treatment planning system employing a pencil beam algorithm with a triple Gaussian kernel. Gamma passing rates (GPRs) and point dose measurements were performed using a two-dimensiona

Prospective feasibility study of diagnostic CT-based simulation-free palliative radiotherapy.

Conventional radiotherapy (RT) workflows require planning computed tomography (pCT) and treatment planning. However, these processes often take several days, leading to delays in RT initiation. Simulation-free radiotherapy (SFRT), which uses diagnostic CT (dCT) instead of pCT for treatment planning, may reduce the time from consultation-to-treatment delivery. However, the number of prospective studies conducted in Japan remains limited. This study prospectively evaluated the feasibility of SFRT in Japan. A single-center, single-arm prospective study (UMIN000054721) was performed in June 2024.

Comparison of Outcomes between Supramaximal Resection and Gross Total Resection for High-Grade Gliomas: A Prospective Study from a Tertiary Care Centre.

BACKGROUND: High-grade gliomas (HGG), including glioblastomas and anaplastic gliomas, are among the most aggressive primary brain tumors with poor prognosis despite multimodal treatment. Surgical resection remains the cornerstone of management, with gross total resection (GTR) traditionally considered the standard operative goal. However, recent evidence supports supramaximal resection (SMR)-removal of the enhancing tumor along with a margin of infiltrating non-enhancing tumor-as a strategy that may improve progression-free survival and overall survival (OS) without significantly increasing ne

Uncertainty-aware auto-segmentation enables adaptive radiotherapy planning of liver metastases.

PURPOSE: Despite remarkable auto-segmentation performance and growing clinical adoption, models frequently fail to generalize beyond the training distribution. Therefore, this study aimed to develop and validate an uncertainty-aware segmentation pipeline that yields high delineation accuracy for liver cancer patients and reliably identifies safety-critical segmentation imperfections in radiotherapy. MATERIALS AND METHODS: Datasets of n = 173 patients with 224 liver metastases were divided into training (n = 100), validation (n = 33), and test (n = 40). Each case included annotated T2-weighted

Development and internal validation of a clinicopathological and CT radiomics based prediction model for extracranial distant metastasis after radical resection in early-stage non-small cell lung cancer.

BACKGROUND: Wide use of low-dose chest CT detects more early NSCLC, though recurrence (mostly extracranial distant metastasis) still occurs. This study aimed to establish and validate computed tomography (CT) radiomics models that can be used for predicting the risk of extracranial distant metastasis (EDM) after radical resection in early-stage non-small cell lung cancer (e-NSCLC). METHODS: This retrospective study included 312 patients with e-NSCLC who underwent radical surgery at Shandong Cancer Hospital from 2014 to 2019. The cases were randomly divided into a training set (n=200) and a va

A Single Center Retrospective Study: Efficacy and Platelet Preservation of Double Plasma Molecular Adsorption System Therapy by Membrane Separation Versus Centrifugal Separation in Acute-on-Chronic Liver Failure Patients With Hepatocellular Carcinoma.

Hepatitis B virus (HBV) infection is a leading cause of hepatocellular carcinoma (HCC) and liver failure. The double plasma molecular adsorption system (DPMAS) reduces bilirubin and may create a therapeutic window for anti-tumor therapy in HCC patients. Two plasma separation techniques are used: membrane-based (MBS-DPMAS) and centrifugal (CPS-DPMAS), but no study has compared them specifically in HCC patients with acute-on-chronic liver failure (ACLF). This retrospective study included 203 HBV-related HCC patients who received DPMAS without plasma exchange (93 MBS-DPMAS, 110 CPS-DPMAS), with c

Selective preoperative lower extremity computed tomography angiography before mandibular reconstruction: Identifying patients unlikely to require fibula free flap reconstruction.

PURPOSE: Not every patient undergoing mandibular continuity resection requires free fibula flap (FFF) reconstruction. Whether clinical judgment reliably identifies patients unlikely to benefit from preoperative lower-extremity computed tomography angiography (CTA) remains unknown, despite the relevance of this question for cost-conscious and resource-efficient imaging strategies. METHODS: A retrospective outcome-based validation was conducted in patients undergoing mandibular continuity resection between July 2012 and June 2023. The primary outcome was secondary FFF reconstruction among patie

A patch-based bolus technique for total scalp irradiation: dosimetric impact of conformity and reproducibility.

Total scalp irradiation (TSI) for scalp angiosarcoma remains technically challenging because of the large, highly curved target geometry and the need for reliable bolus application. This study aimed to develop a practical method to improve bolus conformity using commercially available bolus and to evaluate the dosimetric impact of bolus reproducibility in volumetric-modulated arc therapy (VMAT)-based TSI. An anthropomorphic head phantom was used to simulate TSI conditions. A three-segment 'patch bolus' method was developed to enhance conformity and setup stability. Treatment plans were generat

Bronchial artery embolization followed by bronchoscopic electrosurgical snare resection for diagnosis and treatment of primary pulmonary osteosarcoma: case report and literature review.

Primary pulmonary osteosarcoma (PPOS) is an extremely rare extraskeletal osteosarcoma. Because its clinical and radiological features are nonspecific, establishing a diagnosis is challenging. We report the case of a 58-year-old man who presented with a 1-month history of cough and 5 days of dyspnea. Initial chest computed tomography showed destructive changes in the left lung and an endobronchial lesion in the left main bronchus. To relieve airway obstruction and obtain diagnostic tissue, a sequential strategy of bronchial artery embolization followed by bronchoscopic electrosurgical snare res
