Association of Frailty With Clinical Outcomes in Patients Receiving Primary Prevention Implantable Cardioverter Defibrillators: A Prospective Cohort Study.

BACKGROUND: Frailty predisposes individuals to morbidity and mortality. Increasing numbers of elderly and comorbid individuals are undergoing primary prevention implantable cardioverter defibrillator (ICD) device placement. Little is known about the association of frailty with post-device implantation outcomes. METHODS: We conducted a single-center, prospective cohort study of 71 patients who underwent primary prevention ICD insertion and who had their baseline frailty status assessed using the Fried index. Participants were followed for a median period of 7.8 years. RESULTS: The mean age (±

[Hypovolaemic and haemorrhagic shock].

Hypovolemic and hemorrhagic shock are life-threatening conditions that, if untreated, rapidly lead to multi-organ failure and death. These conditions result from significant intravascular fluid or blood loss, causing critical organ hypoperfusion. The underlying pathophysiology involves complex hemodynamic, inflammatory, and coagulation disturbances that may progress to irreversible organ dysfunction. Rapid diagnosis, early hemorrhage control, and targeted hemodynamic and hemostatic therapy are crucial to improve patient outcomes. Diagnosis is based on clinical symptoms, laboratory parameters,

Management of Vasodilatory Shock: A Concise Review.

Vasodilatory shock is a severe circulatory disorder characterized by excessive vasodilation, resulting in impaired tissue perfusion and organ dysfunction. The condition arises from dysregulated vasodilation and reduced vascular responsiveness to endogenous vasoconstrictors, disrupting normal vasoregulatory mechanisms. This review delves into the complex pathophysiology, clinical significance, and treatment approaches for vasodilatory shock. It offers a comprehensive summary of established and emerging pharmacological and nonpharmacological therapies. Designed for clinicians and researchers, th

Trauma room requirements.

Effective shock (or trauma) room management requires thorough preparation, staff competence, dedicated infrastructure and organised protocols. Shock rooms must be sufficiently equipped according to the hospital's designated level of care. This chapter outlines essential aspects, including equipment and facilities, staff qualifications and composition, and communication practices. Key areas including initial assessment, diagnostic procedures, emergency interventions, and future aspects in the care of severely injured patients are addressed, along with emerging innovations in trauma care.

Low-intensity pulsed ultrasound elevates blood pressure for shock.

Fluid replacement is the primary treatment for life-threatening shock but is challenging in harsh environments. This study explores low-intensity pulsed ultrasound (LIPUS) as a resuscitation strategy. Cervical LIPUS stimulation effectively elevated blood pressure in shocked rats. It also improved cerebral and multiorgan perfusion. Mechanistically, LIPUS activated pathways related to sympathetic nerve excitation and vascular smooth muscle contraction, increasing plasma catecholamines and stimulating blood pressure-regulating neural nuclei. Partial sympathetic nerve transection reduced LIPUS eff

The treatment of autoimmune hemolytic anemia with complement inhibitor iptacopan: a case report.

Autoimmune hemolytic anemia (AIHA) is a type of hemolytic anemia. In this condition, the body produces anti-red blood cell autoantibodies due to immune dysfunction. This results in accelerated destruction of red blood cells. According to the best temperature for autoantibodies to stick to red blood cells, there are three types: warm antibody type (wAIHA), cold antibody type (cAIHA), and mixed warm-cold antibody type (mAIHA). This article presents a case of acute severe warm antibody autoimmune hemolytic anemia in an elderly male patient. The patient exhibited symptoms including jaundice of the

Long-term safety and efficacy of subcutaneous implantable cardioverter-defibrillator compared with transvenous implantable cardioverter-defibrillator in propensity score-matched patients from Japan.

BACKGROUND: Subcutaneous implantable cardioverter-defibrillator (S-ICD) has been reported to be non-inferior to transvenous ICD (TV-ICD) in terms of device-related complications and inappropriate shock (IAS). We aimed to evaluate the long-term clinical outcomes of S-ICD compared with TV-ICD in Japanese patients. METHODS: We studied 315 consecutive patients (TV-ICD, 167; S-ICD, 148) who underwent ICD implantation. A propensity score matching analysis was performed to select patient subgroups for comparison (104 patients in each group). Clinical outcomes, including appropriate and inappropriate

Does pediatric trauma center designation matter for children in shock from gunshot wounds? A Trauma Quality Improvement Program analysis.

BACKGROUND: Recent studies have demonstrated improved outcomes for severely injured pediatric trauma patients treated at pediatric trauma centers (PTCs). Nonetheless, specific injury patterns requiring immediate lifesaving intervention may offset the recognized benefits of PTC over adult trauma centers (ATCs). This study aims to compare the clinical outcomes of hypotensive pediatric trauma patients with gunshot wounds (GSWs), based on trauma center type. We hypothesize that outcomes are equivalent for this clinical scenario. METHODS: The 2013-2021 Trauma Quality Improvement Program data set w

Clinical and metabolic phenotyping of continuous versus intermittent ENteral NUTrition in ventilated adults with shock: ENNUT trial protocol.

INTRODUCTION: Critically ill patients often require enteral nutrition, but the optimal feeding strategy-continuous or intermittent-remains uncertain. While continuous enteral nutrition ensures steady nutrient delivery, it may inhibit key metabolic and cellular processes such as autophagy and ketogenesis. Intermittent enteral nutrition, by mimicking fasting periods, could activate protective pathways, potentially improving clinical outcomes. However, evidence for its efficacy and safety in intensive care units (ICUs) is limited. This study evaluates the clinical and metabolic impacts of fasting

Contemporary Implantable Cardioverter-Defibrillator Benefit: Time to First Therapy in a Large, Real-World Cohort.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) reduce mortality in patients at risk of sudden cardiac death. Recent improvements in heart failure care may impact the rate of ICD therapy received by patients. Thus, an updated examination of ICD benefit is needed. OBJECTIVES: This study sought to assess time to first therapy in contemporary ICD patients and infer the proportion of appropriate therapies based on an adjudicated subset of randomly selected episodes. METHODS: A query of the Medtronic CareLink de-identified database from 2013 to 2024 was performed. First-time Medtronic

Continuous versus intermittent noninvasive blood pressure measurement in patients with shock in prehospital emergency medicine - a single-center prospective pilot trial.

BACKGROUND: Shock is a critical and potentially life-threatening clinical state characterized by circulatory insufficiency and impaired micro- and macrocirculation. Rapid detection and initiation of therapy are essential for patient outcomes. In prehospital emergency medicine, assessment tools are limited, and intermittent noninvasive blood pressure (iNIBP) monitoring is the current standard of care. Recent findings suggest that this method may miss episodes of relevant hypotension. Continuous noninvasive blood pressure (cNIBP) and tissue oxygenation (StO2) measurements could improve the time

Single-walled carbon nanotubes hitchhike on Ly-6Chi monocytes for photoacoustic image-guided photothermal shock therapy of metabolic inflammation.

Metabolic inflammation (metaflammation) may occur throughout the body and evolve into various metabolic syndromes, so developing a universal approach for diagnosing and treating metaflammation is meaningful. Here, we developed a single-walled carbon nanotube (SWNT)-based photothermal shock therapy (PTST) for photoacoustic image-guided therapy of metaflammation. Mechanistically, SWNTs are specifically taken up by Ly-6Chi monocytes in peripheral blood and then rerouted to inflamed tissues via Ly-6Chi monocyte hitchhiking. Due to the accumulation of SWNTs, inflamed tissues can be distinguished by

Restrictive fluid management with early de-escalation versus usual care in critically ill patients (reduce trial): a feasibility randomized controlled trial.

BACKGROUND: Optimal fluid management in critically ill patients varies across different phases of care. This study examined the feasibility of adding a restrictive fluid strategy with early de-escalation to standard care for patients with circulatory shock. METHODS: We performed a single-center, feasibility, randomized controlled trial, involving patients with shock who required fluid resuscitation and were admitted in the medical intensive care unit. After initial fluid resuscitation, patients were randomly assigned (1:1) to either a restrictive fluid strategy or usual care. The restrictive

Sarcopenia index as a predictor of ventricular arrhythmia risk in patients with type 2 diabetes mellitus: a longitudinal study.

OBJECTIVE: The correlation between sarcopenia and diabetes mellitus (DM) holds considerable importance. The prognosis of ventricular arrhythmias in individuals with type 2 diabetes mellitus complicated by sarcopenia remains uncertain. METHODS: A retrospective analysis was conducted on patients with DM who underwent implantable cardioverter defibrillator (ICD) implantation at Fuwai Hospital. Sarcopenia status was assessed using the biomarker sarcopenia index(SI). Patients were divided into low SI (less than 81.33, n = 74) and high SI (greater than 81.33, n = 158) groups according to the best c

Predicting arrhythmia recurrence post-ablation in atrial fibrillation using explainable machine learning.

BACKGROUND: Following atrial fibrillation ablation, it is challenging to distinguish patients who will remain arrhythmia-free from those at risk for recurrence. New explainable machine learning (xML) techniques allow for systematic assessment of arrhythmia recurrence risk following catheter ablation. We aim to develop an xML algorithm that predicts recurrence and reveals key risk factors to facilitate better follow-up strategy after an ablation procedure. METHODS: We reconstructed pre-and post-ablation models of the left atrium (LA) from late gadolinium enhanced magnetic resonance (LGE-MRI) f
