Use of a novel mechanical rinsing and aspiration thrombectomy device for treatment of deep venous and arteriovenous graft thrombosis.

The KPS Rinspirator (Kerberos Proximal Solutions, Cupertino, Calif.) is a new thrombectomy device that operates by manually controlled, simultaneous, intravessel infusion and aspiration of fluid to cause localized clot dissolution. We evaluate the ability of the KPS Rinspirator to treat acute (<3 days) and subacute (3-7 days) deep venous and arteriovenous graft thrombosis in 4 patients and 13 vessels (2 arteriovenous grafts and 11 deep veins). Technical and clinical success was achieved in the two patients with acute arteriovenous graft thromboses. Therefore, in our experience, successful "rin

The hazards of gastric lavage for intentional self-poisoning in a resource poor location.

OBJECTIVE: The 10-20% case fatality found with self-poisoning in the developing world differs markedly from the 0.5% found in the West. This may explain in part why the recent movement away from the use of gastric lavage in the West has not been followed in the developing world. After noting probable harm from gastric lavage in Sri Lanka, we performed an observational study to determine how lavage is routinely performed and the frequency of complications. CASE SERIES: Fourteen consecutive gastric lavages were observed in four hospitals. Lavage was given to patients unable or unwilling to unde

Necrotising fasciitis of the shoulder following muscular strain.

Necrotising fasciitis is a rare, life-threatening, soft-tissue infection characterised by rapidly spreading inflammation and subsequent necrosis of the muscle, fascia and surrounding tissues. We report a case of necrotising fasciitis originating from the shoulder in a 59-year-old female patient. Necrotising fasciitis of the shoulder is very rare and has a poor prognosis because of its potential to spread to the chest wall. It can occur in otherwise healthy people without any predisposing conditions and could present as muscle strain. Disproportionate pain is the hallmark of this condition. Ult

Vascular and nonvascular complications of renal transplants: sonographic evaluation and correlation with other imaging modalities, surgery, and pathology.

Cadaveric or living donor renal transplantation is commonly performed in individuals with end-stage renal disease. In recent years, gray-scale sonography, coupled with color Doppler sonography (CDUS), power Doppler sonography (PDUS), or spectral Doppler sonography, has become the primary imaging modality for these patients. Postoperative serial sonography is performed to detect complications and aid in posttransplant management. In addition, sonography is used to guide percutaneous aspiration of fluid or biopsy to diagnose rejection or renal and perirenal masses. In this article we discuss the

Management of aneurysm sac hygroma.

PURPOSE: To document the management strategies and outcome of patients diagnosed with sac hygroma following open or endovascular abdominal aortic aneurysm (AAA) repair. METHODS: Seven men (median 68 years, range 43-79) with previous open (n=3) or endovascular (n=4) AAA repairs and increasing aneurysm diameters documented on spiral computed tomography (CT) were diagnosed with sac hygroma based on the lack of a demonstrable endoleak on CT imaging; the presence of a gelatinous, clear fluid in the sac; and a nonpulsatile sac pressure that was about one third of the systemic blood pressure. The history of interventional ultrasound.

Ultrasound has been used to guide interventional procedures for more than 30 years. Initial applications included biopsy techniques and simple aspiration of fluid collections. However, with improved sonographic imaging and the use of different ultrasonic probes as well as development of less invasive therapies, there has been an increase in the use of ultrasound to guide interventional procedures. Interventional ultrasound has become routine in most medical specialties. Because of the remarkable success of interventional procedures guided by sonography, combined with an outstanding safety reco

Early prenatal diagnosis by celocentesis.

BACKGROUND: Celocentesis is the ultrasound-guided aspiration of fluid from the extra-amniotic cavity at 7-8 weeks of gestation. This paper reports on the clinical application of celocentesis for early prenatal diagnosis. METHODS: Celocentesis was successfully performed in nine pregnancies and 1-2 mL of fluid were obtained after one needle insertion. The indications were prenatal diagnosis of beta-thalassemia or sickle cell disease (n = 6), Marfan syndrome (n = 1) and paternity testing (n = 2). Molecular biological techniques were used to analyze the celomic fluid and this was successfully car

Infected necrosis complicating acute pancreatitis: experience with 131 cases.

OBJECTIVE: Despite advances in its management, the mortality of infected pancreatic necrosis (IPN) remains high. We report our observations on complications and treatment of IPN. METHODS: We studied 131 patients with IPN seen over a 20-year period. Infection, suspected clinically, was proved by presence of extraluminal air on CT scan (23 cases), or by guided percutaneous aspiration of fluid or solid necrotic tissue, and bacteriological studies of the aspirate. Apart from organ support, vigorous nutritional support and appropriate antibiotic therapy were instituted. Evacuation of pus and surgi

[Place of pathology in the forensic diagnosis of drowning].

The diagnosis of drowning is one of the most difficult in forensic pathology. Drowning is death through the aspiration of fluid into the air passages. Signs of immersion only demonstrate submersion of the body for a period of time but are not signs of drowning. The best signs of drowning are froth around the mouth and nostrils and lung distension. Lung histology in drowning victims shows non-specific lesions such as "emphysema aquosum" and alveolar edema. The diatom test for drowning requires a careful protocol to avoid possible contamination. Sample collection (cardiac blood, lung, liver, kid

Pasteurella multocida tenosynovitis of the hand: sonographic findings.

Pasteurella multocida is a common cause of infection in humans subsequent to bites or scratches by dogs and, particularly, cats. This infection usually results in superficial skin and soft tissue infections. Sonography can be used for diagnosing inflammatory conditions affecting tendons, including acute and chronic tenosynovitis. P. multocida tenosynovitis is rare, and the diagnosis can be missed if adequate tests are not performed. We report 2 cases of P. multocida tenosynovitis of the hand and wrist in which sonography played a valuable role in assessing the affected tissues and guiding fine

ProSeal laryngeal mask protects against aspiration of fluid in the pharynx.

BACKGROUND: The ProSeal laryngeal mask airway (PLMA) is a new device designed to isolate the airway from the digestive tract. METHODS: We studied the ability of the PLMA to isolate the airway in 103 anaesthetized adults who were breathing spontaneously or given neuromuscular blocking agents, by filling the hypopharynx with methylene blue-dyed saline introduced down the drainage tube once the mask was in place. At the beginning and end of the procedure, a fibre-optic bronchoscope was passed down the airway tube to observe any dyed saline in the bowl of the mask. RESULTS: The PLMA was position

Accidental pneumothorax from a nasogastric tube in a patient with severe hemineglect: a case report.

Nasogastric tubes are frequently used for nutrition of patients with neurologic diseases. We report an instance of inadvertant placement of a standard nasogastric tube into the left pleural space in a patient with right parietotemporal intracerebral hemorrhage and severe hemineglect on the left side. The 2 confirmatory maneuvers-aspiration of fluid and auscultating the abdomen on insufflating air-were false-positive. We conclude that only radiologic confirmation of the position of nasogastric tubes and the awareness of the associated dangers will help minimize the occurrence of such events in

Percutaneous aspiration of fluid for management of peritonitis in space.

BACKGROUND: As a medical emergency that can affect even well-screened, healthy individuals, peritonitis developing during a long-duration space exploration mission may dictate deviation from traditional clinical practice due to the absence of otherwise indicated surgical capabilities. Medical management can treat many intra-abdominal processes, but treatment failures are inevitable. In these circumstances, percutaneous aspiration under sonographic guidance could provide a "rescue" strategy. HYPOTHESIS: Sonographically guided percutaneous aspiration of intra-peritoneal fluid can be performed i
