Hybrid endoscopic full thickness resection for a gastric submucosal tumor.

A 53-year-old woman was diagnosed with a 15-mm submucosal tumor located in the fundus of the stomach. Endoscopic ultrasonography suggested that the tumor originated from the muscularis propria layer. Therefore, we decided to use a novel hybrid EFTR to remove the lesion. First the tumor was marked and the mucosal was incised, followed by primary tumor dissection. Then the endoscope was withdrawn. A snare was fixed to the transparent cap at the front end of the endoscope without passing through the working channel. The endoscope and snare were inserted together near the lesion. A foreign body fo

A rabbit model of clinically relevant mucosal injury induced by nasogastric tube intubation.

Mucosal injury is a common complication in nasogastric tube (NGT) intubation and causes continuous pain and secondary inflammation in clinical practice. We developed a New Zealand rabbit model to elucidate the occurrence and progression of the mucosal injury in NGT intubation. To simulate the clinical intubation procedure, the NGT was blindly inserted from the nose to the stomach. We first investigated the impact of the NGT size on the intubation success ratio in the rabbit. The high success rate of intubation made the NGT of Fr/Ch6 an appropriate choice. The degree of mucosal injury along wit

Clinical performance of i-gel and i-gel Plus for airway management of obese patients: A prospective randomised controlled trial.

BACKGROUND AND AIMS: The i-gel Plus (i-gelP) is a refined version of i-gel supraglottic airway device (SAD), featuring an extended cuff tip, an intubation ramp, and a broader gastric channel, which may contribute to better performance. However, despite the anatomical and physiological airway challenges in obese individuals, scientific data on its use in this population are unavailable. The aim of this trial was to compare i-gel and i-gelP for perioperative airway management in obese patients. METHODS: It is a prospective, single-centre, single-blind, randomised controlled trial. Seventy-six a

Risk factors, and outcomes of patients with carbapenem-resistant Enterobacterales bloodstream infection: an eight-year case-case-control study.

Carbapenem-resistant Enterobacterales bloodstream infection (CRE-BSI) represents a major and urgent challenge to global public health. Some patients with CRE-BSI have a greater risk for poor clinical outcomes, thus identifying risk factors for CRE-BSI is required to determine the most at-risk populations. Here, we investigated risk factors for CRE-BSI by conducting a retrospective case-case-control study at Tianjin Medical University General Hospital, between 2017 and 2024. A total of 144 patients with CRE-BSI were enrolled in this case-case-control study. Each case was matched simultaneously

Electromagnetic-Guided Nasogastric Tube Insertion by Nurses: A Multicenter Non-Inferiority Study.

UNLABELLED: <p>Introduction: Nasogastric tube feeding is essential for critically ill patients, yet blind insertion risks misplacement and related complications. Electromagnetic guidance enables real-time tracking, potentially enhancing safety and efficiency. This study aimed to compare the safety and effectiveness of nurse-led electromagnetic-guided nasogastric tube insertion with those of physician-performed blind insertion in critically ill patients. METHODS: This multicenter prospective non-inferiority trial was conducted in seven Japanese intensive care units (July 2023-February 2025). P

Predictors of Percutaneous Endoscopic Gastrostomy-Related Complications in Amyotrophic Lateral Sclerosis: A 19-Year Retrospective Study From a Tertiary Center.

OBJECTIVES: Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease that inevitably leads to swallowing difficulties as the disease progresses. Percutaneous endoscopic gastrostomy (PEG) is recommended for optimal supportive management of dysphagia among these patients. We aimed to investigate PEG-related complications and risk factors in patients with ALS. METHODS: Medical records of the ALS patients who underwent PEG from March 2006 to February 2025 in a single tertiary care center were retrospectively reviewed. PEG-related complications and risk factors were assessed throug

Release-Independent Transcription from Covalently Cross-Linked Hierarchically Folded DNA Condensates.

Ordered DNA condensates assembled with a block copolymer of PEG-poly(l-lysine) hierarchically fold the supercoiled plasmids into anisotropic "nanoworms" that maintain duplex continuity and display sequence-periodic, single-stranded hinges. After irreversible chemical cross-linking to preclude the potential DNA release, these architecturally preserved nanoscale condensates sustain sequence-specific transcription and coupled luciferase translation at efficiencies equaling or surpassing naked DNA, whereas jetPEI-derived disordered spheres remain transcriptionally silent. In both live cells and an

Gastric Proton Pumpopathy Associated with Protein Sorting Machinery: A Narrative Review.

BACKGROUND: Gastric acid secretion by parietal cells depends on the trafficking and exocytosis of H⁺/K⁺-ATPase-rich tubulovesicles (TVs) to the apical membrane in response to histamine stimulation via cyclic AMP elevation. However, the role of parietal cell's protein sorting machinery [eg, coat protein (COP) II/COP I vesicle proteins, soluble NSF attachment protein receptors (SNAREs), Rab GTPases, clathrin-adaptor proteins, etc.] in the dysregulation of gastric acid homeostasis remains poorly understood. This review synthesizes available evidence on the role of protein sorting machinery in gas

Kirigami-Inspired Deployable Microneedle Robot With Enhanced Conformability for Targeted Oral Drug Delivery.

Oral drug delivery is widely accessible to patients for the treatment of gastrointestinal diseases such as gastric ulcers. However, the efficacy of orally delivered drugs is often hindered by their short duration of acting and low local drug concentration, due to the dynamic environment of the gastrointestinal tract. Oral microneedle drug delivery systems offer a promising solution for achieving long-term drug release at target sites by inserting microneedles into tissues to prolong drug retention. Yet, their effectiveness is compromised by poor flexibility, which impedes microneedle insertion

Surgical Management of Left Atrial Myxoma through Right Mini-Thoracotomy in a Patient with Retrosternal Gastric Tube.

INTRODUCTION: Open-heart surgery in patients with a retrosternal gastric tube after esophagectomy presents a significant surgical challenge owing to the risk of conduit injury during median sternotomy. In such cases, a minimally invasive cardiac surgery (MICS) strategy to avoid sternotomy is of paramount importance and must be tailored according to individual anatomy. CASE PRESENTATION: A 71-year-old man with a history of robot-assisted subtotal esophagectomy and retrosternal gastric tube reconstruction was found to have a mobile left atrial mass attached to the interatrial septum. Preoperati

Pattern of Usage of Supraglottic Airway Devices in Pediatric Patients among Anesthesiologists.

BACKGROUND: Supraglottic airway devices (SADs) have become integral to pediatric anesthesia, offering a less invasive alternative to endotracheal intubation with advantages in ease of insertion, hemodynamic stability, and reduced airway trauma. Various SADs, such as the classic laryngeal mask airway (LMA), ProSeal LMA, I-gel, and Ambu AuraOnce are available, each differing in design, seal pressure, and clinical indications. Despite their widespread use, practice patterns among anesthesiologists regarding SAD selection, insertion techniques, and perioperative management in pediatric patients va

Surgical Surprise and a Ryle's Tube Misadventure: An Intraoperative Gastric Perforation.

Nasogastric tube (NGT) insertion is routinely performed for enteral feeding and gastric decompression and is generally considered safe. However, it can lead to complications ranging from minor discomfort, epistaxis, and sinusitis to severe events such as respiratory injury, esophageal perforation, and, rarely, gastrointestinal perforation. Among these, gastric perforation in adults is extremely uncommon. We describe the case of a 47-year-old woman with carcinoma of the left lateral tongue who underwent near-total glossectomy with bilateral neck dissection, tracheostomy, and reconstruction. Aft

Fiber-Guided Red Light-Emitting Diode Localization of an Esophageal Blind End in Long-Gap Esophageal Atresia Surgery: A Case Report.

INTRODUCTION: Esophageal atresia (EA) Gross type A is a congenital malformation in which both the upper and lower esophagus have blind ends without a tracheoesophageal fistula. The gap between the segments is often long (long-gap EA), making intraoperative localization challenging. We report a case in which the lower esophageal blind end was readily localized using Tumguide (Otsuka Pharmaceutical Factory, Tokushima, Japan), a device that transmits biologically transparent red light from a light-emitting diode (LED) through an optical fiber. CASE PRESENTATION: The patient was a 2-month-old gir

Anesthetic management of a gross type C tracheoesophageal fistula by balloon catheter via tracheostoma in an infant with trisomy 18: A Case Report.

RATIONALE: Gross Type C congenital esophageal atresia is associated with tracheoesophageal fistula (TEF). Perioperative airway management, including prevention of aspiration of gastric contents, is important. Generally, the TEF is blocked by 1-lung intubation of the dependent lung, or directly by insertion of a balloon catheter. This case report describes successful management of TEF by insertion of a balloon catheter via tracheostoma in an infant with a difficult airway. PATIENT CONCERNS: A boy with trisomy 18 underwent esophageal banding and gastrostomy for Gross Type C congenital esophagea

Successful Spontaneous Twin Pregnancy Following Bone Marrow Transplant and Gastric Cancer Surgery: A Case Report.

BACKGROUND Pregnancy following bone marrow transplantation (BMT) and treatment for gastric cancer is rare because of gonadotoxic conditioning regimens, major abdominal surgery, and the overall burden of chronic illness. Reports of spontaneous twin pregnancy after both allogeneic BMT and gastrectomy are exceptionally uncommon. CASE REPORT We describe the case of a 22-year-old woman with a history of severe aplastic anemia treated with allogeneic BMT (February 2022), who subsequently underwent subtotal gastrectomy for gastric adenocarcinoma (May 2022). She conceived spontaneously with a dichorio
