Emergence of vancomycin-variable enterococci in urinary tract infections in north India: A cross-sectional molecular study.

BACKGROUND: Vancomycin-resistant enterococci (VRE) and Vancomycin-variable enterococci (VVE) pose significant challenges in urinary tract infections (UTIs) due to limited treatment options and silent resistance genes. OBJECTIVE: To determine the prevalence, resistance profiles, and clinical outcomes of VRE and VVE in UTIs at a Regional AMR Surveillance Centre in North India. METHODS: A cross-sectional study was conducted among hospitalised UTI patients (≥18 years) at a tertiary care hospital. Enterococcus isolates were identified by MALDI-TOF MS and tested for antimicrobial susceptibility (C

Candida auris colonization and bloodstream infection in selected intensive care units of Dhaka city, Bangladesh.

UNLABELLED: Candida auris has emerged globally as a multidrug-resistant fungal pathogen, posing a significant threat in intensive care units (ICUs) due to its ability to spread rapidly and cause severe infections. Although its global significance is well recognized, data on its epidemiology in Bangladesh remain limited. This study aimed to assess the prevalence, acquisition, and clinical implications of C. auris colonization and bloodstream infection among ICU patients in Dhaka. A cross-sectional exploratory study was conducted from August 2021 to September 2022 in the ICUs of one public and o

Catheter-associated urinary tract infections in patients with gastric cancer after surgery: A retrospective study.

Perioperative catheterization is a well-known risk factor for developing postoperative catheter-associated urinary tract infection (CAUTI). The information about the effect of extended urinary catheterization after surgery for gastric cancer is still scarce. This is a retrospective study of 232 patients with gastric cancer who underwent surgery. Patients were divided into 2 groups based on the duration of their catheter placement: <72 hours and 72 hours or more. The primary outcome was CAUTI during the index hospitalization. Logistic regression was used to calculate odds ratios in unadjusted,

Interactive mHealth Applications for Caregiver Training in Urinary Catheterization: A Scoping Review.

Background/Objectives: Urinary catheterization is common across care settings, but safe management at home and during care transitions often depends on caregivers who receive limited and inconsistent training. Mobile health (mHealth) applications may support caregiver education and decision-making. This review mapped and synthesized evidence on interactive mobile applications for caregiver training in urinary catheterization and developed a conceptual framework to inform nursing practice. Methods: A scoping review was conducted according to Joanna Briggs Institute guidance and reported followi

Prostatic Artery Embolization: A Viable Option for Catheter Independence after Failed Medical Management of Urinary Retention.

This report aimed to evaluate trial without catheter (TWOC) success after prostatic artery embolization (PAE) in catheterized men who had failed medical management and to identify predictors of success and retreatment patterns. Retrospective single-center exploratory analysis included 53 consecutive catheterized men undergoing PAE between January 2019 and March 2025. TWOC succeeded in 29 (54.7%) of 53 patients, lower than prior series, likely reflecting the chronic catheter-dependent cohort and lower bilateral embolization rate. Among successful cases, 2 (6.9%) of 29 experienced recurrent rete

Assessing the Effectiveness of Multidisciplinary Interventions in Reducing Catheter-Associated Urinary Tract Infection: An Interrupted Time Series Modeling Approach.

BACKGROUND: Recommendations support multi-interventional catheter-associated urinary tract infection (CAUTI) reduction programs; however, identifying specific interventions with significant impact may be beneficial in resource-limited settings. PURPOSE: To determine which intervention(s) were most effective at reducing CAUTI Standardized Infection Ratio (SIR) and urinary catheter utilization. METHODS: Interventions were implemented from September 2020 to February 2023, including Clinical Quality rounds, silicone to latex urinary catheter transition, nurse-driven removal protocol revision, an

Clinical Characteristics and Risk Factors of Carbapenem-Resistant Escherichia coli in Patients with Urinary Tract Infections: Development of a Predictive Nomogram.

BACKGROUND: Carbapenem-resistant Escherichia coli (CREC) is a growing threat in urinary tract infections (UTIs). This study aimed to analyze the clinical characteristics and risk factors of patients with CREC UTIs and to construct a prediction nomogram. METHODS: This case-control study retrospectively analyzed clinical data of 162 patients with CREC UTIs (2018-2024). One hundred and forty-eight cases were matched 1:1 with non-CREC UTI controls by department, gender, and age (±1 year). Univariate and multivariate logistic regression analyses identified independent risk factors of CREC UTIs. R

Impact of Institutional Protocol on Urinary Catheter Outcome Measures in Orthopedic Children Treated With Epidural Analgesia.

BACKGROUND: Epidural analgesia is commonly used for pain control after major lower-limb orthopedic surgery in children, but it is associated with a risk of postoperative urinary retention. Consequently, urinary catheters are often placed and left in situ for the full duration of epidural analgesia, despite the potential risks of prolonged catheterization, including discomfort, ward reinsertion, and urinary tract infection. AIMS: To assess the impact of implementing a standardized institutional urinary catheter management protocol on postoperative urinary catheter outcomes in children less tha

Omission of indwelling urinary catheter after robotic partial nephrectomy in female patients: a single-surgeon experience.

To assess the necessity of indwelling urinary catheters in female patients undergoing robotic partial nephrectomy by comparing their outcomes. An IRB-approved, prospectively maintained renal cancer database was reviewed for female patients undergoing robotic partial nephrectomy by a single surgeon at one institution between February 2020 and May 2025. Data was stratified by the use of urinary catheters. The primary outcome was the rate of urinary retention, necessitating the discharge from the hospital with a catheter. Secondary outcomes included perioperative variables. Statistical analysis i

[Life-Sustaining Treatments: a requirement for accessing medically assisted suicide, or a trap for doctors and patients?].

The paper analyzes the concept of "Life-Sustaining Treatments" (LST) in the context of Medically Assisted Suicide (MAS) in Italy. The authors highlight how the requirement of LST, established by Constitutional Court Ruling n. 242/2019 along with three other criteria (irreversible pathology, intolerable suffering, decision-making capacity), has often been used to reject requests for access to MAS. Subsequent Constitutional Court rulings (135/2024 and 66/2025) have progressively expanded the definition of LST, including care procedures such as bowel evacuation, urinary catheterization, and mucus

Traumatic extraperitoneal bladder repair is associated with fewer leaks and shorter catheter duration.

BACKGROUND: Societal guidelines for the management of extraperitoneal bladder injuries (EBIs) are based on limited evidence. Current recommendations support nonoperative management of simple EBIs despite a high persistent urinary leak rate. Conversely, operative management is often associated with a lower leak rate. This study evaluated clinical outcomes associated with operative versus nonoperative management of EBIs. We hypothesized that cystorrhaphy would be associated with fewer persistent urinary leaks, reduced catheter duration, fewer infectious complications, and decreased interval imag

Risk factors associated with urinary tract infection within 4 days of male rectal cancer surgery in the era of enhanced recovery after surgery (ERAS) programs.

BACKGROUND: Bladder drainage is systematically used in rectal cancer surgery in male patients, even in the era of enhanced recovery after surgery (ERAS). However, little data is available on risk factors for urinary tract infection (UTI). Identifying the risk factors associated with UTI within 4 days of male rectal cancer surgery in an ERAS program could support more individualized decision-making. METHODS: We used data from the GRECCAR 10 randomized clinical trial, a comparison of outcomes of transurethral catheterization (TUC) or suprapubic catheterization (SPC). 240 patients were randomize

Cumulative Vulnerability in Cardiac Critical Care: A Framework for Understanding Healthcare-Associated Infections and Their Progression to Severe Infection and Sepsis.

Background and Objectives: Healthcare-associated infections (HAIs) remain a relevant complication in coronary care units (CCUs), particularly among patients with cardiac dysfunction requiring invasive monitoring and prolonged hospitalization. In this setting, infection occurrence may reflect the cumulative interaction between baseline biological vulnerability and care-related exposure. This study aimed to explore whether a simple cumulative framework integrating these components can describe patterns of HAI occurrence and support early identification of patients at risk for severe infectious c

Optimal duration of urethral catheterization in ThuLEP: A randomized prospective pilot study.

INTRODUCTION: Lower urinary tract symptoms (LUTS) from benign prostatic obstruction (BPO) are common in aging men. Surgical interventions such as TURP and laser enucleation are established treatments for drug-refractory BPO. Holmium laser enucleation (HoLEP) has demonstrated favorable long-term safety and efficacy with shorter hospitalization and catheterization times compared to standard TURP. Thulium laser enucleation (ThuLEP) emerges as an effective and safe option, potentially offering advantages including reduced blood loss and shorter catheterization periods compared to TURP. Indwelling Urinary Catheters and Infection Risk: An Assessment of Risk Factors, Necessity, and Impact in Male Patients.

INTRODUCTION: Indwelling urinary catheter (IUC) placement is a significant risk factor for catheter-associated urinary tract infections (CAUTI). Most prior studies of CAUTI include both male and female patients, resulting in heterogenous generalizable cohorts. However, male patients have distinct anatomical differences and sex-specific justifications for IUC placement. This study aimed to exclusively characterize the common indications for in-hospital IUC placement in male patients and identify risk factors associated with CAUTI in this cohort. METHODS: This is a retrospective single-center s
