Pre-procedural disclosure of potential diagnostic errors in radiology: public perspectives in the Netherlands.

OBJECTIVE: To assess public attitudes toward prospective disclosure of diagnostic error risk in radiologic imaging (i.e., informing patients in advance about the possibility a radiologic examination may yield an incorrect finding, rather than disclosure of an error after it has occurred) and to identify factors influencing these preferences. MATERIALS AND METHODS: A population-based survey was conducted in the Netherlands. Participants aged ≥ 18 years (n = 1524) responded to CT-scan vignettes describing pre-procedural potential diagnostic errors, varying by patient age (9, 26, 46, or 71 years

Legal and ethical responsibility for AI-assisted diagnostic and referral errors: a scoping review of comparative approaches with implications for Saudi healthcare.

BACKGROUND: Artificial intelligence (AI) systems are increasingly embedded in clinical diagnostics and referral pathways, yet when these tools contribute to patient harm, traditional medico-legal doctrines are strained by algorithmic opacity, automation bias, and distributed decision-making. Beyond legal uncertainty, AI integration raises fundamental ethical concerns regarding clinician moral agency, patient autonomy, and distributive justice. This scoping review maps how jurisdictions worldwide allocate legal and ethical responsibility for AI-assisted diagnostic and referral errors, identifie

Informing Measurement of Timely Follow-up Colonoscopies After Positive Stool Tests for Two-Step Colorectal Cancer Screening: An Environmental Scan.

BACKGROUND AND OBJECTIVE: Delays in the completion of two-step colorectal cancer (CRC) screening, consisting of follow-up colonoscopy after positive non-invasive testing, increase the risk of advanced-stage CRC diagnoses and poor health outcomes. This environmental scan aimed to inform the measurement of timely follow-up colonoscopy after positive stool tests. METHODS: Two main data sources, UpToDate and PubMed, were used to identify society screening guidelines and peer-reviewed literature, respectively. Four websites and two databases were searched for related clinical quality measures. Add

The Clinical Pathway of the Normal Outpatient Modified Barium Swallow: A Case Series.

OBJECTIVE: Modified barium swallow studies (MBSS) are one of two gold-standard assessments for oropharyngeal swallowing. It is unclear how many patients referred for outpatient MBSS have normal swallowing despite a subjective complaint of dysphagia. This study examined the rate, referral diagnoses, and referral patterns of outpatient MBSS. METHODS: A retrospective descriptive case series was conducted of outpatient adult MBSS between January 1, 2019 and December 31, 2019. Normal MBSS were identified and stratified by present/absent history of neurological disease or head and neck cancer (HNC)

FairFlow: A transparency-first framework for verifiable and reproducible bioinformatics.

Bioinformatics analyses depend on external software that itself relies on further programs, or dependencies, each with its own release cycle. Because tools are updated to fix bugs and add features, identical code can produce different results depending on which versions are installed, and incomplete documentation of dependencies and versions therefore undermines reproducibility. Containerization packages software with all its dependencies, but demands technical expertise and lacks a standard operating procedure. Existing workflow managers and container tools have improved reproducibility, yet

Treatment outcomes of an integrated treatment model for patients with co-occurring opioid use and schizophrenia spectrum disorders.

BACKGROUND: Individuals with schizophrenia spectrum disorders are at elevated risk for developing opioid use disorder and often experience higher substance use rates and morbidity. However, integrated treatment models for such patients remain rare. OBJECTIVE: This study addresses a gap in clinical knowledge by evaluating outcomes from a dual-diagnosis program tailored for patients with both disorders. METHODS: This retrospective case series analyzed electronic medical record data from when patients were enrolled in the program through March 1st, 2025 or program discharge. RESULTS: Seventeen

Noninvasive Positive Pressure Ventilation Use in the Prehospital Setting: A Preliminary Cross-Sectional Analysis of State Protocols and Nationwide Clinical Practice.

BACKGROUND: Prehospital respiratory distress is a common and high-risk condition that often requires critical interventions, including noninvasive positive pressure ventilation (NIPPV). OBJECTIVE: We aimed to describe U.S. emergency medical services (EMS) state protocols and nationwide practice patterns related to the prehospital use of NIPPV. METHODS: We completed a cross-sectional analysis of all publicly available, active state-level EMS protocols in the United States related to the prehospital use of NIPPV. Second, we completed a cross-sectional analysis of nationwide EMS practice patter

Streamlining Care: Effective Approaches to Decreasing Observation Status Length of Stay in Community Medical Centers.

INTRODUCTION: To identify areas of opportunity and gaps and develop and test strategies to improve patient treatment in the observation units of two regional community medical centers. METHODS: The Plan-Do-Study-Act (PDSA) approach identified the two common emergency department complaints (syncope and collapse; unspecified atrial fibrillation) with the longest patient length of stay (LOS) in observation. Fishbone exercises elucidated these gaps. Departmental meetings, group discussions, and 1:1 discussion of particular cases provided education to reduce LOS. RESULTS: Plan-Do-Study-Act target

Northwest Axis on Electrocardiogram: Decoding an Unusual Cardiac Axis in a Young Patient.

BACKGROUND: We describe a patient with a "northwest" cardiac axis and electrocardiographic features of right ventricular (RV) pressure overload in the setting of Fontan circulation. TAKE-HOME MESSAGES: In atrioventricular canal defects with Fontan circulation, a northwest cardiac axis on electrocardiography reflects altered RV activation and remodeling, often persisting from childhood into adulthood and secondary to RV outflow tract obstruction. CASE SUMMARY: A 28-year-old woman presented for routine medical follow-up. Medical history was notable for Fontan procedure, with the last operation

A real-world analysis of FDA Adverse Event Reporting System (FAERS) events for liposomal nanoparticle-formulated and conventional anticancer irinotecan.

AIMS: Despite the established use of both liposomal irinotecan and conventional irinotecan, a comparative analysis of their safety profiles is lacking. This study leverages the FDA Adverse Event Reporting System (FAERS) database to fill this gap by comparing their adverse events (AEs) to inform clinical practice. METHODS: AEs were categorized by System Organ Class (SOC) and described using Preferred Terms (PTs) from the Medical Dictionary for Regulatory Activities (MedDRA, v26.1). The signals for liposomal irinotecan and irinotecan were quantified through disproportionality analysis, employin

Public preferences for biopsy risk disclosure in the Netherlands.

PURPOSE: To assess general population perspectives on pre-biopsy disclosure of serious risks and factors associated with a preference for disclosure. METHODS: 2,407 adults from the Dutch general population completed 3,565 case vignettes describing hypothetical patients requiring biopsies, each reporting the risk of serious complications (requiring additional treatment or prolonged hospitalization, or resulting in death). Vignettes varied by patient age (9, 26, 46, or 71 years), probability of serious complications (<0.1%, <1%, <5%, or unspecified), and biopsy purpose (primary diagnosis, stagi

Device malfunctions and patient harm in robotic-assisted prostatectomy: a 17-year review of the FDA MAUDE database.

Robot-assisted radical prostatectomy (RARP) is the predominant surgical treatment for localized prostate cancer. However, postmarket surveillance of device-related adverse events is critical, as robotic systems entered the market through pathways requiring less extensive premarket clinical testing. This study analyzes device malfunctions and patient harm associated with RARP reported to the FDA’s MAUDE database over a 17-year period. We conducted a retrospective analysis of the MAUDE database from 2009 to March 2026. All reports related to prostatectomy using the Da Vinci surgical system were

Evidence-based behaviour change techniques to aid smoking cessation for pregnant women with mental illness: a systematic review.

BACKGROUND: Smoking during pregnancy is the leading preventable cause of maternal and infant morbidity and mortality in high income countries, disproportionately affecting women with mental illness. While behavioural smoking cessation interventions are effective for pregnant women and non-pregnant people with mental illness separately, evidence for specific smoking cessation interventions for pregnant women with mental illness is lacking. This review aimed to identify and map BCTs from effective general mental health smoking cessation trials to determine which may be helpful for pregnant women
