Clinical and Functional Outcomes of Closed Intramedullary Interlocking Nailing in Extra-articular Distal Tibial Fractures: A Prospective Observational Study.

INTRODUCTION: Extra-articular distal tibial fractures are difficult to manage because of poor soft-tissue coverage, limited vascularity, and the tendency for malalignment and delayed union. Closed intramedullary interlocking nailing has become an increasingly preferred treatment option because it preserves fracture hematoma and periosteal blood supply. AIMS: The aim of the study was to evaluate the clinical and functional outcomes of closed intramedullary interlocking nailing in extra-articular distal tibial fractures. MATERIALS AND METHODS: This prospective observational, single-center stud

Posterior Hip Dislocation with Ipsilateral Subtrochanteric Femur Fracture: A Rare Case Report.

INTRODUCTION: Posterior dislocation of the hip associated with an ipsilateral subtrochanteric femur fracture is a rare and technically challenging injury pattern, most commonly seen after high-energy trauma such as road traffic accidents. The associated proximal femoral fracture disrupts the normal femoral lever arm, making closed reduction difficult or impossible and often necessitating open reduction. Delay in diagnosis or treatment may result in complications such as avascular necrosis of the femoral head, post-traumatic arthritis, sciatic nerve injury, and poor functional outcomes. CASE R

Delayed Complication of Near-Drowning Following a Car Crash: Spondylodiscitis by Scedosporium apiospermum: A Case Report.

CASE: A 20-year-old immunocompetent man developed spondylodiscitis 2 months after near-drowning with vehicular submersion. Scedosporium species identified in sputum during admission was initially considered colonization. Diagnosis was eventually confirmed through biopsy. Treatment with voriconazole was discontinued prematurely at 6 weeks due to an allergic reaction, but clinical recovery was favorable at 1-year follow-up. CONCLUSION: Approximately 15 cases of Scedosporium apiospermum spondylodiscitis have been reported, with a minority occurring in immunocompetent patients following near-drow

Complete laryngotracheal separation following blunt neck trauma: A forensic reconstruction of an unusual fatal injury.

INTRODUCTION: Complete laryngotracheal separation (CLTS) is an exceptionally rare and life-threatening consequence of blunt neck trauma. Because external cervical injuries may be minimal or nonspecific, diagnosis can be delayed in both clinical and forensic settings. CASE PRESENTATION: A 35-year-old man was found unconscious at the roadside and died two hours after hospital admission despite emergency management. Autopsy revealed extensive cervical subcutaneous emphysema, complete laryngotracheal separation, bilateral thyroid cartilage fractures, cricoid cartilage disruption, destruction of t

Outcomes of Primary Definitive Internal Fixation of Compound Fractures within 72 h of Injury: A Prospective Cohort Study.

INTRODUCTION: Askin tumor is a rare primitive neuroectodermal tumor originating in the thoracopulmonary region. It belongs to the Ewing sarcoma family and is known for its aggressive nature, typically arising from the chest wall. Ewing sarcoma is the second most common primary malignant bone cancer in children and adolescents, which arises from the neuroectoderm of bone and extra-skeletal soft tissue. Its characteristic chromosomal translocation between chromosomes 11 and 22 causes the EWS-FLI protein to develop in about 80-90% of patients, which might be helpful in targeted therapy in the fut

Trapped and traumatised: a scoping review of the psychological sequelae of entrapment following motor vehicle collision.

BACKGROUND: Motor vehicle collisions (MVCs) are a significant public health challenge, resulting in substantial mortality and morbidity. Entrapment occurs in 12-33% of all MVCs and is associated with greater injury severity, longer on-scene time, and increased mortality. However, the psychological consequences of entrapment remain poorly characterised. OBJECTIVES: This scoping review seeks to establish the current understanding of the psychological sequelae of entrapment following MVCs and draws on insights from related fields of study in order to inform future research and clinical practice.

Treatment outcomes and associated factors among road traffic injury patients in emergency departments of public hospitals in Awi Zone Northwest Ethiopia.

Globally, road traffic injuries (RTIs) cause numerous tragedies such as serious economic loss to the community and death of young people. In Ethiopia, a large proportion of serious injuries result from RTIs and have become major causes of death in hospitals. However, there is insufficient research conducted on treatment outcomes of road traffic injuries and associated factors in the study area. The primary aim of this study was to determine the magnitude of poor treatment outcomes and identify associated factors among patients in the emergency departments of public hospitals in Awi Zone, North

Addressing diagnostic challenges in neglected Bosworth dislocation with open reduction and soft tissue reconstruction: a case report.

INTRODUCTION AND IMPORTANCE: Bosworth fracture-dislocation is a rare and complex ankle injury characterized by the distal fibular fragment becoming entrapped behind the tibia, often following an oblique fibular fracture. Due to its unique anatomical presentation, the injury is frequently misdiagnosed or overlooked, particularly in chronic or neglected cases, leading to long-term complications. This case report explores a novel surgical technique for managing chronic Bosworth fracture-dislocation. CASE PRESENTATION: An 18-year-old woman presented with persistent pain and difficulty bearing wei

Global Perspectives in Plastic Surgery: View of Plastic Surgeons From Uganda.

BACKGROUND: Uganda, an East African country with a population of approximately 46 million, has 72.62% of its people living in rural areas. Plastic and reconstructive surgery was historically delivered by visiting surgical teams, which, although impactful, faced limitations in follow-up and patient reach. Uganda has only 22 trained local plastic surgeons, mostly in urban private practice. Few work in public hospitals, limiting access to reconstructive care for the country's largely rural population. METHODS: A cross-sectional survey of 35 participants (plastic surgeons and trainees) was conduc

[Calcaneal fractures-Current status and treatment approaches].

Calcaneal fractures primarily affect patients aged 40-59 years, with an incidence rate of 17.5 per 100,000 inhabitants. They are typically caused by axial forces resulting from falls from a height or traffic accidents. In 75% of cases the posterior facet of the subtalar joint is affected, which can result in substantial functional limitations. The aim of treatment is to anatomically reconstruct the joint surfaces and the shape of the foot and to prevent long-term damage, such as osteoarthritis. Surgical treatment typically involves open reduction and internal fixation (ORIF), although minimall

Covert hepatic encephalopathy in cirrhosis: implications for early diagnosis and appropriate management.

Covert hepatic encephalopathy (CHE) is a frequent and clinically relevant complication of liver cirrhosis, affecting approximately 30-70% of patients. Despite the absence of overt neurological symptoms, CHE is associated with impaired quality of life and increased risks of falls, traffic accidents, hospitalization, progression to overt HE (OHE), and mortality. The pathophysiology of HE, including CHE and OHE, is multifactorial and involves complex interactions among hyperammonemia, systemic inflammation, oxidative stress, gut dysbiosis, bile acid dysregulation, and sarcopenia along the gut-liv

Preserving the Bone Envelope: A Prospective Observational Study of Closed Reduction and Ilizarov Circular Fixation for Open and Closed Distal Tibial Fractures.

Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

A Comparative Study of Functional Outcomes in Anterior and Posterior Wall Acetabular Fractures Managed Using Modified Stoppa and Kocher-Langenbeck Approaches at a Tertiary Hospital in India.

Background Surgical management of acetabular fractures remains technically demanding, particularly in resource-limited settings. This study evaluates the short-term functional and radiological outcomes of elementary acetabular fractures treated with approach-specific techniques. Methods A prospective study was conducted on 15 patients with isolated anterior or posterior wall acetabular fractures. Patients with anterior wall fractures (n = 8) underwent fixation via the modified Stoppa approach, while those with posterior wall fractures (n = 7) were treated using the Kocher-Langenbeck approach.

Obturator dislocation: the forgotten hip dislocation.

PURPOSE: Obturator dislocations of the hip are a rare subtype of anterior hip dislocation with poorly described reduction techniques. They are associated with high energy trauma and prompt; effective closed reduction is often vital to the patient’s wellbeing. Delayed reduction can lead to high rates of avascular necrosis of the femoral head and failed closed reduction can mean operative intervention is required. METHODS: We propose a new technique utilising inline traction, abduction and external rotation as well as direct lateral pressure on the femoral head to reduce the dislocation. We pres
