Pheochromocytoma Across the Perioperative Course: Current Concepts and Clinical Management.

Pheochromocytoma is an uncommon neuroendocrine tumor with myriad nonspecific clinical presentations. If not recognized and treated, it is associated with the development of target organ damage and complications such as myocarditis, cardiomyopathy, cerebrovascular accidents, and diabetes mellitus, among numerous others. The pheochromocytoma diagnosis is based on biochemical proof of catecholamine excess followed by imaging localization of the tumor. Patients with newly diagnosed pheochromocytoma should be seen by a multidisciplinary perioperative team, including the anesthesiologist, operating

Retrograde Cerebral Perfusion in Type A Aortic Dissection Repair Is Associated With Higher Rates of Postoperative Complications.

Background The comparative short- and long-term outcomes of retrograde versus antegrade cerebral perfusion during circulatory arrest for acute type A aortic dissection repair remain debated. Methods In this single-center retrospective study, postoperative outcomes were compared between retrograde and antegrade cerebral perfusion. Retrograde cerebral perfusion was employed in older patients (median age = 72 vs. 63 years; p = 0.030), with comparable body mass index (29 vs. 28 kg/m²; p = 0.649). Results Relative to antegrade cerebral perfusion, retrograde cerebral perfusion was associated with in

Fatal Administration of Propofol: A Comprehensive Narrative Forensic Review of Propofol Deaths From Accidents, Suicides, and Homicides.

Accidental death, suicide, and homicide have resulted from the intravenous injection of propofol. There have been at least 30 accidental victims of propofol self-administration; more than 75% were medical workers who chronically used it as a recreational drug. At least 13 of the 14 individuals who committed suicide by injecting themselves with propofol were either a physician or a nurse. A fatal injection of propofol was used to murder a minimum of 10 people; for many of these deceased persons, neither the drug nor homicide was initially suspected at the time of death. Therefore, the discovery

Differences in gait and prefrontal cortical activity between recurrent fallers and non-fallers with Parkinson's disease during single- and dual-task walking.

BACKGROUND: Falls are common in people with Parkinson's disease (PD) and are associated with gait impairments, particularly under dual-task conditions. Although gait characteristics differ between fallers and non-fallers, it remains unclear whether walking-related prefrontal cortex (PFC) activity also differs according to fall history. This study compared PFC activity and gait parameters between recurrent fallers (≥2 falls/year) and non-fallers with PD during single- and dual-task walking. METHODS: Participants with PD were classified as non-fallers (n = 34) or recurrent fallers (n = 18). The

An audit of the fracture liaison service at Helen Joseph Tertiary Hospital, Johannesburg, South Africa.

UNLABELLED: Fragility fractures are increasing worldwide, yet osteoporosis often remains underdiagnosed and untreated. This study found a high burden of osteoporosis risk factors among patients enrolled in a South African fracture liaison service, highlighting the need for structured secondary prevention. These findings support expansion of fracture liaison services to improve long-term patient outcomes in resource-limited settings. BACKGROUND: Fragility fractures (FF) driven by osteoporosis result in high morbidity and mortality and are a global health concern. Fracture liaison services (FLS

Preserving the Bone Envelope: A Prospective Observational Study of Closed Reduction and Ilizarov Circular Fixation for Open and Closed Distal Tibial Fractures.

Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

Cellular, Molecular, and Behavioural Sequelae of Early-Life Continuous Low-Dose-Rate Irradiation in Mice.

The Fukushima nuclear accident highlighted that evacuation-related psychosocial harm can outweigh direct radiation risks, underscoring the need to define the health impacts of chronic low-dose-rate (LDR) radiation and evidence-based thresholds for intervention. This study investigated the effects of continuous, postnatal LDR gamma irradiation (1.2 mGy/h, cumulative dose: 5 Gy) in male mice. While no changes in body weight, hippocampal neurogenesis, or major glial and neuronal populations were observed, persistent DNA damage (γ-H2AX foci) in dentate gyrus granule cells occurred in both irradiat

A Comparative Study of Functional Outcomes in Anterior and Posterior Wall Acetabular Fractures Managed Using Modified Stoppa and Kocher-Langenbeck Approaches at a Tertiary Hospital in India.

Background Surgical management of acetabular fractures remains technically demanding, particularly in resource-limited settings. This study evaluates the short-term functional and radiological outcomes of elementary acetabular fractures treated with approach-specific techniques. Methods A prospective study was conducted on 15 patients with isolated anterior or posterior wall acetabular fractures. Patients with anterior wall fractures (n = 8) underwent fixation via the modified Stoppa approach, while those with posterior wall fractures (n = 7) were treated using the Kocher-Langenbeck approach.

The relationship between gait asymmetry and stability in people with sub-acute stroke.

BACKGROUND: People often walk with temporal asymmetry post-stroke; which is related to increased fall risk. The purpose of this study was to determine the relationship between temporal gait asymmetry and mechanical stability among people with sub-acute stroke. METHODS: Thirty-one people with sub-acute stroke (<6 months post-stroke) completed six walking trials. Three-dimensional motion capture was recorded. Swing symmetry was calculated as a ratio of swing time on the more affected limb divided by swing time on the less affected limb. Mechanical stability was the minimum margin of stability,

Association of intradialytic hypotension and physical performance with fall and fracture incidence in hemodialysis patients: The REPnet-HD study.

Although hypotension has been linked to fall-related injuries in older adults, evidence in hemodialysis (HD) patients remains limited. This study examined the effects of intradialytic hypotension (IDH) and physical performance on the risk of subsequent falls and fractures. In this prospective cohort study, 518 ambulatory HD patients were observed over 1 year to track the occurrence of accidental falls, severe falls (those requiring medical intervention), and fractures. Baseline assessments included IDH (defined as nadir systolic blood pressure [BP] during dialysis <90 mmHg or intervention for

Conservative Management of Subluxation Injuries in Young Permanent Teeth Using Fiber-Reinforced Composite Splints: Review and Case Reports.

Subluxation of young permanent teeth poses a significant clinical challenge due to its potential impact on pulp vitality, periodontal healing, and esthetics. Luxation injuries in anterior teeth are more common in children and adolescents, often resulting from sports injuries, falls, or accidents. Management of these injuries requires accurate diagnosis, prompt repositioning, and effective stabilization. Fiber-reinforced composite (FRC) splints, being flexible and esthetic, have emerged as a preferred option in anterior teeth stabilization in pediatric and adolescent patients. This case series

Optimal Reactive Balance Training Characteristics Poststroke: Secondary Analysis of a Clinical Trial.

BACKGROUND AND PURPOSE: Reactive balance training (RBT) has shown promise for enhancing reactive balance control and reducing falls poststroke. However, the optimal training parameters (e.g., intensity, duration) are unknown. This secondary analysis examined dose-response relationships between specific reactive balance training characteristics and improvements in reactive balance control and fall rates poststroke. METHODS: 30 people with chronic stroke (mean age: 66.6 ± 8.8 years; time after stroke: 3.3 ± 2.3 years) completed up to 12 one-hour reactive balance training sessions, twice per wee

Submental Intubation in Panfacial Trauma: A Pictorial Demonstration With Anesthetic Considerations.

Airway management in patients with panfacial trauma is particularly challenging when both nasal and oral routes are unsuitable. Submental intubation serves as an effective alternative to tracheostomy in such scenarios. We report the case of a 30-year-old male who presented with mandibular and nasal bone fractures following a road traffic accident. Limited mouth opening and the need for intraoperative maxillomandibular fixation made conventional airway approaches unsuitable. Submental intubation was performed after initial oral intubation to facilitate surgical access. The procedure was complet

Minimum Foot Clearance Prediction in Stroke Survivors: A Transformer-Based Approach.

A decrease in Minimum Foot Clearance (MFC), which represents the minimum vertical distance of the foot from the ground surface during the swing phase of the gait cycle, is one of the primary contributors to tripping-related falls among people with stroke. Accurate prediction of upcoming MFC values, understanding their potential range, and evaluating whether future values will fall within the variability of actual MFC values are crucial for early fall risk assessment. This work proposes a new Transformer model for predicting multistep MFC values in stroke survivors collected from the affected l

"Epileptics as drivers"- the first report on motor vehicle accidents due to epileptic seizures in 1906.

OBJECTIVE: The 1906 report of Franz Thalwitzer on motor vehicle accidents (MVAs) in two people with epilepsy has been frequently cited as the first report to provide justification for legal regulations to prohibit people with epilepsy from driving. METHODS: On the occasion of the 120th anniversary of the Thalwitzer report, we analyzed the clinical findings and the circumstances of the accidents outlined in the text to clarify whether the events described were definitive epileptic seizures. RESULTS: Both people had a history of epilepsy. Case 1, who drove a car, had been treated with bromides

A machine learning approach to quantifying fall conversion risk in fall-naïve Parkinson's patients.

BACKGROUND: Recurrent falls associated with Parkinson's disease (PD) contribute to diminished quality of life and treatment expense. Current fall prediction models do not adequately predict the transition from non-faller to recurrent faller in people with Parkinson's disease (PwPD). The aim of this project was to develop a machine learning model to identify fall risk to characterize the transition to a recurrent faller. METHODS: Baseline clinical motor, biomechanical, cognitive, and quality of life data from 246 PwPD in a clinical trial were combined with subsequent falls reported in diaries

Age Is Not an Indication: Antibiotic Agents for Urinary Tract Infections in Patients with Geriatric Trauma.

BACKGROUND: Geriatric patients injured in traumatic falls may present with either baseline or acquired cognitive impairment, preventing them from describing genitourinary symptoms. Thus, they are frequently given a screening urinalysis (UA) for possible urinary tract infection (UTI). However, according to the Infectious Diseases Society of America (IDSA), patients without systemic signs of infection should be assessed for other causes of falls and instead closely monitored. We conducted a study in our trauma system to assess UTI screening and treatment. We hypothesized that geriatric falls are
