Public safety (prevention of accidents) is the primary objective in assessing fitness to drive a motor vehicle; however, the general access to mobility should not be restricted if there is no particular risk to public safety. For people with diabetes mellitus, the Driving Licence Legislation (FSG) and the Driving Licence Legislation Health Act (FSG-GV) regulate important aspects of driving safety in connection with acute and chronic complications of the disease. Critical complications that may be relevant to road safety include severe hypoglycemia, pronounced hyperglycemia and hypoglycemia per

PURPOSE: Using quality improvement (QI) processes, we sought to safely achieve >80% efficiency in same-day discharge (SDD) of eligible primary total hip arthroplasty (THA) patients.
METHODS: A "Plan-Do-Study-Act" approach was used. QI elements included: research of prior relevant publications, clinical audit for data-driven evaluation, service evaluation assessments and clinical transformation on a small scale for piloting change. Care interventions were introduced within an already established highly iterative and enhanced recovery after surgery-based programme. Intervention 1 established pa

Bilateral zygomatic arch fractures are uncommon but may result in significant midfacial deformity and functional impairment, particularly when fractures are comminuted and structural continuity is compromised. This report describes the surgical management of a 49-year-old male who sustained severe facial trauma following a mountain-climbing accident, resulting in comminuted bilateral zygomatic arch fractures and marked midfacial disfigurement. Due to the complexity of the injury and loss of structural support, autogenous costal cartilage grafting combined with titanium mesh fixation was select

OBJECTIVES: Falls cause more than 85% of hip fractures in older adults. Older adults with cognitive impairment have been largely excluded from research on people who have sustained a hip fracture. As a consequence, whether exercise reduces fall risk in this high-risk population is unknown. We address this gap by examining the effect of a home-based exercise program on fall risk in older adults with mild cognitive impairment after hip fracture.
METHODS: This was a 6-month randomized controlled trial comparing the effect of the Otago Exercise Program (OEP) versus standard care (SC) on fall risk

INTRODUCTION: Persistent urinary extravasation after high-grade renal trauma is uncommon but may require intervention. Minimally invasive, kidney-preserving options are particularly desirable in young patients.
CASE PRESENTATION: An 18-year-old man sustained blunt trauma in a motorcycle accident. Contrast-enhanced computed tomography revealed renal injury with urinary extravasation. Emergency transcatheter arterial embolization was performed for hemorrhage control. Despite subsequent ureteral stenting and percutaneous drainage, urinary leakage persisted. Fistulography demonstrated a small fis

The optimal anesthetic technique during endovascular thrombectomy (EVT) for acute ischemic stroke (AIS) remains uncertain. General anesthesia (GA) provides airway protection and procedural stability, whereas conscious sedation (CS) offers faster workflow and the ability to monitor neurological status. This updated systematic review and meta-analysis (SRMA) aims to summarize and update the existing literature from randomized controlled trials (RCTs) to guide the selection of the most appropriate anesthetic technique during EVT for AIS. We searched the electronic databases of PubMed, ProQuest, a

INTRODUCTION: Endometriosis is a chronic, estrogen-dependent gynecological disorder affecting approximately 10% of women worldwide that can cause pelvic pain, lead to infertility, and impair quality of life. Whether the condition elevates risk of mortality remains controversial. This systematic literature review and meta-analysis investigated the long-term survival of women with endometriosis.
METHOD: Studies reporting mortality among women with endometriosis were identified through PubMed, EMBASE, Web of Science, Scopus, and OpenGrey in accordance with PRISMA recommendations. Pooled data on

BACKGROUND: Postprandial hypotension (PPH) may contribute to falls among older adults, particularly those taking antihypertensive medication. However, evidence on this association in community-dwelling populations is limited. Since ambulatory blood pressure (BP) monitoring captures BP during daily activities, it may provide accurate assessments of PPH outside the clinic setting.
METHODS: This prospective cohort study examined the association between PPH and fall risk among community-dwelling adults aged ≥65 years taking antihypertensive medication. At baseline, participants underwent 24-hour

BACKGROUND: Sex-related differences in Parkinson's disease (PD) have been frequently reported, yet most studies are descriptive and do not account for confounders. Identifying independent predictors may clarify sex-linked features and guide management.
OBJECTIVES: To determine independent predictors of sex differences in a well-characterized PD cohort through multivariate analysis encompassing motor and non-motor domains.
METHODS: A total of 652 PD patients (253 women, 392 men) from a tertiary movement disorders clinic were retrospectively evaluated. Assessments included MDS-UPDRS Parts I -

Ambulatory anesthesia has grown rapidly, encompassing more complex procedures and higher-risk patients. Analysis of closed claims reveals that while major complications and mortality remain rare, moderate injuries such as dental trauma, nerve damage, and communication-related issues are frequent and remained largely stable in frequency over the past decade. Patient comorbidities-including obesity, obstructive sleep apnea, and diabetes-significantly increase perioperative risks. Improving patient safety in this setting depends on careful risk assessment, multimodal analgesia, opioid-sparing tec

Induction of general anesthesia in the sitting position may be associated with hemodynamic instability resulting from decreased venous return, as well as challenges in airway management due to limited access to the airway. However, in patients who cannot tolerate the supine position because of post-traumatic pain or other conditions, induction in the sitting position may be unavoidable. In this report, we describe a case in which sedation was initiated in the sitting position, spontaneous respiration was preserved during the transition to the supine position, and tracheal intubation was subseq

INTRODUCTION: Chronic kidney disease (CKD) frequently coexists with cardiovascular disease (CVD), diabetes, and obesity through shared biological pathways encompassed within the American Heart Association cardiovascular-kidney-metabolic (CKM) construct. Whether adverse cardiovascular and renal outcomes in CKD are driven predominantly by cumulative multimorbidity burden or by distinct CKM disease phenotypes remains uncertain.
METHODS: We conducted a retrospective longitudinal cohort study of participants in the Salford Kidney Study with non-dialysis-dependent CKD and at least one other CKM com

OBJECTIVE: To improve treatment outcomes in patients with blunt liver injuries using modern protocols and minimally invasive technologies in a multi-field hospital.
MATERIAL AND METHODS: Treatment outcomes were analyzed in 93 patients with blunt traumatic liver injury between 2019 and 2024. There were 74 (79.6%) men and 19 (20.4%) women. Mean age was 42.8±11.3 years; 87 (93.5%) patients were delivered by ambulance, and 6 (6.5%) ones have gone by themselves. Of these, 36 (38.7%) patients presented within the first hour after injury, 49 (52.7%) - within 6 hours, 8 (8.6%) - later. Fifty-four (58

RATIONALE: Cerebral fat embolism (CFE) is a rare but serious complication of fractures, most commonly affecting young males. However, atypical cases in elderly patients with underlying comorbidities are easily misdiagnosed.
PATIENT CONCERNS: We report a case of a 61-year-old male patient with diabetes and hypertension who developed impaired consciousness and fever on the 2nd day after sustaining multiple fractures in a traffic accident. His condition deteriorated after receiving local treatment.
DIAGNOSES: CFE was diagnosed based on the characteristic "starry sky sign" on brain magnetic reso

Diabetes mellitus remains a major global health burden, particularly affecting wound healing in patients with traumatic injuries. Chronic hyperglycemia impairs tissue regeneration and immune function, increasing the risk of infection, delayed recovery, and potential limb loss. We aim to demonstrate the critical importance of glycemic control and multidisciplinary management in enhancing wound healing outcomes among diabetic patients undergoing surgical interventions. using the case of a 65-year-old male with a crush injury to the left foot following a road traffic accident. Patients suffering from traumatic spinal cord injury (SCI) constitute a unique patient population in critical care medicine due to the high rate of mortality, irrecoverable neurological damage, and dependence on advanced life support measures. In this report, we present a patient who sustained a non-survivable cervicomedullary injury due to motor vehicle collision who became dependent on maximal life support and subsequently expired from multiple complications 11 months after the initial accident. On neuropathological exam, extensive Wallerian degeneration in the spinal cord despite non-trauma

We hypothesised that a patient-driven safety checklist would enhance patients' health literacy. This study was conducted as a sub-study of a multicentre cluster trial with a stepped wedge design. Data were collected between March 2022 and February 2024. Healthcare personnel were partially blinded to group allocation and fully blinded to the health literacy questionnaire outcome. The study included seven surgical specialties (clusters) from a tertiary teaching hospital and two community hospitals in Norway. The patients were included from a pool within the cluster trial. BACKGROUND: Traumatic posterior atlantoaxial dislocation (TPAD) with an associated odontoid fracture is exceedingly rare, typically resulting from high-energy trauma such as high-velocity road traffic accidents or falls from height. This report details a previously undescribed case of an old TPAD combined with a Type II odontoid fracture and bilateral locked facets in a patient who remained neurologically intact. We also systematically review the literature on the diagnosis and management of this injury pattern to provide evidence-based guidance for clinical decisions.
