Multidimensional Characteristics of Musculoskeletal Pain and Risk of Falls Among Elderly Adults: The Cross-Sectional Evidence From CHARLS.

OBJECTIVE: Falls are common particularly in elderly people and usually associated with poor prognosis or healing outcomes. Musculoskeletal pain is considered an independent risk factor for falls, by being associated with joint pathology and instability, neuromuscular effects of pain, and changes in cognitive and psychological function. Understanding the relationship between musculoskeletal pain and the risk of falls will help with falls prevention among older adults. METHODS: A nationally representative sample for the Chinese population was derived from the China Health and Retirement Longitu

Association between frailty and fall risk in elderly patients with knee osteoarthritis in community: a propensity score matching analysis.

To analyze the prevalence of frailty in community-dwelling older adults with knee osteoarthritis (KOA) and explore its independent association with fall risk. A cross-sectional study was conducted among older adults with KOA in Zunyi, China, from October 2022 to September 2023 using convenience sampling. Data were collected on sociodemographic characteristics, instrumental activities of daily living (IADL), depression, anxiety, sleep quality, frailty, and fall risk. Propensity score matching (PSM) was performed to match frail and non-frail participants at a 1:1 ratio using nearest-neighbor mat

Radiographic Sagittal Alignment and Neurological Changes Following Conservative Cervical Structural Rehabilitation After Motor Vehicle Collision in a Patient With Pre-existing Scoliosis: A Case Report.

Motor vehicle collisions (MVCs) are a frequent cause of cervical spine injury and may result in persistent neurological and structural sequelae in a subset of patients. Many individuals may improve with standard active management. Complex presentations involving sagittal malalignment and suspected segmental instability remain clinically challenging. The relationship between cervical alignment, dynamic motion behavior, and neurological status continues to be explored within contemporary spinal biomechanics literature. This case report describes a 28-year-old woman who presented with persistent

Rockwood Type VIb Acromioclavicular Joint Dislocation With Ipsilateral Proximal Humerus Fracture-Dislocation and Scapular Spine Fracture: A Case Report.

Rockwood type VI acromioclavicular (AC) joint dislocations are extremely rare high-energy injuries characterized by inferior displacement of the distal clavicle and are often associated with complex shoulder girdle injury. We report the case of a 36-year-old male who sustained a Gustilo-Anderson type IIIA open Rockwood type VIb AC joint dislocation associated with a Neer two-part proximal humerus fracture-dislocation and an ipsilateral scapular spine fracture following a road traffic accident. The patient underwent early surgical debridement and single-stage operative management, including ope

Multiple Second to Fifth Carpometacarpal Fracture-Dislocations: A Case Report on the Surgical Management of a Rare Hand Injury.

Multiple carpometacarpal (CMC) joint fracture dislocations involving the second to fifth rays are rare injuries, typically resulting from high-energy trauma. Due to the complex anatomy of the CMC joints and associated swelling, these lesions are frequently overlooked, leading to delayed diagnosis and suboptimal outcomes. We report the case of a 21-year-old male patient who sustained simultaneous dislocations of the second through fifth carpometacarpal joints with associated fractures following a high-energy trauma to the hand due to a motorcycle accident. Initial radiographic evaluation confir

When Life-Threatening Injuries Overshadow Soft Tissue Trauma: A Missed Morel-Lavallée Lesion.

Morel-Lavallée lesions are closed, shearing injuries that occur in polytrauma patients, most commonly affecting the thigh, hip, and pelvis due to mobile skin over thick fascia. These lesions are often missed upon initial evaluation due to the severity of accompanying traumatic injuries and the focus on life-threatening conditions. We present a case of a missed Morel-Lavallée lesion in a Level 1 trauma patient and our innovative approach to treatment. A 36-year-old female patient was air-transported to the emergency department after a motorcycle accident, where she was struck by a vehicle. She

Clinical Insights Into Bilateral Mandibular Angle Fractures: Our Experience.

Bilateral mandibular angle fractures are uncommon but present a significant challenge in terms of management for maxillofacial surgeons. We present a case of an adolescent male who presented to the emergency department after a road traffic accident with isolated bilateral mandibular angle fractures, with a significant displacement on the left side. The patient was treated using rigid fixation for the more displaced left angle and non-rigid fixation for the right side, which led to a stable reduction without complications.

An Investigation of the Etiology and Pattern of Spinal Injuries Among Patients Presenting to the Emergency Department in a Tertiary Care Hospital.

Background Spinal cord injuries contribute substantially to morbidity and mortality globally. There is a gross disparity in the causes of spinal injuries between developed and developing countries. Thus, understanding the etiology and pattern of spinal injury in our setting is important to plan strategies for effective management and prevention strategies at the national level. Methodology A prospective observational study was conducted among 75 patients with spinal cord injury visiting the Department of Emergency Medicine of a tertiary care hospital. The study aimed to investigate the etiolog

Dual Urinary Tract Pathology in a Frail Elderly Man.

A prostatic abscess is a rare but critical complication of urinary tract infections that may coexist with upper urinary tract involvement, such as pyelonephritis. This case report describes an 84-year-old frail gentleman with dementia who was admitted with a fall into the Accident and Emergency Unit. He was noted to be febrile, with suprapubic tenderness and raised infection markers. He underwent a CT trauma series, which revealed right kidney pyelonephritis with a prostatic abscess. A broad-spectrum intravenous antibiotic was started with no improvement, and the patient was not suitable for s

Outcomes following single-level lumbar fusion in patients with myasthenia gravis.

INTRODUCTION: Patients with myasthenia gravis (MG) are at higher risk for postoperative complications after surgery. Knowledge concerning the impact of the condition on outcomes and complications after spine surgery is currently limited. Using a large-scale, national database, the purpose of this investigation was to compare the rate of medical and surgical complications, healthcare utilization, and need for revision surgery following single-level lumbar fusion between patients with and without MG. METHODS: Data was collected using the PearlDiver database and adult patients with and without M

Bladder and Bowel Dysfunction, Cognitive Appraisals of Disability, and Life Satisfaction in Individuals With Pediatric-Onset Spinal Cord Injury.

BACKGROUND: Individuals with pediatric-onset spinal cord injury (SCI) represent a distinct group; management can be more complex than in adult-onset SCI. Changing medical needs, transfer of responsibility, and cognitive appraisals of disability may all impact life satisfaction in pediatric-onset SCI. Further, bladder/bowel management over many years may be time-consuming and socially disruptive. Cognitive appraisal of disability has been linked to life satisfaction and functioning in pediatric-onset SCI. OBJECTIVES: This study examines associations between bladder/bowel accidents, bowel manag

Intracardiac vs. Transesophageal echocardiography in left atrial appendage occlusion: real-world comparative effectiveness from a propensity-matched cohort.

BACKGROUND: Atrial fibrillation (AF) is the most prevalent sustained cardiac arrhythmia and is associated with a markedly increased risk of thromboembolic stroke, primarily originating from the left atrial appendage (LAA). Percutaneous left atrial appendage occlusion (LAAO) is an established alternative for stroke prevention in patients with contraindications to long-term oral anticoagulation. Transesophageal echocardiography (TEE) has traditionally guided LAAO, but intracardiac echocardiography (ICE) is increasingly adopted to facilitate conscious sedation and operator-controlled imaging. Successful Open Surgical Repair of Traumatic Stanford Type B Thoracic Aortic Dissection Following Motorcycle Trauma: A Case Report.

Traumatic thoracic aortic injury represents one of the most life-threatening complications of blunt chest trauma and is associated with substantial morbidity and mortality if not promptly recognized and treated. Rapid deceleration mechanisms may produce intimal disruption of the aortic wall, resulting in dissection, false lumen formation, and potential aortic rupture. We report the case of an 18-year-old previously healthy male patient who presented with chest pain and hemothorax three days after a motorcycle accident. Computed tomography angiography (CTA) demonstrated a Stanford type B trauma

Chronic Hyponatremia Following Mild Traumatic Brain Injury: A Case of Persistent Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) in a Young Adult.

Hyponatremia is a well-recognized complication of traumatic brain injury (TBI), most commonly resulting from syndrome of inappropriate antidiuretic hormone secretion (SIADH) or cerebral salt wasting (CSW) syndrome. While SIADH is frequently described following moderate to severe TBI, persistent SIADH after mild TBI is rare and has been infrequently reported. A 25-year-old man was admitted to Lady Reading Hospital following a road traffic accident resulting in mild TBI. He experienced an uncomplicated neurological recovery, and neuroimaging showed no significant intracranial abnormalities. Duri

Isolated Hepatic Artery Dissection in a Nonsurgical Patient: A Case Report and Literature Review.

Hepatic artery dissection means a tear in the wall of the hepatic artery, allowing blood to accumulate between arterial layers, leading to fatal or major sequelae, including ischemia and hepatic injury, pseudoaneurysm, or rupture. Spontaneous hepatic artery dissection is rare, especially in patients without any recent hepatobiliary surgery or trauma. It is a nonspecific and often subtle presentation that can delay diagnosis, leading to major/fatal consequences, including hepatic injury due to ischemia, pseudoaneurysm, and rupture leading to death, which highlights the need for clinical awarene

Paradoxical Embolic Stroke via Pulmonary Arteriovenous Malformation Complicated by Bubble-Study-Induced Non-ST-Segment Elevation Myocardial Infarction.

Pulmonary arteriovenous malformations (PAVMs) are uncommon vascular anomalies that predispose patients to paradoxical embolic phenomena, including stroke. Contrast echocardiography is a key diagnostic tool, but complications are rarely reported. We describe a 44-year-old woman with no significant past medical history who presented with acute ischemic stroke involving the right superior parietal lobe. Initial workup with echocardiography and bubble study demonstrated right-to-left shunting, considered to be of intracardiac origin, typically a patent foramen ovale. However, this was complicated

Severe paediatric liver injury following a sika deer attack.

A boy in middle childhood was referred to the trauma centre after being attacked by a sika deer. The boy remained in stable clinical condition, complaining of severe abdominal pain. Physical examination showed a skin laceration on the upper abdomen, and abdominal CT scan revealed a major liver laceration with free intraperitoneal fluid. It was uncertain if the trauma mechanism was blunt or penetrating, but an intraperitoneally piercing injury was suspected. A diagnostic laparoscopy confirmed penetration of the peritoneum and a severe liver laceration with concomitant haemoperitoneum, but no ot

Sleep Apnea and Cardiovascular Disease: Risks, Mechanisms, Complications, and Management.

Sleep apnea is a common sleep disorder characterized by recurrent episodes of breathing cessation and resumption during sleep. The three main types are obstructive sleep apnea (OSA), central sleep apnea (CSA), and mixed sleep apnea (MSA), the latter of which is often used interchangeably with complex sleep apnea (CompSA). OSA, the most prevalent form, results from upper airway obstruction during sleep. Globally, nearly one billion adults aged 30-69 are affected by OSA, with approximately 425 million having moderate to severe disease, making sleep apnea a major global health burden affecting ro
