Liver transplantation of patients in end-stage cirrhosis.

The success of liver transplantation has resulted in its widespread use for end-stage liver disease; 1- and 5-year survival rates of 70-90% and 60-80% respectively have been reported. Indications for assessment for liver transplantation are now evidence-based and early referral is recommended, correlating with improved patient survival. The management of patients on the waiting list for liver transplantation is designed to prevent complications of liver disease and to avoid therapeutic misadventures. Following transplantation, rejection and infection dominate post-operative complications, and

Surgical complications of kidney transplantation.

Over the last 30 years, kidney transplantation has evolved tremendously, from an experimental procedure with barely 50% allograft acceptance to a highly refined management program with a success rate of 80-90%. Not only has the overall rate of complications decreased to less than 5%, due to more secure technical approaches, but also advances in immunosuppressive regimens have reduced the morbidity associated with the procedure. This contribution, addressing all stages of the transplant process (donor nephrectomy, benchwork preparation, and implantation) assesses potential pitfalls and technica

Prostate cancer detection in candidates for open prostatectomy.

PURPOSE: We determine the incidence of biopsy detectable prostate cancer in men with clinical benign prostatic hyperplasia (BPH) and prostate specific antigen (PSA) elevation who are candidates for open prostatectomy, and the histology of prostatic tissue of men who underwent surgery. MATERIALS AND METHODS: Sextant peripheral zone prostate biopsies were performed in 128 consecutive men with obstructive voiding symptoms who had digital rectal examination not suspicious for cancer, PSA greater than 4.0 ng./ml. and prostate volume 75 ml. or greater. Of the patients 59 also underwent transition z

Orbital complications of functional endoscopic sinus surgery.

Sinus disease and its surgical treatment carry the risk of orbital complications, including the possibility of blindness. These complications apply to traditional sinus procedures and the more recent endoscopic approach. Two recent endoscopic cases resulted in eye complications, and the lessons learned in their management are herein shared. One case was complicated in the immediate postoperative period with an orbital haematoma that required urgent decompression through lateral canthotomy/cantholysis. The second case involved a patient who complained of visual changes starting 5 days after an

Administrative databases' complication coding in anterior spinal fusion procedures. What does it mean?

STUDY DESIGN: A review of a cohort of 310 consecutive patients who underwent anterior spinal fusion was performed to evaluate the accuracy of hospital ICD-9-CM complication coding. OBJECTIVES: To better understand the clinical significance of conclusions suggested by studies that rely on electronic administrative databases for their data source. SUMMARY OF BACKGROUND DATA: Despite their availability, there have been no studies to date that have evaluated the accuracy of ICD-9-CM administrative databases as they relate to the actual clinical experience in spinal procedures. METHODS: A physic

The cost of misadventures in colonic surgery. A model for the analysis of adverse outcomes in standard procedures.

Analysis revealed an impressive number of patients transferred to intensive care at the Peter Bent Brigham Hospital after misadventures in standard medical and surgical procedures. The model explored here is that of colon surgery, a therapy standardized for decades. The courses of sixteen patients were studied, wherein adverse outcomes appeared to have been preventable. Failure to diagnose colonic leakage and failure to provide colostomy (or to do so safely) were the major underlying causes. Nephrotoxic antibiotics and immunosuppression were sometimes in the background. Nine patients died, all
