We evaluated 643 patients surviving aortic, mitral, and aortic and mitral ("double") valve replacement with the Björk-Shiley prosthesis from 1 to 72 months (median 38 months) postoperatively. Intermediate-term survival rate was similar to that reported for other prosthetic and bioprosthetic devices. Arteriovenous fistulae and long-term indwelling right atrial (Hickman) catheters have been used as methods to improve venous access in patients with acute leukemia. The creation of 28 fistulae provided short duration of function (median, 12 days), with eight fistulae functioning less than 24 hours. Six cases of ovarian pregnancy were reviewed and their relationship with the IUD, fibromyoma of the uterus and previous spontaneous abortion was examined. In fact, 5 patients had an IUD in situ, in 2 women fibroids were found; 4 of the women had spontaneous abortions in their past history. The incidence and management of infections in 80 polytetrafluoroethylene (PTFE) grafts is reviewed. In a follow-up period of 12 to 30 months, the overall incidence of infection was 19%. In functioning grafts the majority of infections occurred after dialysis puncture or reoperation. Analysis of 79 Dacron interposition shunts performed at Emory University from 1971 to 1977 identified a number of preoperative characteristics that correlate with short-term and long-term morbidity. Valve replacement with an Angell-Shiley bioprosthesis was accomplished in 449 patients. To evaluate the bioprostheses from this total series, 344 patients who did not undergo associated operation, had no previous operations, or had no other valve substitutes were selected. During a 15-year period from January 1962 through December 1976, 42 patients who had undergone a previous aortic valve replacement underwent reoperation. The mean interval between operations was 4.4 years. Positive preoperative urine cultures were obtained from 31 of 107 patients who underwent transurethral prostatectomy (TUR). The incidence was 59% among the patients who had had indwelling catheter and 15% in the non-catheterized cases. The authors have observed forty-two cases of fractures and dislocations of the spine in children. In twenty-eight cases there was a complete spinal cord lesion and in four cases a partial cord lesion with some recovery. Ten children had no neurological lesion. 477 cases of femoral head arthroplasty for fresh femoral neck fractures were reviewed to study the various factors modifying life prognosis during the first six months (age, associated disorders, type of prothesis, postoperative complications). Certain of these factors were found to affect prognosis unfavorably: bad general health, alcoholism, psychiatric or neoplasic disease, diabetes, and also, to a less degree, senility, neurologic and sensorial disturbances, cardiac deficiency. A rate table makes it possible to make a prognosis on the basis of these different factors.

This study from a community hospital documents the relatively frequent occurrence of Actinomyces in 36 women diagnosed by Papanicolaou stained cervicovaginal smears. Actinomyces was identified exclusively in patients wearing an intrauterine or vaginal foreign body (intrauterine devices and pessaries). By using fluorescein isothiocyanate labeled antiserum, the organisms were shown to be Actinomyces israelii in eight randomly selected smears. This presentation recounts the original description of CME and the more recent developments associated with intravenous fluorescein angiography in its study and diagnosis. The work of previous authors to show the incidence of CME and its possible prognosis are reviewed. Our findings in a retrospective study agree with the other authors and show that there is no particular increase in CME following intraocular lens implantation regardless of the type of surgical procedure used.
