Barbed continuous suture reduces the risk of specimen extraction site hernia in high-risk obese patients: a CT-based analysis.

BACKGROUND: Extraction-site hernia (ESH) is a recognized complication after laparoscopic colorectal surgery. Obesity increases the risk of ESH, may make fascial closure more difficult, and has been identified as a risk factor. We investigated whether barbed continuous suturing reduces the incidence of ESH compared with interrupted suturing, particularly in obese patients. METHODS: We retrospectively analyzed 193 patients who underwent laparoscopic colorectal surgery at a single center. Closure technique transitioned from interrupted sutures (n = 93) to barbed continuous sutures (n = 100). Pro

Effectiveness of prophylactic Sugarbaker mesh during laparoscopic abdominoperineal resection: a cohort study with 4-year follow-up.

BACKGROUND: Parastomal hernia is a common and time-dependent complication after permanent colostomy following laparoscopic abdominoperineal resection (APR) for rectal cancer. Although prophylactic mesh placement using the Sugarbaker technique has shown favorable short-term results, long-term real-world evidence remains limited. METHODS: This was an ambispective observational cohort study consisting of a retrospective historical cohort (2018-2021) and a prospectively followed cohort (2021-2025) after the introduction of prophylactic mesh. This study included patients undergoing elective laparo

Triglyceride deposit cardiomyovasculopathy as a predictor of vascular failure after coronary intervention in patients with hemodialysis: a preliminary analysis.

Patients on hemodialysis (HD) are at high risk of vascular failure after percutaneous coronary intervention (PCI), even with new-generation drug-eluting stents (DES). Triglyceride deposit cardiomyovasculopathy (TGCV), a recently recognized cardiac metabolic disorder, features defective intracellular triglyceride hydrolysis with diffuse, concentric coronary narrowing, which may attenuate DES efficacy through distinct vascular remodeling biology, but its clinical impact remains unclear. This is a subanalysis of a retrospective, single-center study evaluating the prevalence and clinical outcomes

Association of localized circumferential elastic compression with short-term recanalization and safety during urokinase thrombolysis for lower extremity deep vein thrombosis.

Systemic urokinase infusion via the dorsal foot vein is commonly used for acute lower-extremity deep-vein thrombosis (DVT) complicated by pulmonary embolism (PE), but drug dilution within the superficial venous system may reduce local efficacy. We evaluated whether localized circumferential elastic compression applied at the thigh level was associated with improved short-term recanalization without compromising safety during urokinase thrombolysis. In this retrospective cross-sectional study, 207 consecutive patients with acute proximal DVT and concomitant PE received urokinase via the dorsal

TAVI in low‑risk severe aortic stenosis: evidence, controversies, and lifetime treatment strategy.

Transcatheter aortic valve implantation (TAVI) has expanded from a treatment for inoperable and high-risk aortic stenosis to an established therapeutic option for selected patients at low surgical risk. This review focuses on the outcomes of major trials comparing TAVI to traditional surgical aortic valve replacement (SAVR), highlighting outcomes and potential complications when considering lifetime valve management as we move into the younger population. A literature search of PubMed using terms such as 'transcatheter aortic valve replacement' and 'low surgical risk' was used to identify rele

Clinical Outcomes and Valve Performance of Intra-Annular Self-Expanding Transcatheter Aortic Valve in Valve-in-Valve Procedures: Insights From the NAViV Registry.

BACKGROUND: Most evidence for transcatheter aortic valve replacement (TAVR) for valve-in-valve procedures comes from balloon-expandable and supra-annular self-expanding valves. However, data on intra-annular self-expanding valves remain limited. The objectives of this registry were to evaluate the short- and midterm outcomes of ViV-TAVR using the intra-annular self-expanding valve Navitor platform in a real-world setting. METHODS: The NAViV (Navitor Valve-in-Valve) registry is a retrospective, multicenter study that included 200 patients who underwent valve-in-valve TAVR with the Portico/Navi

Safety of leadless versus transvenous pacemakers in patients with low body surface area: a matched-pair analysis.

BACKGROUND: Leadless pacemakers (LPs) avoid lead- and pocket-related issues but require large venous access, which raises safety concerns in patients with a small body surface area (BSA), particularly in Asian populations. We defined low BSA as < 1.46 m², based on validated 3D-derived anthropometric standards for Chinese adults. OBJECTIVE: To assess procedural complication rates of LP implantation in patients with low BSA. METHODS: We analyzed LP implantations from 2016 to 2025 at a single center in Thailand. Of 59 patients, 25 (42.4%) had low BSA. We conducted two comparisons: (1) low- vs. no

Clinical Performance and Reproducibility of the LIRA Method for TAVR in Bicuspid Anatomy: The SUBLIME Study.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) in bicuspid aortic valve (BAV) anatomy remains challenging, with heterogeneous outcomes and no standardized sizing strategy. The LIRA (level of implantation at the raphe) method is a bicuspid-dedicated approach that complements standard annular sizing with a single perimeter measurement at the raphe level. A previous single-center experience suggested favorable results. OBJECTIVES: The aim of this study was to evaluate clinical performance and reproducibility of the LIRA method for TAVR sizing in a large BAV multicenter cohort. METHOD

The Relationship of Radiation Therapy Parameters and Perioperative Morbidity and Toxicities on Breast Reconstruction Outcomes.

BACKGROUND: Postmastectomy radiation therapy (PMRT) is a key component of breast cancer care, reducing locoregional recurrence in appropriately selected patients. Concurrently, use of mastectomy with implant-based breast reconstruction has expanded, heightening attention to how patient, surgical, and radiation factors interact. There is a knowledge gap in how reconstructive strategies and PMRT parameters jointly influence complications, toxicities, and treatment timing across the expander-implant continuum. OBJECTIVE: The aim of this study was to determine how reconstructive and radiation the

The complex relation of complications and long-term satisfaction in breast reconstruction after mastectomy: A comparative clinical study.

BACKGROUND: Autologous breast reconstruction (ABR) and implant-based breast reconstruction (IBR) are techniques that are commonly employed following mastectomy, yet their comparative impact on health-related quality of life (HRQL) remains debated. Although both approaches have demonstrated positive effects, studies regarding the superiority of one technique over another are lacking consensus, possibly owing to variations in complications and revision surgery requirements. We aimed to investigate and compare factors associated with HRQL in both techniques. METHODS: This retrospective single-ce

Outcome of breast implant replacement after radiation therapy: A single-centre retrospective comparative study.

BACKGROUND: Implant-based breast reconstruction is a common procedure after mastectomy; however, postoperative radiotherapy (RT) increases the risk of developing complications such as capsular contracture, infection and implant failure. We aimed to compare the outcomes of implant replacement in irradiated versus non-irradiated patients and identify risk factors associated with complications in the RT group. METHODS: This retrospective study included 83 women who underwent implant replacement after two-stage implant-based reconstruction between 2009 and 2021. Patients were stratified into RT (
