The mediating and moderating roles of physical sub-health in the relationship between family functioning and mental health problems among Chinese adolescents.

BACKGROUND: Adolescent mental health is shaped by both family environments and physical well-being. However, the role of physical sub-health, a state of functional imbalance and fatigue prior to diagnosable illness, remains poorly understood in this process. This study examined whether physical sub-health serves both as a psychosomatic pathway and as a vulnerability factor linking family functioning to adolescent mental health. METHODS: A cross-sectional survey was conducted among 647 primary and secondary school students across China. Validated scales were used to assess family functioning, p

Single-stage surgical repair of congenital aortopulmonary window associated with interrupted aortic arch in two toddlers: a case report.

Congenital aortopulmonary window (APW) associated with interrupted aortic arch (IAA) is a rare and severe form of congenital heart disease. The optimal surgical timing and strategy remain challenging, particularly beyond infancy. We report two toddlers diagnosed with APW and IAA who underwent one-stage surgical repair. At the time of surgery, the children were 17 and 22 months old, with body weights of 6.6 kg and 8.0 kg, respectively. Both patients survived and experienced no perioperative complications. During postoperative follow-up of 12 and 19 months, respectively, the children demonstrate

Factors associated with lateral abdominal protrusion after congenital diaphragmatic hernia repair.

PURPOSE: Lateral abdominal protrusion (LAP) is observed after congenital diaphragmatic hernia (CDH) repair. We developed an objective radiographic index and explored factors associated with LAP. METHODS: Seventy-three patients who underwent neonatal CDH repair between 2013 and 2023 were retrospectively evaluated. The standardized LAP index (SLAPI) was developed in the 2013-2018 cohort and validated in the 2019-2023 cohort. Candidate factors available before LAP detection were examined using univariable logistic regression, followed by exploratory Firth logistic regression. Intraobserver relia

Outcomes of modified single trocar laparoscopic catheter placement for maintenance peritoneal dialysis in children.

BACKGROUND: Peritoneal dialysis (PD) access remains a major challenge in pediatric kidney replacement therapy. Although laparoscopic approaches are increasingly favored, pediatric evidence regarding single-trocar placement remains limited. METHODS: This retrospective cohort study included children with kidney failure or advanced chronic kidney disease who underwent a modified single-trocar laparoscopic Tenckhoff catheter insertion between January 2020 and November 2021. All required urgent-start PD on the day of insertion. The technique used a transumbilical 10-mm operative laparoscope, obliq

Impact of FXIII Levels on Hematoma Expansion and Outcomes in Patients with Spontaneous Intracranial Hemorrhage: A Prospective Observational Cohort Study of 132 Patients.

BACKGROUND: Diminished levels of FXIII are significantly correlated with hematoma expansion (HE) and unfavorable clinical outcomes. Although the crucial function of FXIII in coagulation is well-recognized, the connection between FXIII deficiency and either HE or negative prognosis in patients with spontaneous intracerebral hemorrhage (sICH) is not well understood. METHODS: Patients with sICH were confirmed through baseline computed tomography scans conducted within 6 h of symptom onset. Plasma FXIII levels were assessed at the time of admission using a standardized enzyme-linked immunosorbent

Dogmas in Obstetrics: Evidence-Based Standards, Clinical Judgement and Patient-Centred Care.

Obstetrics requires standards, guidelines and established pathways to support physiological birth and to manage rare emergencies reliably. Standards become problematic, however, when they are detached from their evidentiary basis, original indication or individual clinical context and are applied as dogmas. This narrative review and conceptual analysis examines typical mechanisms of dogmatic decision-making in obstetrics: routine interventions without individual indication, seemingly precise thresholds, medico-legal safety routines, inconsistent definitions of obstetric complications, psycholo

Incidence of Retinal Artery Occlusion After Carotid Endarterectomy Versus Carotid Artery Stenting.

PURPOSE: Retinal artery occlusion (RAO) is a rare but vision-threatening complication of carotid revascularization, yet its comparative incidence after carotid endarterectomy (CEA) versus carotid artery stenting (CAS) is poorly characterized. We compared the risk of RAO and associated neurologic outcomes after CEA and CAS. METHODS: Retrospective multicenter cohort study using the TriNetX Research Network (January 1, 2004-March 15, 2026). Adults undergoing CEA or CAS with ≥6 months of follow-up and no prior diagnosis of any studied outcome condition (retinal artery or vein occlusion, glaucoma,

Distinct attentional profiles associated with diabetic ketoacidosis and hypoglycemia in pediatric type 1 diabetes: MOXO-CPT assessment.

INTRODUCTION: Type 1 diabetes (T1D) in children and adolescents has been associated with alterations in attentional and executive functioning. Acute metabolic complications, particularly diabetic ketoacidosis (DKA) and clinically significant hypoglycemia (CSH), may adversely affect neurocognitive development. This study evaluated the associations between acute metabolic events and attentional functioning in pediatric patients with T1D. MATERIALS AND METHODS: The study included 117 children and adolescents with T1D (55 females, 62 males; mean age 12.86 ± 3.16 years). Participants were stratifi

Outcomes of endoscopic sinus surgery (ESS) on sino-orbital osteomas: ophthalmic manifestations and visual outcomes from a 12-year multicenter experience and literature review.

BACKGROUND: Osteomas are the most common benign craniofacial bone tumors that may extend into the orbital cavity. While the majority of them remain asymptomatic, orbital involvement can lead to clinically significant ophthalmologic manifestations. Although endoscopic sinus surgery (ESS) has been increasingly utilized, data focusing specifically on ophthalmologic outcomes following ESS in sino-orbital osteomas are limited. METHODS: This 12-year multicenter descriptive cohort evaluated 12 patients with symptomatic sino-orbital osteomas managed with ESS, including advanced approaches such as Dra

Impact of pre-tracheostomy continuous enteral feeding on aspiration pneumonia risk and clinical outcomes: a prospective observational study.

PURPOSE: The optimal management of enteral nutrition before tracheostomy in mechanically ventilated patients in the intensive care unit (ICU) remains uncertain, and pre-procedural fasting is widely practiced in clinical settings to reduce the risk of aspiration. However, interruption of enteral nutrition (EN) may adversely affect metabolic stability and caloric intake in critically ill patients. This study aimed to prospectively evaluate whether continuation of EN before tracheostomy increases aspiration risk or influences clinical outcomes in mechanically ventilated ICU patients. PATIENTS AN

Medical liability in paediatric robotic surgery - a current concept and a literature review.

Robotic-assisted surgery offers significant technical advantages in pediatric procedures but presents complex, unresolved medico-legal challenges regarding liability. We retrospectively reviewed our ten-year experience comprising 205 pediatric robotic procedures. Technical issues related to the robot were encountered in 7 cases (3.4%). These included joystick latency (3/7), coagulation/cutting pedal malfunctions (2/7), and accidental power cable disconnections causing system shutdowns (2/7). A literature review was conducted to assess the current international state of the art regarding roboti

When legal reasoning contradicts sports orthopaedic evidence.

This manuscript presents the perspective of the International Society of Cartilage Repair of the Ankle (ICCRA) regarding a recent English High Court ruling that challenges established principles of evidence-based sports orthopaedic care. Following a legal case involving a professional footballer and the surgical management of an ankle fracture with concomitant arthroscopy, the authors highlight significant discrepancies between the legal conclusions and the current scientific literature. The ruling suggests that arthroscopic intervention contributed to the development of ankle arthritis and pr

Prevalence of sexually transmitted infections among pregnant migrant and refugee women: Protocol for a systematic review and meta-analysis.

UNLABELLED: Sexually transmitted infections during pregnancy are associated with important maternal, fetal, and neonatal complications, including vertical transmission, congenital infection, stillbirth, preterm birth, and increased maternal morbidity. Pregnant migrant and refugee women may face additional risks because displacement, legal precarity, socioeconomic vulnerability, and limited access to antenatal care can delay screening and treatment. However, the burden of sexually transmitted infections in this population remains poorly characterized, and available estimates are fragmented acro

Complete laryngotracheal separation following blunt neck trauma: A forensic reconstruction of an unusual fatal injury.

INTRODUCTION: Complete laryngotracheal separation (CLTS) is an exceptionally rare and life-threatening consequence of blunt neck trauma. Because external cervical injuries may be minimal or nonspecific, diagnosis can be delayed in both clinical and forensic settings. CASE PRESENTATION: A 35-year-old man was found unconscious at the roadside and died two hours after hospital admission despite emergency management. Autopsy revealed extensive cervical subcutaneous emphysema, complete laryngotracheal separation, bilateral thyroid cartilage fractures, cricoid cartilage disruption, destruction of t

Systems biology analysis reveals miRNA-mRNA networks associated with cardiomyocyte responses to SARS-CoV-2 infection.

Cardiac complications are common and clinically significant in COVID-19, yet their underlying molecular drivers remain poorly understood. Here, we integrate post-mortem histopathology with transcriptomic and microRNA (miRNA) analyses to delineate the regulatory architecture of SARS-CoV-2-induced myocardial injury. Histological examination of cardiac tissue from 29 deceased COVID-19 patients revealed pronounced immune infiltration, cardiomyocyte necrosis, and fibrotic remodeling. To gain new insights into these pathological processes, we first analyzed 42 transcriptomes from SARS-CoV-2-infected

Tranexamic acid use and the risk of postoperative delirium after spine surgery: a retrospective cohort study.

BACKGROUND: Postoperative delirium (POD) is a common complication after major surgery and has been linked to perioperative neuroinflammation. Tranexamic acid (TXA), an antifibrinolytic agent, may influence inflammatory pathways, but its effect on POD remains unclear. This post hoc analysis investigated the association between intraoperative TXA administration, POD, and perioperative biomarkers of inflammation and neuronal injury. METHODS: This post hoc analysis of the prospective CONFESS cohort included patients aged ≥ 60 years undergoing elective spine surgery. POD was assessed using the Nur
