Gastrointestinal Kaposi sarcoma is an uncommon but clinically relevant manifestation of human herpesvirus 8-associated disease in patients with human immunodeficiency virus (HIV) infection. Its diagnosis may be delayed because symptoms are often nonspecific and may overlap with other anorectal disorders, particularly in patients with condylomatous disease, fissures, proctitis, or anal malignancy. We report the case of a 25-year-old male with HIV infection on antiretroviral therapy and an undetectable viral load who presented with one month of anal pain and rectal bleeding. Initial colonoscopy

A wide range of spindle cell proliferations are found uncommonly in the sigmoid colon, rectum and anus. They usually present as polyps, and include reactive lesions and benign and malignant neoplasms which may be primary or metastatic. They are less frequently described in the literature compared to those in the upper gastrointestinal tract, and may be underdiagnosed. The widespread use of sigmoidoscopy in symptomatic patients and bowel cancer screening programmes means that histopathologists must be aware of, and adopt a logical approach to, diagnosing spindle cell proliferations in biopsy an

PURPOSE: Anal sphincter lesions represent the major cause of fecal incontinence, particularly in women. Sphincteroplasty with overlap is the traditional treatment, but a significant reduction in benefits within 5 years of surgery has been reported. More recently, sacral nerve stimulation has been suggested following sphincteroplasty or as primary treatment.
METHODS: Overall, 24 women with fecal incontinence in the presence of anal sphincter lesions underwent sphincteroplasty (14 patients, mean age 47.6 +/- 15.6 years, range 26-70) or definitive implant of sacral nerve stimulation (10 patients

Chemoradiotherapy is the standard treatment for anal cancer. Surgery is reserved for failure of therapy, but there are limited data examining outcomes after surgery. From a prospective population-based database on radiation and surgical therapy, we compare outcomes for patients with anal cancer undergoing rectal resection after radiation with patients undergoing radiation alone. Patients undergoing surgical resection of the rectum after initial radiation (SRT) for squamous cell carcinoma of the anus, anal canal, cloacogenic zone, and overlapping lesions of the rectum and anal canal from 1983 t

PURPOSE: The possible existence of an anatomic and functional separation between the external sphincter and the puborectalis muscle has been reported in the medical literature. In this article we confirm, by means of anatomic and clinical observations, the presence of such a separation, focusing on its importance in understanding the pathway of diffusion for some suppurative anal lesions and to plan advanced sphincter-sparing procedures.
METHODS: Twenty adult anatomic specimens of the anal region (12 from women) were cut in the sagittal, coronal, and paracoronal planes, stained with hematoxyl

Injuries of the colon and rectum are common surgical problems. Lesions can be classified into four groups according to the site of damage and the presence of sphincter tears: 1. intraperitoneal perforation without sphincter damage 2. intraperitoneal perforation with sphincter damage 3. extraperitoneal perforation without sphincter damage 4. extraperitoneal perforation with sphincter damage From 1990 to 1998, 11 patients, 7 males and 4 females presenting an anal and/or rectal trauma were admitted in Geneva University Hospital. 8 patients were admitted as an emergency, the 3 others had been tran

PURPOSE: Extramammary Paget's disease is a rare dermatosis. Wide local excision is recommended in patients with perianal extramammary Paget's disease. After wide local excision, it is necessary to do reconstruction, with preservation of bowel function. We present here two cases of perianal Paget's disease, in which the patients were treated by reconstruction with bilateral gluteus maximus myocutaneous rotation flap after wide local excision.
PATIENTS AND METHODS: A 55-year-old woman and 58-year-old man were admitted with anal pain and bleeding. Histologic examination of the perianal lesion re

The results after surgical repair of traumatic anal incontinence in 23 patients, 12 with simple lesions and 11 with complicated lesions, were analyzed. Overlapping suture was the standard technique in simple lesions, whereas additional procedures were necessary in complicated lesions. Fifteen patients (65 percent; 95 percent confidence limits: 43-83) had excellent results and seven (30 percent; 13-53) had acceptable results. In patients with simple lesions, the result was excellent in nine (75 percent; 43-94), and was excellent also in six patients with complicated lesions (55 percent; 23-83).
