Lateral Opening-Wedge Versus Medial Closing-Wedge Distal Femoral Osteotomy for Adolescent Idiopathic Genu Valgum: A Randomised Controlled Trial.

BACKGROUND: Distal femoral osteotomy is used to correct symptomatic genu valgum after distal femoral physeal closure, but comparative evidence between lateral opening-wedge osteotomy (LOW) and medial closing-wedge osteotomy (MCW) in adolescents is limited. We compared radiographic correction, recovery, and complications between the techniques. METHODS: In this prospective randomised trial, 65 adolescents aged 12 to 18 years with idiopathic genu valgum and radiographic closure of the distal femoral physis were allocated 1:1 to LOW or MCW. Three participants were lost to follow-up, leaving 62 p

Surgical repair of traumatic right subclavian artery pseudoaneurysm after war shrapnel injury: A case report.

A rare right subclavian artery pseudoaneurysm developed after a war-related shrapnel injury in a 46-year-old female with a prior thyroidectomy. There was no initial bleeding or neurologic deficit, and initial noncontrast computed tomography findings were negative. One month later, the patient presented with chest pain, hoarseness, and dysphagia. A chest X-ray showed tracheal compression, Doppler ultrasound revealed leakage from the posterior wall of the right subclavian artery, and computed tomography angiography confirmed a 6 × 7 cm pseudoaneurysm causing 70% tracheal narrowing. Open repair w

Early mobilization in surgery: a narrative historical review of its evolution and contemporary implications.

Early mobilization after surgery is widely recognized as a key component of enhanced recovery after surgery (ERAS) protocols, improving patient outcomes and reducing hospital stays. However, the historical roots of this practice remain insufficiently explored. This study aimed to examine the historical evolution of early mobilization from the 19th century to the mid-20th century and to reflect on its relevance for contemporary perioperative care. This study was designed as a narrative historical review based on primary historical sources, including medical articles, editorials, and case report

Gulf War Illness: A Historical Review and Considerations of a Post-Viral Syndrome.

Gulf War Illness (GWI) is a condition that affects 30-40% of nearly 700,000 Veterans who were deployed to Operations Desert Shield/Storm/Sabre (ODS/S/S) between August 1990 and June 1991 and is characterized by a constellation of symptoms, including fatigue, mood/cognition, chronic pain, gastrointestinal (most frequently referred to as "irritable bowel syndrome"), respiratory, and skin issues. We review the development of various case definitions for GWI, as well as exposure theories. Despite heavy investment in research, both the pathophysiology and underlying cause of GWI remain areas of act

External Fixation for War-Related Mandibular Fractures in a Resource-Limited Setting: A Retrospective Study of 91 Patients.

Background/Objectives: War-related mandibular injuries result in extensive soft-tissue damage, severe comminution, and bone loss, and are associated with high rates of infection and delayed healing. No universally accepted management protocol exists for these injuries. External fixation is commonly used in this context, particularly when internal fixation is unavailable or contraindicated. This study aimed to analyze injury patterns, treatment outcomes, and complications of war-related mandibular fractures treated with external fixation as a primary and definitive stabilization method in a res

Mechanistic signatures of comorbid PTSD with cognitive impairment implicate cortisol-induced neural toxicity.

The men and women who worked in rescue and recovery operations at the 9/11 World Trade Center site are developing cognitive impairment (CI) at mid-life, decades before CI is usually detected. To date, one of the most consistent risk factors for CI in this population is symptoms of post-traumatic stress disorder (PTSD). However, little is known about the mechanistic cascade that drives stress-related neurological changes to accelerate cognitive decline in the human brain. We used machine learning to identify distinct brain signatures from functional magnetic resonance imaging between trauma-exp

Over-the-Scope Clip for Treating Chronic Bariatric Surgery Leaks Refractory to Prior Endoscopic Therapies: Experience From a Bariatric Center of Excellence.

Background Chronic leaks following bariatric surgery continue to present significant therapeutic challenges, particularly when conventional endoscopic interventions are unsuccessful. The over-the-scope clip (OTSC) is increasingly being employed as a salvage approach; however, supporting evidence remains limited. This study sought to assess the efficacy of the OTSC in complex cases in which conventional endoscopic interventions proved unsuccessful. We reviewed all refractory cases and proposed the use of the clip as an innovative strategy for these situations. Methods We performed a retrospecti

The relationship between asthma and glioma: a case-control study in a universal access healthcare system.

PURPOSE: Multiple studies have found an inverse relationship between asthma and malignant gliomas, possibly due to increased immune response from asthma. This study assessed the relationship between asthma and subsequent glioma diagnosis while reducing potential effects of recall and detection bias. METHODS: Data were extracted from the Military Cancer Epidemiology database which includes cancer registry and medical claims data from military and retired servicemembers and their dependents. Cases were individuals with a pathology-confirmed glioma diagnosis between 1998 and 2014. Cancer free-con

Standardizing Tourniquet Reassessment and Conversion Across TCCC Tiers: TCCC Guidelines Proposed Change 25-2.

Operational experience from the Russo-Ukrainian War revealed frequent use of tourniquets that were not medically indicated and an increase in ischemic complications from prolonged tourniquet application as a consequence of extended evacuation times. In response to this, the Committee on Tactical Combat Casualty Care (CoTCCC) convened a working group to evaluate whether tourniquet reassessment and conversion practices should extend to the All Service Member (ASM) level. With input from NATO and partner nations, the Working Group developed a standardized, time-based algorithm that provides plain

MyD88 Inhibition Ameliorates Diabetes-Induced Hepatic Inflammation and Gluconeogenesis Through Adipose IL-10 Induction.

Myeloid differentiation factor 88 (MyD88) signaling plays a central role in inflammatory pathway activation. Adipose-derived interleukin-10 (IL-10), which is induced by insulin and lipopolysaccharides, suppresses hepatic glucose production. This study investigated the role of MyD88/IL-10 signaling in diabetes-induced systemic inflammation and hepatic gluconeogenesis. Stromal vascular fractions (SVFs) were isolated from the adipose tissue of Leprdb/db and Leprdb/dbMyD88-/- mice and treated with IL-10 followed by analysis of inflammatory cytokine expression. IL-10 (10 or 50 ng) was injected into
