Incidence and risk factors of fracture-related infection after SIGN nailing of open femoral fractures in Ethiopia.

Fracture-related infection is one of the most serious complications following open fracture management. Despite its preventable complications, Fracture-related infections continue to threaten public health with significant impacts on human and financial resources. The number of patients with open femur fractures increased in our hospital due to the war in the northern part of Ethiopia, but there was no study. This study aimed to assess the incidence of Fracture-related infection and its associated factors after intramedullary nailing of an open femur fracture using a Surgical Implant Generatio

Contemporary Management of Acute Coronary Syndromes in the Cardiac Intensive Care Unit: From Rapid Diagnosis to Early Secondary Prevention.

Management of acute coronary syndromes (ACS) in the cardiac intensive care unit (CICU) requires rapid diagnosis, timely reperfusion or invasive assessment, appropriate antithrombotic therapy, and early recognition of haemodynamic, electrical, and mechanical complications. This narrative review examines contemporary evidence across ST-segment elevation and non-ST-segment elevation presentations, with emphasis on decisions during hospitalisation and early secondary prevention. High-sensitivity cardiac troponin algorithms support accelerated assessment of suspected non-ST-segment elevation ACS, w

Blunt traumatic right-sided pericardial rupture without cardiac injury: a case report.

Blunt traumatic pericardial rupture is an exceptionally rare condition, often associated with high morbidity and mortality. Isolated pericardial rupture without vascular or cardiac involvement accounts for only 17% of reported cases. We describe a rare case of right-sided pericardial rupture following blunt chest trauma, an unusual presentation since most pericardial tears occur on the left side. A 23-year-old woman involved in a motor vehicle accident sustained multiple injuries, including right hemothorax, bilateral pulmonary contusions, left-sided rib fractures, and a humerus fracture. Extremity hemorrhage control in urban warfare: A retrospective analysis of the Swords of Iron War.

BACKGROUND: Tourniquets are a life-saving intervention for controlling life-threatening extremity hemorrhage. However, prolonged tourniquet application can lead to significant morbidity. This study aimed to assess tourniquet usage among military personnel in modern urban warfare and evaluate the proportion of cases potentially suitable for tourniquet conversion. METHODS: This was a retrospective cohort analysis of all Israeli military casualties hospitalized between October 27, 2023, and November 30, 2024, with war-related injuries during the Hamas-Israel War. Data were obtained from the preh

A retrospective observational study on management of penetrating gunshot and migrating projectile injuries to the heart during the war in Ukraine.

BACKGROUND: Penetrating cardiac injuries caused by firearms are associated with extremely high prehospital and early in-hospital mortality. During wartime, civilian cardiac surgery centers are increasingly involved in the management of complex gunshot and fragment-related cardiac injuries, yet published evidence from nonmilitary practice remains limited. METHODS: We conducted a retrospective observational study of civilian and military patients with penetrating cardiac gunshot or fragment injuries treated surgically at a tertiary civilian cardiac surgery center between February 2022 and Decem

Is Prevena a better choice for patients with pressure ulcers after reconstruction?

OBJECTIVE: Prevena (3M KCI, US) is a new incisional negative pressure wound therapy system (iNPWT) for closed surgical incision management. The surgical management of pressure ulcers (PUs) remains challenging. A high rate of recurrence and overall complications have been reported. This study analysed the clinical efficacy of the iNPWT system in patients with PUs. METHOD: In this retrospective study, patients with stage 3 or 4 PUs who were surgically managed by a single plastic surgeon specialising in PUs at Tri-Service General Hospital, Taiwan, between January 2011 and June 2022 were reviewed

Efficacy of a Disposable Skin-Stretch Closure Device Combined With Vacuum Sealing Drainage in the Treatment of Refractory Skin and Soft Tissue Defects: A Single-Arm Prospective Study.

OBJECTIVES: The treatment of refractory skin and soft tissue defects (RSSTDs) has long posed a global challenge for surgeons. Traditional closure methods, such as skin grafting and flap repair, are often associated with significant trauma and a steep learning curve. Skin stretching, which enables the generation of "extra skin," has emerged as a promising alternative. This study aims to evaluate the efficacy of a disposable skin-stretch closure (DSSC) device combined with vacuum sealing drainage (VSD) in the treatment of RSSTDs. METHODS: A prospective single-arm clinical study was conducted to

Impact of Cytochrome P450 and P-glycoprotein Gene Polymorphisms on the Risk of Hemorrhagic Complications in Older Patients with Non-Valvular Atrial Fibrillation Treated with Rivaroxaban.

Background: The administration of direct oral anticoagulants to older adults requires careful risk stratification against a backdrop of polypharmacy and age-related renal function decline. The role of single nucleotide polymorphisms in cytochrome P450 genes and efflux transporters in this context remains a clinically understudied issue. This study aimed to investigate the association of ABCB1, CYP3A4, and CYP3A5 allelic variants with the safety of rivaroxaban therapy in geriatric patients with non-valvular atrial fibrillation (AF). Materials and Methods: This prospective cohort study consecuti

Clinical and Hematological Characteristics of Rhabdomyolysis-Associated Kidney Injury in Military Personnel With Limb Wounds.

INTRODUCTION: Tourniquet (TQ) use is a standard method of prehospital hemorrhage control in combat trauma. Prolonged limb ischemia with tourniquet syndrome (TS) may lead to severe skeletal muscle injury, traumatic rhabdomyolysis, and acute kidney injury (AKI). In recent armed conflicts, prolonged evacuation intervals have increased the frequency and clinical significance of these complications, while detailed clinical and laboratory descriptions remain limited. MATERIALS AND METHODS: A retrospective analysis was performed of 84 active-duty service members with combat-related extremity injurie

The impact of the 7th October war on preterm birth rates and neonatal outcomes: a retrospective comparative study from northern Israel.

PURPOSE: Maternal psychological stress is associated with adverse obstetric and neonatal outcomes, including preterm birth and low birthweight. Warfare represents a significant source of acute and chronic stress, yet its impact on pregnancy outcomes remains unclear. This study aimed to evaluate the association between a 6-month period of continuous wartime exposure following October 7, 2023, during the "Iron Swords" conflict and preterm birth, neonatal outcomes, and obstetric management. METHODS: This retrospective cohort study included all deliveries at ZIV Medical Center during the 6-month

Unveiling glomerulonephritis in the Pacific: a Fijian cohort study.

The epidemiology of kidney diseases is poorly known in several settings, including the Pacific Islands. To contribute to filling this gap, the current study aims to explore the incidence, distribution and patterns of biopsy-proven glomerulonephritis (GN) in Fiji, and describe the experience of organizing a kidney biopsy service in our resource-limited setting. All native kidney biopsies conducted in Fiji's public sector from 2019 to 2023 were included in the annual crude biopsy and complication rate. Biopsies that did not lead to diagnosis, that diagnosed a non-glomerular pathology, and repeat

Evidence based management of popliteal vessel injuries: A critical review, updates and controversies in the management of a difficult injury.

Popliteal artery injuries are rare even in urban Trauma Centers. The vast majority occur secondary to penetrating trauma. These injuries are uncommon; therefore, few Trauma Surgeons and Trauma Centers have developed significant experience with their management. Experiences reported in both military and civilian publications consistently report the highest complications and amputation rates of all vascular injuries due to popliteal vessel injuries. The popliteal artery is an end artery. Injuries cause significant ischemia that significantly threaten limb viability. The popliteal vein provides t

Implementation of Modified Enhanced Recovery After Surgery Protocols for Elective Incisional Hernia Repair After Damage Control Surgery: A Retrospective Observational Study.

BACKGROUND: The ongoing full-scale war in Ukraine has led to a significant increase in the number of patients undergoing damage control surgery following abdominal shrapnel wounds. These injuries are consistently associated with extensive soft tissue defects of the abdominal wall and secondary wound healing that frequently lead to the formation of large ventral hernias. In such patients, the primary goal is to provide the safest possible treatment and facilitate rapid recovery. The implementation of Enhanced Recovery After Surgery (ERAS) protocols has shown proven benefits in elective surgical
