Bridging gaps in diabetic neuropathy care: A Delphi-based regional consensus from the Gulf Cooperation Council.

AIMS/INTRODUCTION: Diabetic peripheral neuropathy (DPN) is a common complication of diabetes in the Gulf Cooperation Council (GCC) region, associated with delayed diagnosis, significant pain burden, and preventable complications. This Delphi-based consensus aimed to identify regional gaps in screening and management to develop practical, evidence-based recommendations tailored to GCC healthcare systems. MATERIALS AND METHODS: A three-round Delphi process was conducted involving 12 diabetes experts from six GCC countries, who reviewed international guidelines, identified unmet clinical needs,

Chinese expert consensus on tetanus prophylaxis in major trauma (2026 Edition).

Tetanus is an acute infectious disease caused by Clostridium tetani, which enters the body through contaminated wounds, proliferates under anaerobic conditions, and produces neurotoxin. Patients with major trauma are at a markedly increased risk of tetanus owing to extensive tissue injury, heavy wound contamination, and trauma-associated impairment of immune function. Existing guidelines and consensus statements on tetanus prophylaxis largely address general open wounds and provide little specific guidance for patients with major trauma, particularly those undergoing emergency life-saving surg

Immediate Rehabilitation of Critical-Size Gunshot- and Mine Blast-Related Maxillary Defects Using Cortically Anchored Single-Piece Implants: Two Case Reports.

Background and Clinical Significance: Implant rehabilitation of patients with acquired maxillofacial defects remains challenging, particularly following high-energy war-related trauma. Gunshot and mine blast injuries frequently result in extensive hard and soft tissue loss, often requiring complex reconstructive procedures. Although cortically anchored implants have been successfully used in patients with severe maxillary atrophy and selected traumatic defects, evidence supporting their use for the immediate rehabilitation of critical-size war-related maxillary defects remains limited. Cortica

Mechanisms of Obesity-Related Kidney Disease: From Adipose Depot Biology to the Chymase-Aldosterone and Ghrelin-Leptin Axes.

Obesity is an increasingly important and modifiable driver of chronic kidney disease (CKD), with effects that extend well beyond its associations with type 2 diabetes, hypertension, and dyslipidemia. To synthesize the evidence that excess adiposity is a causal and modifiable determinant of kidney disease, and to examine how specific adipose depots injure the glomerulus and the tubulointerstitium, and then map these mechanisms onto established and emerging therapies. Throughout, obesity-related kidney disease (ORKD) denotes the full spectrum of diposity-driven renal injury, whereas obesity-rela

Surgical outcomes in an improvised underground operating room during missile attacks.

PURPOSE: Armed conflict disrupts healthcare when hospital infrastructure is vulnerable to missile attacks. During a 12-day missile strike period, our hospital established an improvised operating room (OR) in an underground parking lot. This study evaluated perioperative and short-term outcomes among patients treated in the improvised OR compared with those treated in the hospital's fortified standard OR. METHODS: This retrospective cohort study included patients undergoing general or orthopaedic surgery during the 12-day conflict. Procedures identified via surgical logs were categorized by OR

Saving Hands in Times of War in Conflict Zones with Limited Resources: A Cohort Study from Sudan.

BACKGROUND: The hand is essential for daily function, yet war-related injuries can cause complex damage leading to disability or amputation. This issue is particularly pronounced in resource-limited settings. In light of these challenges, this article examines the epidemiology, types of injuries, surgical interventions, and complications of combat-related hand injuries in Sudan. METHODS: This prospective cohort study was conducted at a single hospital from January 2024 to January 2025. Fifty-three patients with combat-related hand injuries underwent reconstructive surgery, and data were colle

Safety and Efficacy of rhBMP-2 for Treating Acute Traumatic Fractures of the Upper and Lower Extremities: A Multicenter Prospective Study.

Background: Delayed or non-union fractures comprise 5-10% of cases, indicating the need for biologic interventions. Recombinant human bone morphogenetic protein-2 (rhBMP-2) is a potent osteoinductive agent; yet, collagen carrier-based uncontrolled release causes adverse events. We evaluated the safety and efficacy of a hydroxyapatite (HA) carrier-based rhBMP-2 delivery system for acute traumatic upper and lower fractures exhibiting bone defects. Methods: This prospective, multicenter, single-arm clinical trial enrolled 90 patients who underwent surgery using a hydroxyapatite (HA) carrier-based

Open resection versus arthroscopic resection for wrist ganglion cysts: A systematic review and meta-analysis.

This study aimed to review the postoperative outcomes of surgical treatment options for wrist ganglion cysts and perform a meta-analysis comparing the 2 most common techniques: open and arthroscopic resection. We searched PubMed, Embase, Web of Science, and Cochrane Library databases for articles that compared the clinical outcomes of patients who underwent arthroscopic or open resection of the wrist ganglion. After screening, we identified two randomized controlled trials and four retrospective comparative studies. Recurrence rates, complications, functional scores, pain, operative time, and

Chronic inflammatory diseases and survival among patients with pancreatic cancer in the U.S. Military health system.

BACKGROUND: Systemic inflammation promotes the initiation and progression of pancreatic cancer, but the association between chronic inflammatory diseases and survival is less understood. This study aimed to investigate the relationship between chronic inflammatory diseases and survival among patients with pancreatic cancer in the US Military Health System (MHS), a healthcare system with universal access. METHODS: The study utilized the Military Cancer Epidemiology database (MilCanEpi), a linked database comprising data from the Department of War's Central Cancer Registry (CCR) and the Militar

From Shrapnel to Stability: A Reverse Perforator Flap for Definitive Elbow Coverage Seven Decades Post-Trauma. A Case Report.

This case report details the lifelong sequelae and complex management of a shrapnel injury sustained by a 4-year-old male during a 1943 World War II bombing. The initial trauma caused a severe right elbow fracture and complete ulnar nerve transection, resulting in ankylosis and chronic joint dysfunction. Decades later, the patient developed recurrent ulcerations and scar instability over the olecranon. A suspected chronic osteomyelitis was initially managed conservatively with antibiotics and scar revision with lipofilling. However, ulceration recurred, ultimately necessitating aggressive surg

Osteomyelitis of the Third Metatarsal Mimicking a Neoplastic Lesion Due to a Retained Wooden Foreign Body.

Plantar puncture wounds are a common entity among children, particularly in areas where walking barefoot is common, but the detection of radiolucent foreign bodies (FBs), like those made of wood, remains elusive, with the risk of delayed diagnosis. The patient was a four-year-old girl who presented with chronic foot pain, limping, and a localized dorsal mass six months following a plantar puncture injury. Initial management at another facility consisted of exploration for a suspected FB and abscess, guided by an ultrasound report and an attempt at removal guided by ultrasound; however, her sym

Early radiographic accuracy and intra-operative parameters of patient-specific instrumentation versus conventional technique in knee osteotomies for adolescent deformity correction: A prospective comparative study.

BACKGROUND: Patient-specific instrumentation (PSI) has been proposed to improve geometric precision in osteotomy around the knee, but prospective comparative evidence in skeletally maturing adolescents is sparse. We compared the two techniques in this group. METHODS: In this prospective comparative cohort study, 24 adolescents (15-20 years) with genu varum (n = 10) or genu valgum (n = 14) underwent medial opening-wedge high tibial osteotomy (HTO) for varus or lateral opening-wedge distal femoral osteotomy (DFO) for valgus, allocated to a PSI-assisted (n = 12) or conventional (n = 12) group. International Delphi Consensus on the Diagnosis and Management of Septic Arthritis of the Hip in Prewalking Children.

BACKGROUND: Septic arthritis of the hip in infants and prewalking children is a surgical emergency that can lead to rapid joint destruction and long-term sequelae. Despite its urgency, diagnostic and management practices vary considerably, and high-quality comparative evidence remains limited. This study aimed to develop expert consensus on the key principles of diagnosis and management using a modified Delphi methodology. METHODS: A working group of 3 pediatric orthopaedic surgeons developed 20 statements addressing clinical diagnosis, imaging, medical and surgical management, postoperative

Dorsal Spinal Cord Herniation Following Thoracic Laminectomy for Medulloblastoma Metastasis: A Case Report and Literature Review.

BACKGROUND: Dorsal spinal cord herniation is an exceptionally rare postoperative complication, most often associated with iatrogenic dural defects or adhesions, and may result in progressive neurological decline if not promptly recognized. Unlike the more frequently encountered idiopathic ventral subtype, dorsal herniation typically follows spinal intervention and requires early radiological suspicion in symptomatic patients. REPORT OF CASE: A 14-year-old boy developed dorsal spinal cord herniation after thoracic laminectomy for medulloblastoma drop metastasis. Initial treatment included subo

[Surgical management of spinal metastases].

BACKGROUND: Spinal metastases from malignant tumors represent an increasingly relevant clinical problem due to improved systemic therapies, prolonged survival, and the growing prevalence of long-term survivors with metastatic disease. The spine is not only a central biomechanical component of the musculoskeletal system, but also protects the spinal cord and spinal nerves. Accordingly, the treatment of spinal metastases is complex and must always be embedded within an overarching oncological treatment concept. BASICS: This review summarizes epidemiological and pathophysiological principles and

Intra-abdominal pressure in children receiving peritoneal dialysis for acute kidney injury.

BackgroundCritically ill children on peritoneal dialysis (PD) are at an increased risk of raised intra-abdominal pressure (IAP). The aim of this study was to describe IAP and complications with a standard PD fill volume, using conventional PD and continuous flow PD (CFPD) in critically ill children with acute kidney injury. A secondary objective was to compare two IAP measurement techniques, which were direct measurement from the PD catheter via a manometer and indirect measurement via the bladder catheter to a transducer.MethodThis study was a secondary analysis of a previously published rand
