Surgical outcomes of liver resection for hepatoblastoma in infants versus older children: A bi-centric, propensity-score-matched, comparative study.

BACKGROUND AND AIM: Hepatoblastoma (HB) is the most common pediatric liver malignancy, with most patients undergoing surgical resection after neoadjuvant chemotherapy at an age older than one year. Infants (≤12 months) undergoing liver resection represent a distinct and less studied subgroup, considered more vulnerable and at increased surgical risk by some clinicians. This study aimed to compare perioperative outcomes between infants and older children undergoing hepatectomy for HB to determine whether surgery in infancy is associated with increased risks. METHODS: This bi-centric study incl

Outcomes of major liver resection for children with hepatoblastoma weighing less than 10 kg: avoiding liver transplantation with extended or extreme resection.

BACKGROUND: Hepatoblastoma is a rare disease and major hepatectomy in small children is technically challenging surgery due to small size of blood vessels, bile ducts, small liver volume, and the challenge of intraoperative monitoring. Detailed information about surgical outcomes of major liver resection in children with HB weighing < 10 kg are limited. The threshold of the future liver remnant (FLR) in children remains controversial and is not defined. We aim to report our institutional experience of major liver resection in children with HB weighing < 10 Kg. METHODS: A single center retrospe

Targeting ATR to overcome cisplatin resistance in hepatoblastoma.

BACKGROUND & AIMS: Hepatoblastoma (HB) is the most common pediatric liver cancer. While standard cisplatin-based therapy cures ∼80% of patients, resistance remains a major challenge, with no standard second-line treatment. This study aimed to identify new therapeutic options for cisplatin-resistant HB. METHODS: Fourteen HB cell lines and one pediatric hepatocellular carcinoma cell line were molecularly profiled. Eight cell lines were screened with 101 compounds and 58 combinations in monolayer cultures. The most synergistic combination was further validated in two low-passage patient-derived

Birth weight and neonatal morbidity as early-life risk factors for childhood cancer: a nationwide cohort study.

BACKGROUND: Childhood cancer is a significant public health issue, and growing evidence suggests that perinatal factors such as birth weight and neonatal complications may influence cancer risk. However, the potential impact of neonatal interventions on subsequent cancer development remains poorly understood. This study aimed to investigate the association between birth weight and childhood cancer risk, and how this relationship may be modulated by neonatal morbidities and interventions. METHODS: We conducted a nationwide population-based cohort study using the National Health Insurance Servi

Hepatoblastoma-Derived Exosomal FASN Affected the Differentiation of Hepatic Stellate Cells Into Cancer-Associated Fibroblasts by Regulating the Stability of HK2 and Mediating Histone Lactylation.

Cancer-associated fibroblasts (CAFs) interact with tumor cells in the tumor microenvironment (TME), enhancing glycolysis in CAFs and tumor malignancy. However, the regulatory mechanisms between hepatoblastoma (HB) cells and CAFs are unclear. This study aimed to elucidate the crosstalk mechanism between HB cells and CAFs and identify a new therapeutic target for HB. Exosomes were successfully extracted from Huh-6/HepG2 cells, and hepatic stellate cells (LX2) were treated with conditioned medium or exosomes from these cells. We found that HB cells may stimulate the differentiation of LX2 cells i

Identification of CD7 as a novel biomarker of embryonal hepatoblastoma.

PURPOSE: This study aimed to investigate the expression of cluster of differentiation 7 (CD7) in hepatoblastoma (HB) and its potential use as a novel biomarker of HB. METHODS: CD7 expression was investigated in human HB samples at the gene level by bulk, single-cell RNA sequencing, and spatial transcriptomic analyses, in addition to the protein level by immunohistochemical (IHC) staining. CD7 gene expression-based survival analysis was also conducted, along with gene set enrichment analysis (GSEA) of the CD7-SECTM1 receptor-ligand gene pair. RESULTS: CD7 was differentially expressed in human

Dinaciclib improves treatment response in chemoresistant hepatoblastoma.

Chemoresistant hepatoblastoma (HB) is associated with poor outcomes. Cyclin-dependent kinases (CDKs) are potential therapeutic targets because of their critical role in cell growth and chemoresistance. To evaluate the efficacy of CDK inhibition, HB cell lines were treated with dinaciclib and evaluated with cytotoxic, cell-death reversal, and immunoblotting assays. A HB patient-derived xenograft (PDX) model was treated with placebo, vincristine + irinotecan (VI), dinaciclib, or VI + dinaciclib to evaluate tumor growth and response to therapy. We found that dinaciclib had a marked effect on HB c

Multimodality Imaging Diagnosis of Metastatic Intracardiac Hepatoblastoma.

Metastatic intracardiac hepatoblastoma is an extremely rare entity. Metastasis pathways include lymphatic, hematogenous routes, direct, or transvenous spread. The multimodality imaging has enabled the identification of the etiology of intracardiac mass in many cases. The significant characteristics of indicating malignant cardiac tumors include irregular borders, vascular invasion, and rapid growth on echocardiography. Additionally, malignant cardiac tumors are characterized by heterogeneous enhancement, necrosis, and restricted diffusion on cardiac magnetic resonance (CMR).

Blastemal Tumours: Cytological Features and the Usefulness of Fine Needle Aspiration.

BACKGROUND: Blastemal tumours are quite frequent malignancies in childhood. In many oncological centres, fine needle aspiration is a part of the specific diagnostic procedure. In this review, the cytological features of the most common entities - i.e., neuroblastic tumours, nephroblastomas, retinoblastomas, and hepatoblastomas - are covered. SUMMARY: Blastemal tumours are composed of blastemal cells, which are frequently rounded or oval. This morphological similarity among different entities requires detailed clinical and radiological information for accurate diagnosis. Cytological specimens

Effective therapeutic targeting of CTNNB1-mutant hepatoblastoma with WNTinib.

Hepatoblastoma (HB), the most frequent pediatric liver cancer (2.16 cases/million), has surgery and perioperative chemotherapy as primary treatment, with severe lifelong side effects. This study evaluates the efficacy of the Wnt/CTNNB1 inhibitor WNTinib as a potential HB treatment, since CTNNB1 mutations occur in 70-90% of HBs. WNTinib's efficacy was assessed in three animal models (n = 48): (a) patient-derived xenograft (PDX) HB tumors (n = 5 CTNNB1-mutant, n = 1 CTNNB1 wild-type) implanted in NSG mice; (b) PDX-derived TT001- and (c) HepG2-HB cells subcutaneously implanted in Fox1nu mice; and

Maternal Autoimmune Disease and the Risk of Childhood Cancer Among the Offspring: A Population-Based Cohort Study in Taiwan.

OBJECTIVES: Autoimmune diseases (ADs) are characterized by a loss of immune tolerance to self-antigens. Maternal ADs are associated with pregnancy complications and are related to various diseases among their offspring. Existing studies have reported that maternal ADs increase the risk of certain types of malignancies in children. We aimed to investigate the association between maternal ADs and childhood cancer in Taiwan. METHODS: We established a population-based cohort study using the Taiwan Maternal and Child Health Database, which links parents to their children, and connected it to the T

Insights into the mechanisms of microRNAs in hepatoblastoma: from diagnosis to treatment.

Hepatoblastoma (HB) is the most common malignant liver tumor in children. Early diagnosis and effective treatment are crucial for improving the prognosis of children with HB. In recent years, microRNAs (miRNAs), an important class of noncoding RNA molecules, have been increasingly recognized for their key regulatory roles in the occurrence, development, and treatment of HB. This review systematically reviews the expression characteristics, molecular mechanisms, and potential application value of miRNAs in the diagnosis and treatment of HB. Research indicates that the interaction network betwee

Association of the YTHDF2 rs3738067 A>G variant with hepatoblastoma risk: a multicenter case-control study in Chinese children.

BACKGROUND: As a rare cancer, hepatoblastoma is the most prevalent malignant liver tumor in children, yet its underlying causes are poorly understood. YTHDF2, a canonical m⁶A reader, regulates mRNA stability and decay, and its dysregulation has been implicated in tumorigenesis across multiple malignancies. Whether YTHDF2 polymorphisms influence hepatoblastoma risk is unclear. This multicenter case-control study was therefore designed to analyze the YTHDF2 rs3738067 A > G variant in Chinese children. METHODS: We recruited 313 hepatoblastoma cases and 1446 controls from seven hospitals across Ch

Hepatoblastoma Cell Lines: Past, Present and Future.

Hepatoblastoma (HB), the most common pediatric liver malignancy, tends to be highly curable although advanced or recurrent disease has less favorable outcomes. Because patients are invariably <3-4 years of age, chemotherapies can cause significant long-term morbidities. Immortalized HB cell lines could be of great utility for drug screening, for the identification of novel therapeutic susceptibilities, and for studies requiring highly regulated and/or rapidly changing in vitro environments. However, HB research is hampered by a paucity of these lines that could be used for such purposes, with

Ultrasound-Boosted Liposomal Prodrug Overcomes Age-Associated Biodistribution Disparity in Pediatric Solid Tumor Therapy.

Clinical adoption of nanodrugs for pediatric cancer is hindered by age-associated disparities, with unclear mechanisms limiting nanotherapeutics for childhood solid tumors. Here, we originally find that the in vivo biodistribution of nanocarriers shows marked age differences: juvenile tumor-bearing mice have worse off-target distribution than adults, due to vigorous neovascular proliferation in normal tissues and heightened macrophage phagocytosis in liver and spleen. To address this challenge, an ultrasound-activated liposomal prodrug (CSCPTL) is designed to overcome age-related disparities i

Precision Oncology for Pediatric Solid Tumors Using In-Hospital Pediatric/AYA Malignancy-Specific Panel Sequencing.

Precision oncology, based on the genomic profile of tumors, is increasing in cancer treatment. It is important in pediatric and adolescent and young adult (AYA) malignancies, a group of rare cancers that are difficult to diagnose and progress quickly. We evaluated the usefulness of in-hospital molecular profiling using a specific sequencing panel for pediatric and AYA malignancies in clarifying the diagnosis, predicting the prognosis, and identifying therapeutic targets. We evaluated the Oncomine Childhood Cancer Research Assay (203 genes, 1700 fusions) and analyzed 153 samples at diagnosis an
