Hepatoblastoma is the most common primary hepatic tumor in young children and its radiological findings have been well described. We report ultrasound and computer tomography findings in a case of hepatoblastoma diffusely involving the entire liver and presenting as inhomogeneity and numerous cystic lesions in the hepatic parenchyma.

Hepatoblastoma is a rare but highly malignant liver tumour, predominantly occurring during early childhood. Clinical data of 71 children treated from 1988 to 1993 according to the protocol of the multicentre liver tumour study (HB89) of the German Society for Paediatric Oncology and Haematology were evaluated to assess the effects of surgical therapy in these patients. The protocol prescribed an initial laparotomy for all children with a liver tumour, except for infants with distant metastases and a high serum-alpha-fetoprotein; for small hepatoblastomas confined to one liver lobe primary rese

The wide range of epithelial and mesenchymal lines of differentiation seen in hepatoblastoma suggests that this tumor derives from a pluripotent stem cell. In order to test this hypothesis, seven hepatoblastomas of various subtypes were investigated for the presence of cells with the features of the oval cells found during hepatocarcinogenesis in rodents that are thought to be closely related to hepatic stem cells. The specimens were investigated by electron microscopy, electron microscopic immunocytochemistry, and immunohistochemical staining for cytokeratins nos. 7, 8, 18, and 19. Small epit

Nodular hyperplasias ("hyperplasiomas") are new formations whose development as a required and regulated response can be traced either to compensatory reactions to the loss of cells (regeneration in a narrow sense) and to decreased cellular performance, or to primary growth impulses. Included in this group are: the "macroregenerative nodules" after extensive cell losses; solitary nodules of uncertain etiology; and the minute foci of "micronodular transformation" whose origin can be traced to a particular disturbance of the hepatic blood supply. The so-called "adenomatous hyperplasias" of the c

Our patient was the first registered and operated case caused by multifocal hepatoblastoma, located in the right lobe. The decision to operate, our young patient, was a certain challenge, considering local diagnostic and surgical possibilities. The results of the two-years follow up (based on every three-months check-ups), level of the alpha-fetoprotein, showed very fast normal level, even it were no so high preoperatively. We also did not have any sign of local reoccurrence or long distance metastasis. Chemotherapy does not have to be necessary prerequisite of extension surviving, specially i

The ability to accurately assess tumor size and orientation to surrounding vital structures is an important consideration during preoperative evaluation. The authors report on nine children with solid tumors (hepatoblastoma [1], neuroblastoma [2], adrenal cortical carcinoma [2], liver adenoma [1], primitive neuroectodermal tumor [PNET] [1], and stage V Wilms' tumor [2]) for whom tumor resectability was questioned because of the tumors' close proximity to major blood vessels (noted through conventional radiographic imaging). The children had scanning with spiral volumetric acquisition computeri

The diagnosis of malignant liver tumors has long been an important part of surgical pathology, but now with improved radiographic and biopsy techniques, the appropriate pathological identification and differentiation of malignant from benign liver lesions has become even more critical. This review discusses five examples of malignant lesions that may be underdiagnosed as benign: well-differentiated hepatocellular carcinoma, fetal and macrotrabecular hepatoblastoma, well-differentiated cholangiocarcinoma, Kaposi's sarcoma, and epithelioid hemangioendothelioma.

Fatty acid ethyl esters are esterification products of fatty acids and ethanol. These compounds have been detected in the serum and cells of individuals following ethanol ingestion. Fatty acid ethyl esters can be quantitated by gas chromatography-mass spectroscopy (GC-MS) in the serum following ethanol ingestion and have been found in concentrations up to 42 microM. Fatty acid ethyl esters have also been isolated from adipose tissue of subjects ingesting fatty acid ethyl ester capsules as well as from subjects ingesting ethanol. HepG2 cells, a human hepatoblastoma cell line, have also been sho

BACKGROUND/AIMS: Fatty acid ethyl esters (FAEEs) are nonoxidative products of ethanol metabolism. They have been implicated as mediators of ethanol-induced organ damage because FAEE and FAEE synthase have been found specifically in the organs damaged by ethanol abuse. This study showed toxicity specifically related to FAEE or their metabolites for intact human hepatoblastoma-derived cells (HepG2).
METHODS: The lipid core of human low-density lipoprotein (LDL) was extracted and the LDL particle reconstituted with either ethyl oleate or ethyl arachidonate. Cultured HepG2 cells were incubated wi

An overview is presented of current management strategies in primary hepatic tumours in children. Recent advances include research in the genetic and molecular properties, especially of hepatoblastoma, the application of pre-operative chemotherapy, and the development of pre-surgical staging. Problem areas include management of relapse, chemotherapeutic drug resistance and multifocality. Hepatocellular carcinoma continues to have a much worse outcome than hepatoblastoma.

Azaspiranes are novel immunomodulators which are effective in a variety of autoimmune diseases. One azaspirane analog, SK&F 105685 (N,N-dimethyl-8,8-dipropyl-2-azaspiro [4.5] decane-2-propanamine dihydrochloride), caused a decrease in total serum cholesterol in dogs after oral administration. To determine whether an effect on cholesterol was common to this class of compounds, the immunomodulatory activity was compared with the cholesterol-lowering activity of six azaspirane analogs. The compounds were given to beagles at a dose of 1 mg/kg p.o. for 28 days, and the effect on serum cholesterol w

Two amine chelators, namely N,N'-bis(2-aminoethyl)-1,3-propanediamine-4 HCl (2,3,2-tetramine) and diamsar, were used at various levels to deplete copper (Cu) from cultured Hep G2 cells. For cells cultured at 0.63 mumol of Cu/L, maximal depletions were attained after 24 h of incubation with 10 mumol of either chelator/L, which resulted in an average significant depletion of 45% of cellular Cu. In addition, when cells were cultured continuously for four passages at 0.63 mumol Cu/L with 20 mumol of 2,3,2-tetramine/L, cellular Cu was significantly depleted more than 40% compared with controls. In previous studies, we found that male D2B6F1 mice fed phenobarbital (PB) for 53 weeks following N-nitrosodiethylamine (NDEA) initiation developed a high (70-80%) incidence of malignant hepatoblastomas. A very low (3.3%) incidence of such tumors occurred in the absence of promoter treatment in NDEA-initiated mice observed for 60 weeks, although nearly 50% of these animals developed hepatocellular lesions. To investigate whether hepatocellular lesions in NDEA-initiated mice or spontaneous hepatocellular lesions promoted by PB in mice given PB but no NDEA, progress to hepatoblastomas later in l

Starting from a case report of a six-month-old boy with a hepatoblastoma, we discuss this topic. New virologic and cytogenetic findings in hepatoma are mentioned in the chapter "epidemiology". In "diagnosis" we focus on history, physical examination, echography, conventional RX, the role of CT-scan of abdomen and thorax, of fine needle aspiration, arteriography and magnetic resonance. In "therapy" we emphasize the new therapeutical means: chemotherapy, orthotopic liver transplantation, resection of pulmonary metastases, as well as radiotherapy and immunotargeting chemotherapy. We conclude that

Between 1981 and 1993, 41 children were treated for hepatoblastoma. Clinical, radiological and pathological data were reviewed retrospectively, focusing on surgical aspects of treatment and outcome. Fourteen children underwent primary resection of the hepatic tumour. One infant with severe congenital anomalies received only palliative treatment. Of 26 with irresectable disease, pulsed cytotoxic chemotherapy (cisplatin and doxorubicin) enabled subsequent surgical excision in 22 and one child with persistent extensive intrahepatic disease was successfully treated by liver transplantation. Thus,
