Though the development of neoplasia is frequent with Beckwith-Wiedemann Syndrome its association with hepatoblastoma is extremely rare. Such a case in a fifteen month old child was studied in terms of its clinico-pathological features.

A child with stage 4 hepatoblastoma failed to respond to treatment with cisplatin and adriamycin. She then showed a response to carboplatin with complete clearing of pulmonary metastases. Bilobar liver disease persisted, although significantly reduced in size. A liver transplant was subsequently performed and she remains in complete remission 36 months later. After the first course of carboplatin, there was a dramatic rise in alpha-feto protein which then fell exponentially. Carboplatin warrants further study in hepatoblastoma.

The concentration of mRNA of insulin-like growth factor-II is (IGF-II) much elevated in some embryonic tumours such as Wilms tumour (nephroblastoma). In order to prove whether or not IGF-II is produced by the tumour tissue, IGF-II was extracted from freshly frozen tissue of Wilms tumour and hepatoblastoma. Normal adjacent tissue of kidney and liver was used as a control. The total IGF-II in Wilms tumour was 548.4 +/- 77.4 ng/g (n = 7) compared to 112.8 +/- 38.2 ng/g (n = 5) in kidney. Cytologic features of a rare case of small cell undifferentiated ("anaplastic") hepatoblastoma are presented. The tumor cells were primitive and closely resembled the group of "small blue cell" pediatric neoplasms--neuroblastoma, lymphoma, Ewing's sarcoma and rhabdomyosarcoma. This particular subtype of hepatoblastoma is the least common and is associated with the worst prognosis.

Thrombocytosis and fever are frequent symptoms in children with hepatoblastoma. Interleukin-6 (IL-6) has been shown to mediate thrombocytosis and an acute-phase reaction including fever. We therefore investigated samples from 14 untreated patients with hepatoblastoma for this cytokine and in addition for interleukin-1 alpha (IL-1 alpha), interleukin-1 beta (IL-1 beta) and tumor necrosis factor-alpha (TNF-alpha), all of which are known to induce IL-6 production. A 5' flanking region of the guinea pig liver transglutaminase gene was cloned and sequenced. The sequences for TATA box and potential binding sites of some regulatory factors were found in this region. We examined the effect of interleukin-6 (IL-6) on the expression of transglutaminase in human hepatoblastoma HepG2 cells. Treatment of cells with IL-6 increased their transglutaminase activity in a time- and dose-dependent way. Dexamethasone strengthened the stimulation by IL-6. Expression of the LAC9 gene from the yeast Kluyveromyces lactis in HepG2 human hepatoblastoma cells efficiently induced luciferase expression from reporter plasmids containing the four LAC9 binding sites from the K. lactis GAL1-GAL10 gene linked to a basal promoter. Induction was approximately 100fold and was dependent on the presence of the UAS sequence and an intact reading frame in the LAC9 gene.
