The hepatic transcription of the angiotensinogen gene is regulated by both glucocorticoids and cytokines generated as products of the acute phase reaction. Immunohistochemical staining of cell lines derived from human liver tumours showed that five cell lines derived from hepatocellular carcinoma (HCC) and hepatoblastoma were stained positively with monoclonal keratin antibodies, CK-5 (Ker-18-specific) and KL-1 (broad specificity), but not with CK-7 (Ker-7-specific). Hyperferritinemia, an unclear mechanism, is frequently observed in chronic alcoholics. The aim of this work was to study the effect of alcohol on ferritin expression in a human hepatoblastoma cell line, HepG2. This cell line proved to be sensitive to alcohol, since alcohol increased gamma-GT activity both in cells and media. The authors present an account of 37 children with primary hepatic tumours at the age of 1 to 16 years. The mortality in the group was 50%. Right-sided lobectomy on account of a hepatoblastoma was performed in 13 children, probatory excision on account of an advanced process in 9, left-sided lobectomy only in five children. Identical male twin infants who in 1987 presented with hepatoblastoma at the age of 7 months are reported. Twin B was admitted for investigation due to enlarged liver and spleen. He was found to have an inoperable hepatoblastoma of fetal type and was treated with chemotherapy and local irradiation. However, 7 months after the chemotherapy the tumor recurred. The asymptomatic twin A was examined because of the known familial cases, and he proved to have an identical tumor. He was successfully radically operated on and 2 years after the chemotherapy is still doing well with no evidence of tumor.

Owing to the recent progress of chemotherapy, a concept of surgical intervention to the malignant tumor in childhood needs to be changed. Data from 31 children, 23 females (74.2%) and eight males (25.8%) with precocious puberty were analysed. They included 19 cases with central precocious puberty, 5 with premature thelarche, 5 with premature adrenarche and 1 each with adrenal adenoma and virilizing hepatoblastoma. Their clinical and hormonal profile and the treatment outcome are briefly discussed.

33 patients with liver malignancy underwent transplantations. The overall survival rate 1, 2 and 5 years after transplantation was 42.4%, 30% and 24.2%, respectively. These figures agree with those of other centres. 16 patients with primary hepatocellular carcinoma had a median survival of 2 years and 2 months and a 5-year-survival rate of 37.5%, the longest followup being more than 11 years. 10 patients with cholangiocellular carcinoma had a 5-year-survival rate of only 10%, the longest followup being more than 6 years. The authors report a case of primary osteosarcoma of the liver in a 75 year-old man with post hepatitic B cirrhosis. The hepatic tumor was discovered on ultrasound and computed tomography. Angiography showed a hypervascularized tumor. The patient died rapidly. Diagnosis of primary osteosarcoma of the liver was established through histopathological examination of post mortem specimens and was confirmed by immunohistochemical study. From this and two other previously reported cases the authors describe the different primary hepatic tumors mimicking osteosarcoma.

Clinicoroentgenological evidence is described for 3 patients with nonresectable hepatic angiosarcoma. Two of these underwent therapeutic embolization of hepatic artery. Differential diagnosis between angiosarcoma and other hepatic malignancies by angiographic images seems impossible. Hepatic artery embolization can be employed as palliative treatment of advanced angiosarcomas.

The Liver Cancer Study Group of Japan analyzed statistically 12,887 cases of primary liver cancer diagnosed from January 1, 1982 to December 31, 1985 in more than 500 institutes throughout the country.
