Magnetic resonance (MR) imaging was performed in 10 children with intrathoracic disease. Entities imaged included mediastinal lymphoma and neuroblastoma, pulmonary metastases from hepatoblastoma and osteogenic sarcoma, hemorrhagic pleural effusion, and nonmalignant disease such as abscess, cystic fibrosis, and bronchogenic cyst. MR imaging was particularly valuable in delineating mediastinal and parenchymal masses from adjacent vascular structures without the need for contrast material enhancement. Ultrasound imaging and CT scans were performed in a homogeneous group of five patients with hepatoblastoma. Effectiveness in determining characteristics of the tumor mass and its location and possible vascular extension were compared for the two exploratory methods. Findings suggest that these two examinations are useful and complementary for the diagnosis and follow up of hepatoblastoma.

A 15-month-old girl had a case of mixed hepatoblastoma. The unresectable tumor became surgically resectable after treatment with doxorubicin hydrochloride and cisplatin, which was also continued postoperatively. The effect of diethylstilbestrol (DES) on sister chromatid exchange (SCE) induction was measured in four cell lines to determine whether metabolic activation of DES is a factor in its genotoxic potential. There is no definitive treatment of hepatoblastoma. Except for radical resection, the prognosis of unresectable cases remains pessimistic. Therefore, the development of new anticancer drugs for this tumor is urgently needed. Using the human hepatoblastoma cell line (c-HB-3) we demonstrated that horse anti-alpha-fetoprotein containing serum suppressed proliferation of the hepatoblastoma cells. This effect was dependent on the duration of exposure and the concentration of anti-alpha-fetoprotein serum.

The Liver Cancer Study Group of Japan statistically analyzed 2396 cases of primary liver cancer diagnosed from January 1, 1978 to December 31, 1979 in over 500 hospitals throughout the country. Hepatoblastoma is the third most common intraabdominal malignant neoplasm in infants. Since 1977, eight children have had major liver resections with the aid of normovolemic hemodilution anesthesia. A two year old boy presented with precocious puberty associated with hepatoblastoma. Serum concentrations of beta human chorionic gonadotrophin and alpha-fetoprotein were raised. An aggressive chemotherapeutic regimen resulted in useful palliation and interesting changes in the beta human chorionic gonadotrophin and alpha-fetoprotein concentrations. Since 1950, 48 infants and children from 10 weeks to 16 years of age presented with primary hepatic malignancy. Signs and symptoms ranged from asymptomatic to those of malignant disease.
