Six main types of histopathological changes were found in 463 patients with chronic alcoholism admitted during the 10-year period from 1966 to 1975: group I, normal liver in 2.6%; group II, fatty liver in 8.4%; group III, acute alcoholic hepatitis (AAH) in 7.6%; group IV, cirrhosis with or without steatosis in 68.7%; group V, cirrhosis with AAH in 12.8%; group VI, liver cell carcinoma (LCC) in 1.9% (all of the latter patients were also included in group IV). Aflatoxin B1, B2, G1, G2 levels were determined in sera of 20 patients with primary liver cell carcinoma (CC). Values of B1 above 0.15 microgram/ml were obtained in sera of three patients. Hepatitis B surface antigen (HBsAg) was detected in only 35% of patients. 50-90% of serum levels were cleared in 24 h in three patients studied. Although it is inconclusive as to the carcinogenic potential of aflatoxins in adult man, foodstuffs should be protected from excessive aflatoxin contamination.

Post mortems of 2289 patients with liver cirrhosis revealed that 37% died of causes related directly to cirrhosis i.e. liver failure or hemorrhage from oesophageal varices. Patients who died of ruptured oesophageal varices in 82% have splenomegaly and most frequently hepatomegaly. Livercell carcinomas were recognized in 7,5% of cirrhotic livers, increasing to 10% in 1976-1978. This rise particularly is conspicuous in women. Liver carcinoma is 4,5 times more frequent in postnecrotic than in portal cirrhosis.

Serum activities of two lysosomal enzymes, beta-glucuronidase and acid phosphatase, were estimated in 66 patients with liver cell carcinoma, 10 with secondary liver cancer, 14 with cirrhosis of the liver, and 9 normal controls. We compared the presence of serologic markers of hepatitis B virus (HBV) infection with the presence of the viral DNA in the livers of patients with alcoholic liver disease with or without hepatocellular carcinoma. The distribution of hepatitis B surface antigen (HBsAg) and core antigen (HBcAg) in surgically removed liver cell carcinomas of 60 Japanese patients was studied by an immunohistochemical method. Cystic hepatoblastomas share ultrasonographic characteristics with simple hepatic cysts; however, a distinction can be made by the demonstration of internal septations. The differentiation is important because cystic hepatoblastoma may have malignant potential. The ultrasonographic and scintigraphic findings in five children with cystic hepatoblastoma are reviewed.

An observation of congenital hepatoblastoma of a mixed type in a baby dying on the 50th day of life is described. The boy was born with a metastasis of the tumour into the left superciliary area. The process of hepatoblastoma metastasising had begun in the intrauterine period. The main and metastatic nodes of the tumour consisted of epithelial and mesenchymal components. The multicomponent structure of the tumour attests to its origin from a pluripotent blastoma like Wilms tumour. In addition to the hepatoblastoma with disseminated metastases the boy had malformations in the liver and kidneys.

A review of the literature shows minimal success in relation to the treatment of hepato-blastoma. Recent improvements have been related to better techniques of liver resection. No final therapeutic recommendations can be made but prophylactic chemotherapy is indicated.
