A case of thorotrastosis occurred 25 years after thorotrast angiography, with the previously unrecorded association of myeloma type paraproteinaemia. The relationship between haemangioendothelioma due to thorotrast and other vascular sarcomas of the liver is briefly reviewed.

Virilizing hepatoblastoma was found to be the cause of precocious puberty in a 3 1/4-year-old boy. Both human chorionic gonadotropins and alphafetoprotein in increased amounts were detected in serum and tumor tissue. These substances disappeared from the blood after removal of the tumor. Ultrastructure studies revealed the presence of dense, membrane bound, secretory granules responsible for the hormone production.

Two cases of hemangiosarcoma of the liver and spleen are described. In the first case, a great number of cavernous blood vessels has been found and this, according to the authors, may support the idea that the tumour aroses within the benign hemangioma. In the other case a thrombocytopenia, a rare complication of the liver hemangioblastoma, has been found. It should be stressed that in spite of a malignant nature of these tumours, neoplastic foci were limited only to the liver and the spleen.

Vascular complications occurring late after exposure to thorium dioxide (thorotrast) are described in two patients. One patient had both cerebral andmyocardial infarcts and died at age 25 years. Necropsy disclosed both adventitial and intimal fibrosis of the left carotid artery and greater than 75 per cent cross-sectional area luminal narrowing of both the left main and left anterior descending coronary arteries. Hepatoblastoma is a primary embryonic liver tumour usually found in children. It extremely rarely occurs in adults and, when it does, is usually found in men, although it has been postulated that in women it may be associated with use of the oral contraceptive pill. We present a case of a 51-year-old White woman with no history of oral contraception. The features of this tumour are reviewed.

Ther serum concentration of alpha-fetoprotein (AFP) was measured by radio-immunoassay in 98 patients with liver disease including hepatoma, chronic active hepatitis, alcoholic cirrhosis, and acute virus B hepatitis. Raised AFP levels, above 30 ng/ml, were found in 87% of patients with acute viral hepatitis, in 82% of patients with primary liver cell carcinoma, in 58% with chronic active hepatitis and in 14% of patients with alcoholic cirrhosis. However, levels above 1 000 ng/ml were found only in patients with hepatoma and in acute viral hepatitis.

This memorandum provides guidelines on the definition, nomenclature, and classification of cirrhosis, chronic hepatitis, and hepatic fibrosis. These are considered according to morphological characteristics and aetiology. It is hoped that this system will serve as a standard for diagnostic, research, and epidemiological purposes. The relationship of cirrhosis to liver cell carcinoma is briefly discussed and the possible morphological markers of an increased risk of malignancy are defined.

A group of patients with only moderately active chronic hepatitis has been studied. The follow-up was long (mean 87 months). All patients except one were treated with prednisone and/or azathioprine. Of the hepatitis B virus positive patients two-thirds developed cirrhosis between the second and fifth year of evolution, while in the hepatitis B negative group this occurred in less than one-third. The transition to cirrhosis was clinically silent. The patients were all allowed to do their normal work except in the terminal stages of cirrhosis.
