A case report of fatal chlormequat poisoning: Identification of potential additional mechanism of toxicity.

Chlormequat is a quaternary ammonium salt used as a plant growth-regulating agent. It is structurally similar to choline, acting at muscarinic and nicotinic receptors. Herein, we present the case of suicidal poisoning with an unknown dose of chlormequat by a 45-year-old woman. Initially, carbofuran was suspected of ingestion. Following ingestion, circulatory arrest occurred. After hospital admission, gastric lavage was performed, and activated charcoal was administered. Initial toxicological analysis identified mild alcohol intoxication, while high-resolution mass spectrometry later discovered

Suicide or not? Issues in the demonstration of anaphylaxis, a review of the literature.

The authors review the literature on the determination of post-mortem serum tryptase values and present the case of a young man who was hit by a train. However, his family believes he has no motivation to commit suicide. Collision with a train is one of the most common methods of suicide, especially among young men under 40 years of age. (1). The forensic autopsy showed that the man died due to the collision with the train, with traumatic hemorrhagic shock stated as a cause of death. Following toxicological, biochemical, and immunological tests created a supposition that the incident was not a

Deployment of Alternative Response Units in a High-Volume, Urban EMS System.

Faced with increasing demand for their services, Emergency Medical Services (EMS) agencies must find more efficient ways to use their limited resources. This includes moving beyond the traditional response and transport model. Alternative Response Units (ARUs) are one way to meet the prehospital medical needs of some members of the public while reducing ambulance transports. They are non-transport vehicles tasked with very specific medical missions. These can include acute management of low-acuity complaints, ongoing home care for chronic medical conditions, preventive medicine, and post-hospi

Deaths Associated With Brotizolam Poisoning From a Single Drug Overdose: Four Reported Cases.

Brotizolam is a short-acting hypnotic in the benzodiazepine family, and fatal poisonings by an overdose of brotizolam are rare. This report describes 4 cases of deaths associated with brotizolam poisoning from a single drug overdose. The ages ranged from 51 to 90 years, and the postmortem interval between death and tissue sampling was 1.5 to 2.5 days. These deaths were classified as 1 homicide and 3 suicides. The concentration of the brotizolam ranged from 0.05 to 0.21 mg/L in the blood samples. Ethanol, which could cause mild alcohol intoxication, was detected in the blood samples from 2 case

Creativity on tap? Effects of alcohol intoxication on creative cognition.

Anecdotal reports link alcohol intoxication to creativity, while cognitive research highlights the crucial role of cognitive control for creative thought. This study examined the effects of mild alcohol intoxication on creative cognition in a placebo-controlled design. Participants completed executive and creative cognition tasks before and after consuming either alcoholic beer (BAC of 0.03) or non-alcoholic beer (placebo). Alcohol impaired executive control, but improved performance in the Remote Associates Test, and did not affect divergent thinking ability. The findings indicate that certai

Influence of acute alcohol ingestion on the hormonal responses to modest hypoglycaemia in patients with Type 1 diabetes.

AIMS: To determine whether mild alcohol intoxication (45-50 mg/dl) influences counterregulatory hormone responses to moderate hypoglycaemia (2.8 mmol/l)in patients with Type 1 diabetes. METHODS: Seventeen subjects (14 male, age range 21-46 years) with Type 1 diabetes underwent four hyperinsulinaemic glucose clamps: euglycaemia with placebo; euglycaemia with alcohol (0.4 g/kg); hypoglycaemia (2.8 mmol/l for 65 min)with placebo; and hypoglycaemia (2.8 mmol/l for 65 min) with alcohol (0.4 g/kg). Arterialized venous blood samples were taken for measurement of insulin and counterregulatory hormone

Cerebral blood flow and dynamic cerebral autoregulation during ethanol intoxication and hypercapnia.

More than one-third of patients diagnosed with head injury are intoxicated with ethanol. Most clinical and animal studies have shown alcohol to have a deleterious impact in the setting of cerebrovascular trauma; however, there are also data showing neuroprotective effects in low ethanol doses. Human studies using imaging modalities suggest that small doses of alcohol produce cerebral vasodilatation and higher doses cerebral vasoconstriction. The aim of this study was to investigate the effect of ethanol intake on dynamic cerebral autoregulation and velocities in the middle cerebral arteries, a

Reduced binocular depth inversion in patients with alcoholism.

Binocular depth inversion represents an illusion of visual perception, serving to invert the perception of implausible hollow objects, e.g. a hollow face into a normal face. Such inversion occurs frequently, especially when objects with a higher degree of familiarity (e.g. photographs of faces) are displayed. Cognitive factors are assumed to override the binocular disparity cues of stereopsis. The hypothesis was tested that during mild and moderate alcohol withdrawal, and severe and mild alcohol intoxication, the central nervous system is unable to correct implausible perceptual hypotheses. Acute, withdrawal, and chronic alcohol effects in man: event-related potential and quantitative EEG techniques.

The results presented here strongly suggest that quantitative electroencephalography and event-related potentials are excellent research tools and may be clinically useful as non-invasive monitors of psychotropic drug action and encephalopathies. Our initial data with acute mild alcohol intoxication show that acute tolerance may be reflected in qEEG but not in P3 latency. Since predictably some brain functions may show tolerance, and others not, these approaches may be useful probes. The amplitude of N1-P2 appears to differentiate alcoholics with and without a history of withdrawal seizures. T

Cardiovascular effects of acetaldehyde accumulation after ethanol ingestion: their modification by beta-adrenergic blockade and alcohol dehydrogenase inhibition.

Left ventricular function was examined by echocardiography and systolic time intervals in nine healthy male volunteers, who ingested ethanol 0.35 g/kg 4 hr after a 50-mg peroral dose of calcium cyanamide, an aldehyde dehydrogenase inhibitor. Accumulation of acetaldehyde in blood was accompanied by marked increases in heart rate (53%) and cardiac output (78%) as well as by decreases in diastolic arterial blood pressure (19%) and peripheral vascular resistance (46%). Ejection fraction and maximum circumferential fibre shortening velocity increased by 25 and 47%, respectively; the preejection per

Effects of naloxone on ethanol and acetaldehyde-induced electroencephalographic changes in rabbits.

Rabbits treated with a single IV dose of ethanol, 0.6 g/kg, exhibited rapid EEG synchronization and behavioral changes that were reversed and, in part, prevented by the opiate antagonist, naloxone, 40 micrograms/kg. The continuous intracerebroventricular infusion of acetaldehyde, 10 or 120 micrograms/min. caused biphasic changes with EEG activation and severe bradycardia followed by overt synchrony during the post-infusion period. The results of the present study provide further evidence for the ability of naloxone to counteract some acute effects of ethanol. Moreover, they do not support a ro

[Intoxication at the wheel].

A large body of experimental and statistical evidence demonstrates that even mild alcohol intoxication with blood alcohol concentrations above 0,5% is associated with a significantly increase in the susceptibility for an accident. The lowering of the critical concentration limit is indicated from a medical, social-political and economical point of view.
