Oral supplementation of Pediococcus pentosaceus SMM847 ameliorates hyperuricemia in rats and alcohol-induced injury in mice.

UNLABELLED: Excessive dietary purine intake can lead to hyperuricemia, facilitating the precipitation and deposition of monosodium urate crystals in joints, which subsequently triggers inflammatory cascades leading to gout. Alcohol consumption exacerbates this condition by accelerating purine metabolism and impairing renal uric acid excretion, thereby further increasing serum uric acid concentrations and aggravating gout severity. In this study, Pediococcus pentosaceus SMM847 was screened based on its inosine degradation efficiency. Notably, the strain SMM847 also exhibited alcohol dehydrogena

Diagnostic correction based on toxicology: A case of combined dichlorvos (DDVP), ethanol, and pesticide co-solvent intoxication.

Organophosphate pesticides (OPs) are commonly encountered toxic substances in poisoning incidents, often associated with severe clinical symptoms. The diagnosis and treatment become more complicated, especially when mixed with other toxic substances, requiring detailed toxicological analysis to identify the involved agents. This report presents a case of mixed poisoning in a 37-year-old male who ingested 250 mL of 40% alcohol and was admitted with impaired consciousness, absent pupillary light reflexes, and severe metabolic acidosis, which did not improve with initial treatment. Electric scooter-related traumatic brain injuries: a scoping review of injury patterns, risk factors, management, and clinical outcomes.

OBJECTIVE: Electric scooters (e-scooters) have rapidly gained popularity in urban environments worldwide. While promoted as sustainable and convenient, their widespread adoption has led to a rise in injury rates, particularly traumatic brain injuries (TBIs). Despite increasing recognition of e-scooter-related head trauma, the literature remains fragmented and lacks focused synthesis. METHODS: A comprehensive search of PubMed, Embase, Scopus, Web of Science, and Cochrane databases was conducted in March 2025. Studies were included if they reported on e-scooter-related TBIs in real-world settin

Delayed Traumatic Subarachnoid Hemorrhage Following Initially Negative Computed Tomography (CT) Imaging in a Patient With a Mechanical Mitral Valve and Markedly Elevated International Normalized Ratio (INR).

Warfarin remains the anticoagulant of choice for patients with mechanical heart valves because of its effectiveness in preventing thromboembolic events. However, management becomes particularly challenging when severe supratherapeutic anticoagulation is complicated by intracranial hemorrhage. We present the case of a 76-year-old male patient with a mechanical mitral valve on chronic warfarin therapy who presented after a ground-level fall with head trauma and an admission international normalized ratio (INR) of 9.0. Initial computed tomography (CT) of the head demonstrated only a scalp hematom

Protective efficacy and related mechanism of a new ADH activating peptide YRPG from Lactococcus lactis against acute alcohol-induced injury.

Excessive alcohol intake can cause severe acute alcohol injury (AAI). The alcohol dehydrogenase (ADH)-activating peptide is an effective means to alleviate this injury. This study investigated the protective effects of the ADH-activating peptide YRPG from Lactococcus lactis against AAI and further elucidated its underlying mechanism, while also assessing its stability. Results demonstrated that at a concentration of 10 mmol L-1, YRPG achieved an in vitro ADH activation rate of 48.05% ± 1.11%. Notably, YRPG exhibited high gastrointestinal stability (81.61% ± 1.13%) and predominantly accumulated

Simultaneous quantification of ceramides and sphingosine-1-phosphate in serum by LC-MS/MS with application to alcohol intoxication cases.

Ceramides and sphingosine-1-phosphate (S1P) are bioactive sphingolipids critically involved in the regulation of cell survival and death, whose homeostasis is disrupted by alcohol exposure. A liquid chromatography-tandem mass spectrometry (LC-MS/MS) method was developed and validated for the simultaneous quantification of nine ceramide species and S1P in serum. Following protein precipitation, analytes were separated on a CSH C18 column within an 11-min chromatographic run. Calibration curves were linear over 5-500 ng/mL with R2 greater than 0.99 for all analytes. Intra- and inter-day accuracy

Acute intoxication and trauma severity in emergency department patients: a systematic review.

BACKGROUND: Every day, trauma patients under the influence of alcohol and/or psychotropic drugs are treated in Emergency Departments (EDs). Psychoactive substances alter cognition, judgment, and behavior and may influence injury severity, clinical management, and outcomes following trauma. Although numerous observational studies have examined intoxication among trauma patients, only a limited number of reviews have synthesized evidence on its association with injury severity and clinical outcomes. OBJECTIVE: To evaluate the association between acute alcohol and/or psychotropic drug intoxicati

Orbital and Ophthalmologic Injuries Associated With Standing Electric Scooter Accidents: A Narrative Synthesis of Clinical Outcomes and Injury Patterns.

Standing electric scooters (e-scooters) have introduced a distinct pattern of facial trauma to emergency departments worldwide. Their unique biomechanics, with riders standing upright on a narrow platform, creates a characteristic forward pitching mechanism during sudden stops that directs high-energy impacts to the anterior midface and orbital region. This differs fundamentally from bicycle accidents, where lateral falls and arm bracing are typical. This review synthesises available clinical evidence on orbital and ophthalmologic injuries from electric scooter accidents. Twelve primary studie

External Validation of a Prognostic Model for Outcome After Mild Traumatic Brain Injury at 6 Months Post Injury.

BACKGROUND: While most people fully recover after mild traumatic brain injury (mTBI), a substantial minority experience persistent symptoms and incomplete recovery. This may be prevented by early interventions specifically targeting patients at risk of incomplete recovery. The UPFRONT-model was developed to identify patients at risk of poor functional outcome. This study aimed to externally validate the UPFRONT-model in an independent sample. METHODS: A prospective, longitudinal, multicenter cohort study with 126 mTBI patients recruited from emergency and neurology departments (ED) of six hos

The impact of alcohol on fall-related maxillofacial fractures: a retrospective study.

The aim of this retrospective study was to examine the characteristics of patients sustaining maxillofacial fractures due to falls at ground level. The specific aims were to analyse the severity of the fractures and the time interval between the accident and diagnosis (delay of diagnosis) in alcohol-intoxicated vs non-intoxicated patients. A total of 1200 consecutive adult patients diagnosed with maxillofacial fractures at a tertiary trauma centre over a 7-year period were included. Outcome variables were the delay of diagnosis and severity of the fracture; the latter was determined based on a

Impact of altered consciousness on the risk of fasciotomy for acute compartment syndrome following traumatic tibial fracture.

BACKGROUND: Acute compartment syndrome (ACS) following tibial fracture is a limb-threatening emergency that relies heavily on clinical assessment for diagnosis. In patients with altered consciousness (hereafter referred to as obtunded status), key symptoms such as pain out of proportion cannot be reliably assessed, potentially influencing surgical decision-making. Whether obtunded status independently affects fasciotomy rates remains poorly understood. OBJECTIVE: To determine whether obtunded status is independently associated with increased likelihood of fasciotomy for suspected ACS in patie

Post-traumatic Exacerbation of Grade II L5-S1 Spondylolisthesis Requiring Surgical Stabilization.

Spondylolisthesis is characterized by the forward movement of one vertebra over the one beneath it, most frequently occurring at the L5-S1 level due to its biomechanical susceptibility. Although often asymptomatic, it may become clinically significant when underlying instability is exacerbated by external factors. Low-energy trauma can unmask previously undiagnosed spondylolisthesis, resulting in acute pain, neurological symptoms, and functional impairment. We report the case of a 51-year-old woman with a history of hypertension treated with atenolol, with no prior medically documented history
