What is the best approach for parenteral sedation to manage severe acute behavioral disturbance in the emergency department?

INTRODUCTION: Patients with severe acute behavioural disturbance commonly present to the emergency department. Differing expert opinion dominates treatment strategies. We describe an evidence-based approach to parenteral sedation for the management of emergency department patients with severe acute behavioural disturbance. APPROACH TO MANAGING SEVERE ACUTE BEHAVIOURAL DISTURBANCE WITH PARENTERAL SEDATION: The most common cause of severe acute behavioural disturbance in the emergency department setting is alcohol and drug intoxication, both being relatively short-lived. The goal of parenteral

Fatal Free Falls: A Clinical and Forensic Analysis of Skeletal Injury Patterns Using PMCT and Autopsy.

Background: Free fatal falls (FFF) are a frequent occurrence in forensic medicine. Many variables, such as the victim's sex, BMI, intoxication, height of the fall, and mental illness, can influence injury patterns. Previous studies identified fracture patterns and frequencies mostly with general anatomical detail, focusing on broad areas. As specific fractures might be roots for new statistical connections, this leaves a gap in our understanding. Using postmortem computed tomography, we aim to establish fracture frequencies and identify possible new statistical connections. Methods: In total,

Lessons learned from the outbreak: the use of fomepizole in children with acute kidney injury due to ethylene glycol/diethylene glycol poisoning.

Ethylene glycol (EG) and diethylene glycol (DEG) intoxication, defined as > 20 mg/dL in plasma, can cause severe morbidity and mortality due to their metabolism into toxic metabolites via alcohol dehydrogenase (ADH). In August 2022, Indonesia experienced a surge in acute kidney injury (AKI) cases in children, initially misdiagnosed as multisystem inflammatory syndrome in children (MIS-C). Following a World Health Organization (WHO) alert on EG/DEG-related AKI in Gambia, toxicology screening confirmed EG/DEG contamination in syrup-based medications. This study evaluates the efficacy of fomepizo

A Whiff of Garlic, a Turn of Fate: Recognizing Organophosphorus Poisoning Amid Diagnostic Uncertainty: A Case Report.

OP compounds, commonly used as pesticides, are readily available over the counter and frequently implicated in intentional poisoning. We report a case of a 43-year-old male with OP poisoning initially misdiagnosed as alcohol intoxication. He was intubated in the emergency department on arrival due to a low Glasgow Coma Scale (GCS) of 6/15 and then admitted to the Intensive Care Unit (ICU) where he developed 5 episodes of generalized tonic clonic seizure (GTCS) and ventilator-associated pneumonia (VAP), progressing to severe acute respiratory distress syndrome (ARDS). The patient required reint

E-bikers at risk for severe traumatic brain injury and skull fractures.

BACKGROUND: The popularity of electric bicycles (E-bikes) in The Netherlands has surged in recent years. Simultaneously, bicycle-related traffic injuries in The Netherlands have reached record levels. Given the significant societal and individual impact of traumatic brain injury (TBI) we investigated the relationship between E-bike usage and the occurrence of severe TBI. METHODS: All bicycle crash victims aged twelve years and older admitted to the Isala Hospital from 1 January 2018 to 31 December 2022, were included from the National Trauma Registry. Data on bicycle type, anticoagulants, alc

Role of blood alcohol level on the risk and severity of trauma recurrence.

BACKGROUND: Alcohol intoxication at the time of index trauma is associated with an increased risk of recurrent traumatic injury. It is unclear, however, whether the degree of intoxication impacts the risk of recurrence or its severity. This study aimed to analyze the relationship between alcohol level at index trauma and risk of recurrent trauma. We hypothesized that increasing levels of alcohol would be associated with an increased risk of trauma recurrence and severity. METHODS: We conducted a retrospective cohort study of adults who presented to our Level 1 trauma center between January 20

Chromosome malsegregation induced by methanol in Drosophila melanogaster females.

Methanol (MeOH) is a colorless, flammable, poisonous, alcohol that causes intoxication by ingesting, inhaling or by contact with formulations that include it. It is produced in large volumes and there is high level of human exposure, especially by the inhalation route. It has been reported as innocuous in various test systems; thus, the aim of this work was to search for in vivo genotoxic effects of MeOH in Drosophila melanogaster, studying its ability to induce nondisjunction (ND) in females. Treatments were acute (60 min) and via inhalation. It was toxic in 1-day old flies (MI = 63 % for 75

Impact of Illicit Drug Use on Facial Fracture Patterns and Hospital Resource Utilization.

INTRODUCTION: Facial trauma is a public health concern, with reports of about 25% of all traumas having a component of facial injury. Alcohol and illicit drug use have previously been shown to exacerbate the severity of trauma injuries. This study investigates the relationship between illicit drug use and clinical outcomes amongst patients presenting with facial fracture injuries. METHODS: A retrospective multicenter cohort study through the TriNetX database was performed. Drug Involved Facial fracture injury with illicit drug use (DIFFI+) was compared to a Non-Drug Involved Facial Fracture I

Acute Alcohol Intoxication and Chronic Alcohol Use Increase Risk of Infection After Open Tibia Fractures.

BACKGROUND: Alcohol use is a known risk factor for impaired wound healing. This study evaluated the impact of acute and chronic alcohol use on fracture-related infection (FRI) incidence in open tibia fractures. MATERIALS AND METHODS: A retrospective review of skeletally mature patients with open tibia fractures at a level I trauma center from 2017 to 2021 was conducted. Six-month follow-up was required. Missing blood toxicology results were excluded. Ethanol levels >10 mg/dL on admission defined acute alcohol use, and daily alcohol use or documented history defined chronic alcohol use. The pr

Characteristics of Traumatic Lower Extremity Amputation: Medical and Social Comorbidities.

Background Traumatic amputations are life-altering injuries that remain prevalent despite the best efforts of physicians. While a multitude of research demonstrates various medical comorbidities that contribute to amputations, little attention has been paid to specific factors linked to traumatic amputation, even though trauma is the second most common etiology of amputation. The aim of this study was to determine whether any underlying medical conditions previously associated with non-traumatic amputation in the literature may also be associated with increased rates of amputation in cases of

Beyond the Bruise: Unmasking Disseminated Intravascular Coagulation in Alcohol-Related Presentations.

Disseminated intravascular coagulation (DIC) is a rare but life-threatening complication encountered in the emergency department (ED). It results from widespread activation of coagulation pathways, leading to simultaneous thrombosis and consumption of clotting factors, often culminating in severe bleeding and multi-organ failure. We report the case of a 39-year-old female with alcohol-related cirrhosis and autoimmune hepatitis who presented to the ED with a minor head injury and active bleeding from a groin ulcer. Her initial evaluation was confounded by alcohol intoxication and suspicion of d

A Case of Black Esophagus in a Patient Presenting With Dyspnea.

Acute esophageal necrosis (AEN), also known as "black esophagus," is a rare clinical condition marked by severe damage and death of the tissue lining the esophagus. During endoscopy, AEN typically presents with a blackened appearance. Risk factors for AEN include esophageal ischemia in the setting of multi-organ dysfunction, sepsis, hypoperfusion, vasculopathy, traumatic transection of the thoracic aorta, thromboembolic phenomena, gastric volvulus, diabetic ketoacidosis, alcohol intoxication, or malignancy. Here, we report a case of AEN diagnosed in a patient presenting with dyspnea and diabet

A Case of Severely Impaired Consciousness and Convulsions From Attempted Suicide by Ingesting Triflumizole Emulsion: Clinical Features and Literature Review.

Triflumizole (TFZ) is a fungicide widely used in agriculture to prevent fungal infections of fruits and vegetables. Although it is considered safe for humans and animals, its toxicity profile in humans remains largely unexplored. Here, we describe a case where an individual experienced symptoms suggestive of intoxication after ingesting TFZ emulsion. A 70-year-old man ingested TFZ emulsion (Trifumin emulsionTM) and alcohol in an attempt to commit suicide. He developed a severe disturbance of consciousness, which was not explained by the estimated blood alcohol concentration, and experienced co
