Among comas of toxic origin, in children, alcoholic coma due to accidental poisoning is uncommon compared with comas due to drugs or household products. It is however important to make an early diagnosis, for this is a severe from of poisoning, liable to cause irreversible cerebral lesions if not treated very quickly. It almost always causes a flask coma, without localising signs, hypothermia and hypoglycemia, but hypoglycemic comas are not always of alcoholic origin, and only measurement of blood alcohol gives a definite diagnosis.

The effect of ethanol intoxication and hangover on immunoreactive plasma arginine vasopressin (AVP) concentration was studied in 7 healthy supine men in controlled clinical conditions. In 6 subjects plasma AVP increased above control values at the time of maximal blood ethanol concentration. The highest AVP values were observed in the subjects having nausea and vomiting and the worst hangover symptoms. The interpretation of the morphological features of alcoholic hepatitis is discussed in terms of a comparison with the results of an ultrastructural and histoenzymological study of the liver biopsies of nine patients. In these patients liver biopsies were performed in the initial stage of the illness and fifteen days after five were re-biopsied, when the clinical and biological signs were improved. The correlations between morphological and biological data were good, especially for the levels of serological and histoenzymological alkaline phosphatase and gamma-glutamyltranspeptidase evaluation

The clinical picture of toxic rhabdomyolysis after alcohol and drug intoxications is characterized by severe pain, swelling, and inability to move the affected muscle group. The disintegration of necrotic muscle tissue leads to hyperkalaemia, excessive increase of creatine kinase in serum, and a myorenal syndrome with anuria. Pathogenetically the combined effects of toxic vascular damage and local pressure and cold influence are assumed to occur. In anuria, resistant to conventional medication, early and repeated plasma exchanges are considered to be a therapeutic alternative to haemodialysis.

The rheological properties of human erythrocytes were investigated after incubation in ethanol-containing suspensions. Ethanol concentrations were adjusted to the blood ethanol level reached in severe alcohol intoxication. A young non-diabetic woman ingested 2.5-3.7 g of chlorpropamide and 0.6-0.9 g of phenformin together with a large dose of alcohol, nitrofurantoin and promethazine in a suicidal attempt. She developed severe metabolic acidosis, hypokalemia, polyuria and prolonged hypoglycemia in the absence of typical hypoglycemic symptoms. These unusual clinical features were in part explained on the basis of the interactions between alcohol and the other drugs.

The ocular manifestations of acute methyl alcohol intoxication were studied in 24 men during an outbreak in Port Moresby, Papua New Guinea. The visual acuity, pupillary reactions to light, fundal appearances and visual fields were recorded in all patients within 72 hours of ingestion of methanol and again three months later. The authors hold their case reported in this paper interesting for the following reasons: in several minutes two shots were fired by intention of suicide. Two entrance openings were found that had the characteristics of shots fired within close range, but no exit openings were to be seen. Considering the entrance openings, the bilateral fracture of the atlas and a comminuted unilateral fracture of the axis and spotting the jammed missiles inside the tunnel, one must deduce that the missiles went by vital structures without severing them.

Eighty-eight patients with craniocerebral trauma were studied prospectively to assess the effects of the injury on sodium and water balance. Abnormalities of serum sodium and osmolality occurred in 11 of the 76 patients who were on the study more than 24 hours, and the incidence of these abnormalities was directly related to the severity of the craniocerebral injury. A report is given on 28 resections on patients with chronic pancreatitis with a high frequency of severe destructive lesions in the head of the pancreas. Pseudocysts, single, multiple or extrapancreatic were present in almost 50%. Strictures of the Ductus Wirsungianus were found in 24 cases and 50% had multiple pancreatic stones. Snowmobile accidents are now common in winter; there is a local pattern of facial fractures associated with accidents involving these machines. Facial fractures connected with snowmobile accidents over a period of two successive winters were reviewed; 14 were found to be of special interest because of their severity and localized extent. Two cases with acute renal failure after prolonged hypothermia are presented. Both patients were found in come, became rapidly uremic and required hemodilaysis treatment.
