Severe organophosphate poisoning complicated by alcohol and turpentine ingestion.

A 52-year-old comatose white male was admitted to the hospital with what was later learned to be severe poisoning by a turpentine solution of dicrotophos, an organophosphorous insecticide he had ingested while drunk. The patient was treated effectively with atropine and pralidoxime chloride and required the largest doses of both agents reported to data. The case was complicated by difficulty in evaluating the response to therapy because of the effects of alcohol and turpentine. The patient was discharged from the hospital on the thirty-third day with no apparent sequelae.

Mental disturbed violent offenders in Sweden.

Among perpetrators of crimes of violence against persons submitted to psychiatric examination, 16 men with severe mental disorder have been examined by standardized psychologic tests and clinical interviews. Objective anamnestic data have been used to assess social background, individual development and onset of mental disorder. Psychotic as well as nonpsychotic men reacted with violence against threat to the offender's physical existence of his self image. In most instances ego weakness or depleted mental energy exaggerated the feeling of threat or stymied the ability to choose alternative so

Management of potentially fatal alcohol intoxication.

Severely alcohol intoxicated patients pass through all stages of anesthesia according to the Guedel classification. Additional illnesses or injuries might be present, however. These require careful investigation. Blood alcohol levels are no criterion for the degree of intoxication. The treatment varies and is based on clinical symptoms. In our series of more than 100 alcohol intoxicated patients the treatment generally consisted o Apomorphine, unless unconsciousness was present, and peripheral vasopressors, if necessary. Occasionally endotracheal intubation was required. On gastric aspiration

[Forensic problems of fatal hand-inflicted injuries].

In the last years in Hungary continual decrease of homicidal cases could be registered, however other forms of violence, severe injuries followed by the death of the victim are almost as high as formerly. Author in the last ten years performed autopsy of 14 such cases. On the base of them summarizes problems arising in connection with the medico-legal investigation of them. He concludes that among these problems-beside alcoholic intoxication-the most important is to reveal the natural disease-if any-of the victim. If there is a natural disease it may be accounted as a factor in the death. Then

Blood alcohol levels in a series of injured patients with special reference to accident and type of injury.

Blood alcohol was determined in 1012 injury victims. It was found that the alcohol involvement (AI) rate was 37% in the total number of patients, 19% in industrial, 38% in traffic, 36% in home, 45% in other freetime accidents, and 69% in the victims of fights, assaults, and suicide attempts. The most common external cause of injury was falling (447 cases) with an AI rate of 38%, while the 172 other nontraffic true accident cases had a rate of 24%. The road and stairs were the usual places of falls while intoxicated. Head injuries had a high (47%) and upper extremity injuries a low (25%) AI rat

Acute renal failure due to nontraumatic rhabdomyolysis.

Twenty-one patients developed acute renal failure in association with nontraumatic rhabdomyolysis and myoglobinuria. The illness followed an overdose of ethanol, heroin, or other depressant drug in 18 patients. Lethargy or coma was present in 17 patients and muscle swelling in 11. Evidence of rhabdomyolysis included markedly elevated creatine phosphokinase, myoglobinuria, and aldolase in blood. Initial biochemical findings were similar to those of acute renal failure due to other causes, but the abnormalities were exaggerated. There was a disproportionate rise in serum creatinine concentration

The perception of the common cold, and other ailments and discomforts, as related to age.

Perception of the common cold and other discomforts was studied as a function fo age among 456 normals ranging in age from 18 to 82. Respondents completed a questionnaire which deal with the frequency and duration of their colds, their symptoms and treatments-and, finally, with the perceived severity of various discomforts. Though older Rs were found to suffer more than younger with body aches, the former had less difficulty with a variety of other physical and psychological symptoms-and also contracted relatively few colds. Older Rs treated their colds less often with Contac and Dristan, but

[Therapeutic tactics in treatment of patients with cranio-cerebral injury and alcoholic intoxication].

Case-histories of 1077 victims of cerebro-cranial injuries, 38 per cent of whom were admitted in the state of alcoholic ebriation, were analyzed. Alcoholic ebriation was found to materially change the clinical picture of the cerebro-cranial trauma, simulating, sometimes, a picture of a traumatic brain lesion. Curative measures aimed at the treatment of the traumatic lesion of the brain were applied in a complex set, depending upon the nature and the severity of the cerebro-cranial injury, the phase (stage) and degree of alcoholic ebriation.

The pathology and haematological changes in experimental Pimelea spp. poisoning in cattle ("St. George disease").

Six young cattle were intoxicated with either stem or extracts of stem of Pimelea trichostachya or P. continua. Oral dosage with whole plant or alcoholic extracts was shown to reproduce all the clinical and pathological features of the various field syndromes known collectively as "St. George disease". Subacute intoxication resulted from relatively high oral dose rates, of the order of 60 mg of whole stem/kg body weight/day, maintained for about 2 weeks. Diarrhoea, weakness and anaemia were the predominant features of this sundrome. Lower dose rates, regulated according to the severity of diar

Ethanol dependence in the rat: a parametric analysis.

Rats were maintained on liquid diets as their sole source of calories and fluid for 10, 15, 20, and 30 days. The diets consisted 35-40% of total calories in the form of ethanol. This procedure resulted in substantial ethanol intake leading to behavioral intoxication. Blood ethanol concentrations were found to be elevated throughout the day with a peak during the dark phase of the light cycle. The removal of ethanol resulted in evidence of physiological dependence, including behavioral manifestation of autonomic and somatic dysfunction and an increased susceptibility to audiogenic convulsions.

Induction of physical dependence upon ethanol and the associated behavioral changes in rats.

This paper reports findings relative to a simple, rapid and reproducible technique for the induction of physical dependence upon ethanol in the rat. The dependence was induced by intragastric intubation of 20% (w/v) ethanol solutions at 9-15 g/kg in 3-5 fractional doses daily for 4 days, maintaining blood ethanol concentrations above a threshold level sufficient to sustain observable sedation throughout the entire period of intubation. Two phases were distinguished during the withdrawal period: 1. Prodromal detoxication, characterized by a spectrum of signs and responses of diminishing severit

Factors influencing outcome after blunt duodenal injury.

Thirty-six patients with blunt duodenal injury have been treated at Detroit General Hospital since 1960. The majority of the patients were driving an automobile under the influence of alcohol and none were wearing seat restraints. Diagnosis was often delayed due to a failure to recognize the significant, but subtle, physical and roentgenographic findings of retro-peritoneal injury. Morbidity and mortality were related to a delay in operative intervention, the severity of duodenal injury, the presence and degree of associated pancreatic injury, and the choice of operative therapy. Patients with

Truths, traps, and tactics concerning alcohol, other drugs and highway safety.

Ingestion of alcohol by pedestrians is an important factor in severe highway crashes, just as alcohol is important in the injury of drivers and their passengers. Blood alcohol concentrations below 50 mg per 100 ml (0.05 percent by weight) do not increase crash risk, but above that concentration the risk rises rapidly. Three distinct groups-problem drinkers (many of whom do not have blatant alcoholism), teenagers, and heavy social drinkers-make up the over-whelming majority of persons in alcohol related crashes, and countermeasures specific to each group must be applied and evaluated. Past and
