Developmental Trajectories of Alcohol, Mental Health and Physical Health among American Indian and First Nation Adolescents: Understanding the Role of Land-Based Cultural Engagement, Family, and Peers.

We applied the Indigenous Traditional Ecological Knowledge (ITEK) framework to understand patterns of wellbeing among American Indian and First Nation (AI/FN) adolescents; risks and protective factors associated with alcohol use, mental health, and physical health that can be translated into prevention intervention strategies. Latent profile analysis and growth mixture modeling (GMM) were applied to identify and examine patterns of change among distinct subgroups of AI/FN youth (Wave 1 N = 735, M age = 11.09, 50.2% female) residing on or near a Tribal Nation or Reserve on the role of land-base

Intersectional discrimination and noncommunicable disease risk factors among sexual minority men of color.

BACKGROUND: Sexual minority men (SMM) of color face disproportionate risk for noncommunicable diseases (eg, hypertension) and associated risk factors (eg, substance use) due to intersecting ethnoracial and heterosexist stressors. Existing research often examines experiences of ethnoracial and heterosexist discrimination separately, thereby overlooking the distinct nature of intersectional discrimination. PURPOSE: We used 2 independent samples of SMM of color to examine (1) the impact of intersectional discrimination on noncommunicable disease outcomes and risk factors above and beyond experie

Etomidate vaping: A new form of anesthetic abuse.

Background: The emergence of etomidate-containing e-cigarettes represents a significant and rapidly evolving public health challenge, marking a transition of this clinical anesthetic into illegal use of an anesthetic agent. Originally utilized as a short-acting intravenous anesthetic for intubation induction, etomidate's high lipophilicity allows it to be easily aerosolized for inhalation via vaping devices. Misuse is primarily driven by its rapid-onset effects, including relaxation, dissociation, and transient euphoria, as well as a pervasive "illusion of safety" among users who perceive hosp

[Testing gamma-glutamyl transferase (GGT) in clinical practice: Rationalizing its prescription to limit financial and ecological costs].

Gamma-glutamyl transferase (GGT) is a membrane-bound enzyme involved in extracellular glutathione metabolism and amino-acid transport, widely expressed in hepatobiliary epithelium and other tissues. In clinical practice, serum GGT is primarily used as a sensitive but non-specific marker of hepatobiliary injury and cholestasis, and as a companion test to alkaline phosphatase (ALP) to confirm hepatic origin. Beyond liver disease, elevated GGT reflects systemic oxidative stress and inflammation and has been associated with cardiometabolic disorders, some cancers, and drug-induced liver injury. Th

Exploring the Expanding Role of Nuclear Receptor LRH-1/NR5A2 in Cell Biology and Immunity.

LRH-1/NR5A2 is a member of the NR5A subfamily of nuclear receptors, acting as a crucial transcription factor involved in diverse biological processes. Although LRH-1 has been extensively studied in vitro and in vivo, in relation to its role in embryonic development and differentiation and in adult tissue processes, such as cholesterol, glucose, and ethanol metabolism, current knowledge about its function in immunity is very limited. This review, therefore, presents the most recent knowledge regarding NR5A2 regulation and LRH-1 function, including the wide spectrum of its target genes. In addit

Diagnostic Accuracy of Endometrial Aspiration Cytology Compared With Histopathology in Abnormal Uterine Bleeding: A Cross-Sectional Study at a Tertiary Care Hospital.

Introduction Abnormal uterine bleeding (AUB) is a frequent indication for endometrial evaluation. Histopathological examination remains the diagnostic reference standard, but tissue sampling by curettage or biopsy may be invasive and resource-intensive. Endometrial aspiration cytology (EAC) offers a rapid, minimally invasive alternative that can provide an early impression of endometrial pathology. This study assessed the diagnostic performance of EAC against histopathology in women presenting with AUB. Materials and methods This prospective cross-sectional study was conducted from January 202

An N-acyl Donor for Serine/Threonine Ligation Involving a Direct N-to-N Acyl Shift.

Developing precise peptide modification protocols is crucial for peptide-based drug development. While chemical ligation addresses this, conventional methods often face difficulties such as side reactions due to the high electrophilicity of S- or O-acyl donors. Here we developed an N-(2-formylphenyl)-hydroxamate (HDX) ester as an N-acyl donor for chemical ligation involving a direct N-to-N acyl shift, eliminating the need for S- or O-acyl compounds. Mechanistic studies revealed kinetic selectivity between substrates and the reversibility of the acyl shift, suggesting the possibility achieving

Sleep and Drug Use: Neuroimaging Evidence for a Multidimensional Brain Framework.

Sleep plays a critical role in the initiation and maintenance of drug use, as well as recovery among individuals with substance use disorders. Sleep is a multidimensional construct, and a growing body of research has demonstrated the relative independence of various components. Specific sleep characteristics including insomnia (self-reported sleep disturbances), short sleep duration, late sleep timing and sleep irregularity have each been associated with increased risks of drug use. However, it remains unclear whether these sleep components contribute to substance use through shared neurobiolo

Hospitalist perspectives and practices in treating hospitalized patients with alcohol withdrawal syndrome in the era of phenobarbital re-emergence.

Medication treatment for moderate to severe alcohol withdrawal syndrome (AWS) is evolving, particularly with renewed phenobarbital use. However, little research has examined how hospitalists manage AWS. We conducted a multiple-methods study involving hospitalists from a national US Hospital Medicine research network. Thirty-three hospitalists from 27 tertiary hospitals completed surveys and participated in seven focus groups on August 8, 2025. Benzodiazepines remained the most common treatment, but phenobarbital use was reported in 67% of emergency departments and 44% of inpatient medical unit

Declines in stimulant-involved overdose mortality occurred only with fentanyl co-involvement, United States, 2023-2024.

BACKGROUND: Between 2023 and 2024, overdose mortality in the United States declined substantially. It is unclear whether declines in stimulant-involved overdose mortality reflect changes in stimulant-related mortality independent of fentanyl co-involvement. METHODS: We analyzed U.S. overdose mortality data from CDC WONDER for 2023 and 2024, stratifying stimulant-, cocaine-, psychostimulant-, and non-fentanyl opioid-involved deaths by fentanyl co-involvement (assigned ICD-10 multiple-cause code T40.4). Mortality rate ratios comparing 2024 with 2023 were calculated. RESULTS: The decline in sti

The Benefits of Managing Anaphylaxis at Home.

Based on current data, immediate activation of EMS in all cases of anaphylaxis does not provide significant benefit. Instead, the option for home management or a "watchful-waiting" approach allowing for increased patient autonomy in anaphylaxis management and emphasizing prompt epinephrine use at home. In this model, patients are instructed to quickly administer epinephrine and monitor for resolution of symptoms. The option to escalate care is available, but in 90% of cases, symptoms will resolve after one dose of epinephrine, rendering EMS/ED involvement unnecessary. Thus, patients can immedi

Citri reticulatae pericarpium viride attenuates metabolic dysfunction-associated steatotic liver disease by activating PGC-1α-mediated fatty acid oxidation in mice.

Citri reticulatae pericarpium viride, a fruit peel from immature Citrus reticulata Blanco known as Qing Pi (QP) in Chinese, is a traditional food ingredient widely employed for culinary flavoring and aroma enhancement. Although QP exerts various metabolic benefits, the impacts and mechanisms of QP on preventing metabolic dysfunction-associated steatotic liver disease (MASLD) remain unclear. Here, we found that QP extracts (extracted with 75% concentrated ethanol and ethyl acetate) not only decreased high-fat diet (HFD)-induced hepatic triglyceride accumulation and oxidative damage, ameliorated

Photobiocatalytic Linear Deracemization of β-Hydroxynitriles.

A photobiocatalytic strategy for the linear deracemization of β-hydroxynitriles is presented. This method consists of a two-step process, beginning with the oxidation of the corresponding alcohol racemate (250 mM) using tetrabutylammonium decatungstate (TBADT, 2.5-5.0 mol%) under 390 nm light irradiation in acetonitrile. The second step involves an asymmetric biocatalytic reduction, carried out sequentially by addition of the required reagents with or without solvent evaporation depending on the selected alcohol dehydrogenase (ADH) enzyme and its tolerance to the organic solvent. Under the res

Implementing the 5A's for Lifestyle Behavior Counseling and Chronic Disease Prevention.

Lifestyle behaviors including not smoking, healthy weight, regular physical activity, a healthy diet, limited alcohol, and adequate sleep can prevent cardiovascular disease, type 2 diabetes, and many other chronic diseases. Yet remarkably few adults practice them consistently. Clinicians often do not deliver preventive counseling, largely because they lack the time, training, and reimbursement to provide guidance and referrals within standard office visits. This article proposes a team-based implementation of the United States Preventive Services Task Force-endorsed 5A's framework (Assess, Adv

Brain structural covariance networks in co-occurring PTSD and alcohol use disorder: An ENIGMA PTSD and ENIGMA addiction working group collaboration.

High rates of comorbid posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD) may stem from a shared neurobiological basis; however, research exploring this overlap remains limited. We examined whole-brain structural covariance (SC) across PTSD, AUD, comorbid PTSD & AUD, trauma-exposed controls, and trauma-naïve controls to identify shared and unique SC profiles. Structural brain scans were acquired from individuals across 14 sites (N = 1565), and cortical thickness estimates were extracted to compute SC. Group differences in SC between pairs of cortical regions were examined usin
