A 67-year-old male patient with pulmonary emphysema was diagnosed with liver cirrhosis. Further investigations revealed an alpha 1-antitrypsin deficiency caused by a PiZZ mutation. The liver cirrhosis was complicated by the development of a hepatocellular carcinoma. The patient died from the consequences of mesentary vein thrombosis. Tumor tissue (n = 338) and liver parenchyma (n = 276) from patients of Asian (n = 31) and European descent (n = 307) with hepatocellular carcinoma (HCC, n = 299), cholangiocellular carcinoma (CCC, n = 16) and combined HCC/CCC (n = 23) were screened with immunohistochemical methods for HBV antigens (HBs, preS1, preS2, HBc, HBe and HBx). Seventy patients with primary liver cell carcinoma (PLC) were studied for the occurrence and relative incidence of paraneoplastic syndromes (PNS) between 1985 and 1990. Here we communicate results of the study involving hypercholesterolaemia, hypercalcemia, hypoglycaemia, gynaecomastia and polycythaemia. The case described is of a primary liver cell carcinoma (PLCC), which was initially misdiagnosed as hepatic hydatid disease. The computed tomographic appearances of hepatic hydatid disease are often characteristic although not specific. When supported by the clinical findings, however, they will probably not be confused with other liver masses. In this case the presence of peripheral rim calcification, commonly seen in hydatid disease but which has not been previously noted in PLCC, lead to an erroneous diagnosis.
