Marital status and incident screen-positive cognitive impairment in a Taiwanese longitudinal cohort (2005-2016): Associations vary by cognitive screening instrument.

Marital status has been associated with cognitive aging; however, whether such associations vary according to cognitive assessment modality remains unclear. This study examined whether associations between marital status and incident screen-positive cognitive impairment (MCI) differ by screening instrument and population subgroups in a large longitudinal cohort of community-dwelling older adults in Taiwan. We analyzed 103,863 adults aged ≥65 years from the Taipei City Elderly Health Examination Database (2005-2016). Cognitive status was assessed using the Short Portable Mental Status Questionn

Primary Intraosseous Cavernous Hemangioma of the Petrous Bone Involving the Jugular Fossa Mimicking a Jugular Paraganglioma: A Radiologic-Pathologic Correlation and Review of Relevant Literature.

Primary intraosseous cavernous hemangioma (PICH) is a rare benign vascular tumor that predominantly involves the vertebral column and calvarium, while skull base involvement, particularly the jugular fossa, is exceptionally uncommon. We report a histopathologically confirmed PICH centered in the petrous temporal bone showing jugular fossa and jugular foraminal involvement, radiologically mimicking a jugular paraganglioma. A 46-year-old man presented with progressive unilateral sensorineural hearing loss. Magnetic resonance imaging (MRI) demonstrated a heterogeneously enhancing jugular foramen

The challenge of introducing aldosterone synthase inhibitors into multidisciplinary practice.

Novel therapeutic agents are required to improve hypertension control globally, which remains one of the leading modifiable risk factors for premature cardiovascular mortality. Aldosterone synthase inhibitors (ASIs) are a novel class of agents that suppress aldosterone production and have demonstrated clinically meaningful blood pressure reductions in phase 2 and phase 3 trials in patients with uncontrolled and resistant hypertension. As these therapies approach clinical adoption, important questions remain regarding effects on cardiovascular and kidney outcomes, safety profile, and optimal po

From low renin to the primary aldosteronism spectrum: aldosterone synthase inhibitors and the endocrine management of hypertension.

BACKGROUND: Hypertension remains a major contributor to global cardiovascular morbidity despite decades of available therapy. AIM: This review advances an endocrinological framework in which chronic hypertension, including resistant hypertension, is understood as a consequence of dysregulated hormonal control of renal sodium handling, mediated by excess renin-angiotensin II activity, inappropriate aldosterone excess, or both. RESULTS: Within this framework, the traditional binary distinction between "essential" and "secondary" hypertension gives way to a mechanistic continuum characterized b

Aldosterone synthase inhibitors: biological rationale, mechanism of action, and results of clinical trials.

Resistant and uncontrolled hypertension remain major unmet needs, with aldosterone dysregulation or frank excess increasingly recognized as a central driver. To this end, aldosterone synthase inhibitors (ASIs) represent a novel therapeutic approach targeting CYP11B2, the key enzyme responsible for aldosterone biosynthesis. By reducing aldosterone production at its source, ASIs may overcome limitations associated with conventional renin-angiotensin-aldosterone system (RAAS) blockade and attenuate both genomic and non-genomic effects of aldosterone. Advances in drug development have led to secon

Modulation of human primary motor cortex plasticity during proactive and retroactive interference between motor practice and paired associative stimulation.

Proactive and retroactive interference have been widely demonstrated behaviorally, but their neurophysiological effects have rarely been compared within the same experimental framework. This study compared the primary motor cortex (M1) plasticity responses when motor practice (MP) was performed before or after paired associative stimulation (PAS25) and explored whether these responses were accompanied by changes in intracortical inhibition. A within-subject design was employed. A right-hand precision grip task with real-time electromyographic feedback was used to examine practice-related refin

A Regulator SNP Affecting the Transcription Factor SP1 Binding Site in the PIK3R3 Gene Contributes to Noise-Induced Hearing Loss Risk.

BACKGROUND: Noise-induced hearing loss (NIHL) is a key occupational hazard, and its specific pathogenic mechanism is not fully elucidated. Here, we performed this study to explore the potential role of single nucleotide polymorphisms (SNPs) located in the PI3K/AKT pathway in human susceptibility to NIHL. METHODS: A case-control study including 688 NIHL patients and 667 normal subjects was conducted. Genotype analysis was tested by multiplex polymerase chain reaction (PCR) with next generation sequencing. Luciferase assays were conducted to explore the effect of SP1 on PIK3R3 promoter activity

Actionable diagnostic delay in primary malignant bone tumors: a trigger-to-action framework for first-contact clinicians.

Primary malignant bone tumors may initially resemble common musculoskeletal conditions, creating uncertainty about when first-contact clinicians should escalate care. This narrative review integrates selected guidelines, diagnostic-pathway studies, and imaging and biopsy literature to translate established principles into practical decisions before specialist referral. We propose actionable diagnostic delay as a descriptive, unvalidated framework: the potentially avoidable interval between a documented clinical trigger for escalation and initiation of the corresponding action. It complements,

Measured fetal-region dose in X-ray pelvimetry and low-dose computed tomography using a realistic pregnant phantom: a comparative study.

This study evaluated fetal-region doses and dose distributions for X-ray pelvimetry (Guthmann and Martius methods) using exposure conditions from a nationwide survey in Japan and compared them with those of a low-dose computed tomography (CT) protocol using a realistic late-pregnancy phantom. A pregnancy model phantom combining an anthropomorphic phantom with a custom-made abdominal phantom was used. Real-time dosimeters were placed at 18 sites across three abdominal levels. The low-dose CT protocol showed a nearly uniform fetal-region dose distribution, whereas the X-ray methods showed hetero
