Cardiovascular disease-specific mortality after partial versus radical nephrectomy for stage II renal cancer: A SEER-based propensity-matched analysis.

BACKGROUND: Surgery remains standard treatment for localized renal cancer. Whether partial nephrectomy (PN), compared with radical nephrectomy (RN), affects long-term cardiovascular disease-specific mortality (CVD-SM) in stage II disease is uncertain. We assessed this association using competing-risk methods. METHODS: Patients with T2N0M0 renal cancer diagnosed in the SEER database from 2004 to 2022 were included. Missing tumor grade and unspecified T2 subclassification were addressed by multiple imputation. Propensity-score matching was performed within 10 imputed datasets. Cumulative incide

UPDATE IN GENETIC CAUSES OF SUDDEN CARDIAC DEATH.

Sudden cardiac death (SCD) is a devastating complication of several different cardiovascular disorders. Over the age of 40 years the most common cause of SCD is due to coronary artery disease and myocardial infarction. In the young under the age of 40 years, inherited/genetic cardiovascular disorders are more common, including the inherited arrhythmia syndromes and the primary cardiomyopathies. Detailed evaluation of the premorbid clinical history, including a detailed family history, and comprehensive expert postmortem examination is essential. In SCD cases where no cause of death is identifi

Development and Internal Validation of a Clinical-Epidemiological Model for Active Pulmonary Tuberculosis Among Patients Already Evaluated for Suspected Disease in a Low-Incidence Hospital.

BACKGROUND: In low-incidence hospital settings, airborne isolation capacity is limited. We developed a clinical-epidemiological model to estimate active pulmonary tuberculosis (PTB) probability among adults already selected for PTB evaluation, using information available before definitive diagnostic testing. METHODS: We retrospectively analyzed consecutive adults who underwent microbiological testing for suspected PTB and were managed with airborne precautions at a university hospital in Rome from January 2016 through December 2021 (n=682 complete cases). A LASSO-penalized logistic regression

Visit-to-visit variability and longitudinal change of the triglyceride-glucose index are associated with early carotid plaque progression.

BACKGROUND: The triglyceride-glucose (TyG) index is a surrogate marker of insulin resistance, but the clinical relevance of its longitudinal dynamics remains unclear. This study aimed to evaluate whether visit-to-visit variability in TyG (TyG-CV) and longitudinal change in TyG (TyG-Slope) were associated with early carotid plaque progression and improved risk prediction beyond conventional factors and mean TyG. METHODS: In this single-centre retrospective longitudinal cohort, 1,519 adults aged 18-65 years who completed three annual examinations from 2023 to 2025 were included. Fasting triglyc

Sleep disorders as an emerging risk factor for liver cirrhosis and hepatocellular carcinoma: a systematic review and meta-analysis of cohort studies.

BACKGROUND: Sleep disorders are increasingly recognized as contributors to metabolic dysfunction and chronic liver disease, but their associations with hepatocellular carcinoma (HCC) and cirrhosis remain unclear. We conducted a systematic review and meta-analysis to evaluate the relationship between sleep disorders and liver-related outcomes. METHODS: Following PRISMA guidelines (PROSPERO: CRD420261342781), we searched PubMed, Embase, Web of Science, and the Cochrane Library through February 2026 for cohort studies assessing sleep disorders and liver outcomes. Adjusted hazard ratios (HRs) or

Diabetes care goals and diabetic retinopathy in adults at tertiary hospitals, Rwanda: a cross-sectional hospital-based study.

BACKGROUND: Attainment of diabetes mellitus (DM) care goals is essential to improving quality of life and preventing complications in patients living with this increasingly prevalent non-communicable disease. Despite the existence of evidence-based national and international guidelines, achievement of glycaemic, blood pressure, and lipid targets remains suboptimal, particularly in resource-limited settings. This study was designed to assess the extent to which diabetes care goals are achieved at major tertiary healthcare facilities in Rwanda, in accordance with established process and intermed

Müllerian-type high-grade serous carcinoma presenting exclusively with pleural lesions and no detectable abdominopelvic primary tumor: a case report.

BACKGROUND: Müllerian-type high-grade serous carcinoma (HGSC) most commonly originates from the fallopian tube, ovary, or peritoneum and usually presents with abdominopelvic disease. Thoracic presentation without a detectable abdominopelvic primary tumor is extremely rare and poses a diagnostic challenge. CASE PRESENTATION: We report the case of a 60-year-old woman who was referred to our hospital after pleural thickening was detected during a routine medical checkup. Chest computed tomography (CT) revealed left pleural thickening with mild pleural effusion. Thoracoscopic pleural biopsy was p

Validity of Administrative ICD-10 Diagnosis Codes for Ventricular Arrhythmia and Cardiac Arrest in a French University Hospital.

PURPOSE: Drug-induced QT interval prolongation increases the risk of ventricular arrhythmias and cardiac arrest. Assessing these outcomes in electronic health databases and evaluating their association with drug exposure requires prior validation of administrative diagnostic codes. This study aimed to evaluate the positive predictive value (PPV) of International Classification of Diseases, 10th revision (ICD-10) codes for ventricular arrhythmias and cardiac arrest in the French hospital discharge summary database (Programme de Médicalisation des Systèmes d'Information (PMSI)). METHODS: In a r

Dual benefit of esketamine-based opioid-free anesthesia on postoperative pain and mental health outcomes in patients undergoing bariatric surgery: a randomized controlled trial.

Patients with morbid obesity are at a high risk of comorbid depressive and anxiety symptoms. While bariatric surgery is an effective treatment, conventional opioid-based anesthesia carries significant perioperative risks. Opioid-free anesthesia (OFA) presents a safer alternative. Esketamine, known for its dual analgesic and rapid antidepressant effects, is a promising component of OFA. However, clinical evidence supporting its simultaneous benefits for both physical and psychological recovery in bariatric patients remains limited. In this double-blind, randomized clinical trial, patients sched

Autism with social anxiety is associated with acquiescent behaviors and enlarged amygdala volume.

Social anxiety (SA) frequently co-occurs with autism spectrum disorder (ASD), but it remains unclear whether SA in ASD reflects greater symptom severity or a distinct subtype. Across two complementary studies, we characterized the behavioral and neural correlates of SA in ASD using a naturalistic social navigation task measuring affiliative and power-related behaviors. In Study 1 (ASD = 575; controls = 357), autistic individuals with SA exhibited significantly reduced task-derived power behaviors (e.g., compliance with others' requests) compared to those without SA-a pattern not observed in no

Prevalence and determinants of metabolic abnormalities among adult tuberculosis patients at Gondar Town, Northwest Ethiopia: an institution-based comparative cross-sectional study.

BACKGROUND: Metabolic abnormalities, such as diabetes mellitus, hypertension, dyslipidemia, and central obesity, are significant contributors to the global burden of non-communicable diseases. Tuberculosis (TB) patients may be at increased risk of these conditions due to chronic inflammation, nutritional depletion, and anti-TB drug effects; however, locally grounded comparative evidence from resource-limited settings such as northwest Ethiopia remains scarce. This study aimed to assess the prevalence and determinants of metabolic abnormalities among TB patients in Gondar Town, Ethiopia. METHO

Partial Immersion Using Body Bags for Exertional Hyperthermia: A Randomized Crossover Study.

INTRODUCTION: Recent recommendations suggest emergency medical personnel should consider rapid whole-body cooling for heat stroke patients. Cold-water immersion maximizes cooling rate and subsequent outcomes following heat stroke. Body bags filled with ice and water may provide portable, accessible partial cold-water immersion for this setting. The purpose of this study was to determine whole-body cooling rates when using body bags to facilitate partial cold-water immersion to treat exertional hyperthermia. METHODS: A total of 9 healthy participants (8 male; mean [SD] age, 24 [4] years; heigh
