We investigated the relationship between use of L-tryptophan and development of eosinophilic fasciitis by two methods: a retrospective patient survey and a case-control study of patients with eosinophilic fasciitis diagnosed at the Mayo Clinic between 1977 and 1989. The presentation of a pig farmer with acute arthritis of the shoulder, cardiac murmurs, and Streptococcus suis growing on blood cultures highlights one of the rheumatological presentations of bacterial endocarditis. The need for a thorough general medical examination together with synovial fluid and blood culture in patients with acute monarthritis is emphasised. This is a cross-sectional survey on the health conditions of railways workers active in 258 interconnection and conversion substations all over Italy. Ambulatory encounters in a community-based family practice residency program were analyzed using diagnosis clusters. During an 18-month period (July 1982 through December 1983), demographic information and clinical diagnoses for 44,453 successive patient visits were collected and stored in a computerized data base.
