The shortage of organ donors poses a major challenge, often leading to patient deaths due to the lack of transplants. Additionally, transplant recipients face lifelong immunosuppression, which brings further health risks. The use of stem cells and biomaterials to develop bioinks for bioprinting technology holds the potential to create personalized tissues and organs that closely replicate the size and structure of their natural counterparts. This could provide a revolutionary solution to the organ shortage crisis. Even though the technology has not yet reached the stage of fully functional org

AIMS/HYPOTHESIS: The role of intra-pancreatic lipid and collagen in type 2 diabetes pathogenesis remains unclear. We sought to examine this in pancreases from organ donors with and without diabetes.
METHODS: Tissue biopsies from 36 adult donor pancreases with/without type 2 diabetes were collected from 16 anatomically defined regions, with H&E, Sirius Red Fast Green and chromogranin A immunohistochemical staining and quantification performed. Intracellular lipid droplet area was quantified using transmission electron microscopy in acinar, islet endocrine, beta and alpha cells identified throu

BACKGROUND: The peroneus longus (PL) tendon is increasingly used as an autograft for ligament and tendon reconstructions, particularly in populations where hamstring graft dimensions may be insufficient. However, donor-site ankle function and patient-reported outcomes measures (PROMs) after PL harvest remain underreported. This study aimed to evaluate changes in PROMs and range of motion (ROM) at the donor-site ankle 1 year after PL autograft harvest and the proportion exceeding minimal clinically important difference (MCID) thresholds.
METHODS: Patients who underwent PL autograft harvest fro

BACKGROUND: While various models are useful for donor-recipient size matching, the optimal approach for predicting the correct match remains unclear. We sought to determine the association between ideal body weight (IBW) ratio and post-transplant survival.
METHODS: The United Network for Organ Sharing (UNOS) database was queried for all patients who underwent isolated heart transplantation between July 2000 and July 2023. Donor-recipient size matching was assessed using predicted heart mass (PHM), ideal predicted heart mass (IPHM), and IBW. Ratios were stratified as undersized (≤0.8), properl

This paper develops a theoretical model to show how changes in the default system of organ donor registration, along with a Priority Rule, affect society's cadaveric organ donation rate. The developed model is on the lines of Kessler and Roth (2012) and seeks to inspect the hypotheses put forward by Li et al (2013), solely based on their laboratory experiment. The model shows that some of these results can change under specific assumptions regarding the population of potential donors. Among the hypotheses examined in this paper, special emphasis has been given to hypothesis 4, which evaluates

As solid-organ transplant capabilities improve, transplants are becoming a life-saving solution for an increasing number of patients. However, the number of organs procured from donors following brain death (DBD) falls far short of the number of waiting recipients. Organs procured from donors following circulatory death (DCD) are often comparable to DBD organs and attempt to address the shortage of viable allografts. Strikingly, consistent protocols governing DCD organ procurement do not exist, with one potential reason being an imperfect understanding of the underlying science and philosophy.

BACKGROUND: Dual kidney transplantation (DKT) from small pediatric donors, either en-bloc or split dual kidney transplantation, contributes to mitigating organ scarcity. This study investigates the prognosis of DKT from pediatric deceased donors, and influencing factors.
METHOD: A retrospective study included recipients who underwent DKT from pediatric donors between 2012 and 2022. Recipients were categorized into low mismatch (BWLM, n = 30) and high mismatch (BWHM, n = 10) groups based on donor-recipient weight ratio of 1:10. Outcome encompassed recipient and graft survival, renal function,

INTRODUCTION: Potential donors for transplantation are screened for malignancies that can be transmitted on to the recipient. The discovery of cancer or a history of cancer often leads to contraindications for organ donation, while in some cases the cancer transmission risk is very low. The current context of organ shortage leads us to reconsider the use of organs from donors with cancer or a history of cancer when the risk of transmission is minimal or less than the risk of death on the waiting list. The Transplantation Committee (CTAFU) in partnership with the Cancer Committee (CCAFU) of the

BACKGROUND: Obesity is a common risk factor for heart failure, and heart transplantation (HTx) is the treatment of choice for end-stage heart failure. Due to the limited availability of the donor's heart, efforts are made to increase the donor pool on one hand and to find predictors that can impact HTx outcomes on the other hand. These predictors can help improve donor organ allocation and HTx outcomes. This study aims to investigate the impact of the donor-recipient BMI ratio on follow-up mortality and other outcomes after HTx.
METHODS: From 2012 to 2021, 821 patients underwent HTx in our ce

Trauma victims are a significant part of the donor pool, though injuries to donor organs normally exclude them from their procurement. We present the case of a 37-year-old male with a stab wound to the chest, resulting in penetrating sharp force injury to the right ventricle of the heart. Despite the ultimately fatal injury, the heart was successfully repaired, and later donated, after brain death. Resuscitation and surgical intervention, along with the collaboration of medical and legal parties, enabled heart donation to occur without compromising the quality of forensic postmortem examinatio

BACKGROUND: Radiotherapy and cyclophosphamide (CYP) treatment can adversely affect various organs, including the liver and pancreas. In addition to hepatic toxicity, CYP and/or ionizing gamma radiation (R) may impact the exocrine and endocrine functions of the pancreas. This study aims to determine whether sodium thiosulfate (STS) can protect male rats against CYP and/or R-induced damage to the pancreas and liver. MATERIALS AND METHODS: Sixty-four rats were divided into eight groups, with STS administered for 14 days prior to CYP and/or R treatment. Enzymatic activities of alanine aminotransfe

Bartonella henselae is an important zoonotic pathogen with a wide range of clinical manifestations in humans. Despite advances in understanding its pathogenesis, there is no broadly applicable laboratory animal model for bartonellosis. This study aimed to assess the potential utility of an experimental model of chronic B. henselae infection using ferrets, a species previously utilized in various human pathogen studies. Six ferrets (n = 6) were divided into three groups: a control group (n = 2), a low-dose infection group (n = 2), and a high-dose infection group (n = 2). The two infection group

The endocrine and exocrine compartments of the pancreas are spatially related but functionally distinct. Multiple diseases affect both compartments, including type 1 diabetes (T1D), pancreatitis, cystic fibrosis, and pancreatic cancer. To better understand how the exocrine pancreas changes with age, obesity, and diabetes, we performed a systematic analysis of well-preserved tissue sections from the pancreatic head, body, and tail of organ donors with T1D (n = 20) or type 2 diabetes (T2D) (n = 25) and donors with no diabetes (ND; n = 74). Among ND donors, we found that the incidence of acinar-t

Size-matching donors to recipients in lung transplantation continues to be a clinical challenge. Predicted total lung capacity equations, or more simply, donor and recipient heights, while widely used, are imprecise and may not be representative of the pool of donors and recipients. These inherent limitations may result in size discrepancies. The advent of easily accessible software and the widespread availability of computed tomography (CT) imaging in donor assessments have made it possible to directly measure lung volumes in donors and recipients. As a result, there is a growing interest in

Organ transplantation is one the most important contributions of modern medicine to society since it provides a unique therapy for terminal organ failure. However, the development of this therapeutic approach is hindered by the limited organ supply. In Italy, organ procurement requires a multidisciplinary medical-surgical approach in which legal medical doctors (LMDs) are generally tasked with ascertaining the reality of death. Sometimes, a medico-legal report is required when the deceased's family deny their consent to the organs and tissues removal from a potential deceased donor. LMDs can a

BACKGROUND: Normothermic ex situ liver perfusion (NESLiP) has the potential to increase organ utilization. Radiological evidence of localized liver injury due to compression at the time of NESLiP, termed cradle compression, is a recognized phenomenon but is poorly characterized.
METHODS: A retrospective analysis of a prospectively collected database was performed of transplanted livers that underwent NESLiP and subsequently had a computed tomography performed within the first 14 d posttransplant. The primary study outcome was 1-y graft survival.
RESULTS: Seventy livers (63%) were included in

INTRODUCTION: Informed consent is critical for maintaining the ethical standards associated with the utilization of human donor bodies by tertiary education institutions. Body donation programs undertake the responsibility for procuring human donor bodies for didactic and research purposes. However, its processes require scrutiny regarding best practice guidelines and the South African National Health Act (SA-NHA) (2013). Moreover, acknowledging and addressing the current perceptions of human body donation are indispensable in bridging the gap between academia and society. This study aimed to

BACKGROUND Long-term patient survival after intestinal transplantation (IT) remains low compared with other organ transplants despite years of advancement in clinical experience. While patients with extremely high or low body mass index (BMI) are often considered ineligible for IT, the impact of BMI on post-transplant IT survival remains understudied. MATERIAL AND METHODS Using the United Network for Organ Sharing Standard Transplant database, we conducted a retrospective cohort study on patients who underwent IT between April 11, 1994, and September 29, 2021. We assessed the association of re
