Total vascular exclusion in pediatric hepatectomy: reducing hemorrhage without added morbidity.

BACKGROUND: Major hepatectomy in children carries distinct risks and challenges, with intraoperative hemorrhage being a critical determinant of morbidity and survival. Total vascular exclusion (TVE) provides complete vascular control, but its use in adults is associated with high morbidity. Evidence for pediatric hepatectomy is lacking. METHODS: A single-center study of all children (<18 years) undergoing major hepatectomy between 2013 and 2023 was performed. Patients were grouped by TVE use. Primary outcomes were major complications (Clavien-Madadi ≥ III) and posthepatectomy liver dysfunctio

Catecholamines Activate a Tumour Progression Program in Differentiated Tumour Hepatocytes.

BACKGROUND AND AIMS: The sympathetic nervous system is involved in the progression and aggressiveness of several cancers, including hepatocellular carcinoma (HCC). However, the specific effects of sympathetic innervation and catecholamines on tumour hepatocyte behaviour remain poorly understood. In this study, we aimed to investigate the role of sympathetic fibres and catecholamines in the progression of HCC. METHODS: The distribution of sympathetic nerves was mapped in 10 human HCC samples using multiplex immunofluorescence labeling. Adrenergic receptors (ADR) and enzymes involved in catecho

A robust dynamic 3D liver spheroid platform for (sub)chronic toxicity assessment: Metabolic characterization of HepG2 spheroids as a case study.

In vitro assessment of (sub)chronic chemical exposure requires liver models that maintain stable functionality during long-term cultivation and whose baseline behavior is well characterized. HepG2 cells remain widely used in toxicology due to their robustness and reproducibility; however, their hepatoblastoma origin confers persistent tumor‑associated traits. Although three‑dimensional (3D) HepG2 spheroids provide greater physiological relevance than two‑dimensional (2D) cultures, their baseline behavior during prolonged dynamic cultivation remains insufficiently characterized, limiting confid

Ablation for paediatric solid tumours: current evidence from a scoping review.

Ablation therapy offers a minimally invasive alternative for selected paediatric solid tumour lesions but published outcomes are inconsistently reported. We Performed a scoping review of the literature from 2000 to 2024, including studies reporting ablation in children aged under 18 years with solid tumours. Data on tumour type, anatomical site, ablation modality, treatment indication, reported oncological outcomes, recurrence, and complications were extracted. Thirty-three studies reported 202 children with 304 treated lesions across various anatomical locations. Radiofrequency ablation was t

Textbook outcome after hepatoblastoma resection: Definition, failure modes, and risk-adjusted benchmarking in a tertiary European center.

BACKGROUND: Textbook Outcome (TO) has emerged as a composite quality metric for benchmarking perioperative performance in adult hepatobiliary surgery. Its role in pediatric oncologic liver surgery remains insufficiently explored and TO has never been defined for hepatoblastoma resection. METHODS: Single-center cohort study including children undergoing curative-intent liver resection for hepatoblastoma. Textbook Outcome after hepatoblastoma resection (TO-HB) was defined as an all-or-none composite comprising: (i) no postoperative complications (CCI = 0), (ii) no prolonged length of stay (≤pro

Associating Liver Partition and Portal Vein Ligation for Staged Hepatectomy (ALPPS) in Children with Advanced Hepatoblastoma-Lessons from a Case Series and Literature Review.

BACKGROUND: Liver transplantation is the standard treatment for children with advanced hepato-blastoma when complete resection is not feasible. However, transplantation may be contraindicated because of persistent metastatic disease, severe comorbidities, poor clinical condition, or donor-related limitations. Associating Liver Partition and Portal Vein Ligation for Staged Hepatectomy (ALPPS) has emerged as a potential rescue strategy in highly selected patients. This study evaluated the feasibility, safety, and oncological outcomes of ALPPS in pediatric hepatoblastoma. METHODS: A retrospectiv

SMAP1 promotes hepatoblastoma cell proliferation by modulating the C-Kit-activated ERK/MAPK pathway.

Hepatoblastoma (HB) is the most common pediatric liver malignancy. We investigated the role of SMAP1 in HB proliferation and its mechanism. Sixty-seven HB tissues were analyzed for SMAP1 expression via immunohistochemistry and correlated with clinical outcomes. qRT-PCR quantified SMAP1 mRNA in HB cell lines versus normal hepatocytes. SMAP1 was knocked down (siRNA/lentivirus) or overexpressed (plasmid) in HB cells; proliferation was assessed by CCK-8 and colony formation. A nude-mouse xenograft model was used to evaluate tumorigenicity. Western blotting determined C-Kit and ERK/MAPK pathway act

Prospective comparison of near-infrared I and II fluorescence imaging for intraoperative navigation in pediatric hepatoblastoma resection.

BACKGROUND: Complete resection is critical for cure in hepatoblastoma (HB), but small, superficial, or deep lesions may be missed by conventional intraoperative assessment or near-infrared window I (NIR-I) fluorescence imaging. We prospectively evaluated the feasibility and comparative performance of indocyanine green (ICG)-based near-infrared window II (NIR-II) fluorescence imaging during pediatric hepatoblastoma resection. METHODS: Fourteen children with HB were prospectively enrolled. All patients received preoperative ICG administration and underwent intraoperative paired NIR-I and NIR-II

[Infantile Hepatic Hemangioendothelio- ma with Elevated Alpha-fetoprotein: Case Report and Review].

Infantile hemangioendothelioma is the most common benign mesenchymal tumor of the liver and usually occurs within the first six months of life. Although serum alpha-fetoprotein is a significant tumor marker in cases of hepatoblastoma, hepatocellular carcinoma and germ cell tumors, elevated levels are rarely observed in patients with infantile hepatic hemangioendothelioma. In such cases, given that the differential diagnoses have different prognoses, accessible diagnostic options should be sought in order to to guide clinical management. The exact cause of alpha-fetoprotein elevation in hemangi

Pulmonary Metastasectomy After Liver Transplantation: Indications, Timing, and Surgical Decision-Making Across Primary Tumor Histologies.

Liver transplantation (LT) has evolved from a treatment for end-stage benign liver disease into a therapeutic strategy for selected oncological indications, including hepatocellular carcinoma (HCC), unresectable colorectal liver metastases (CRLM), hepatoblastoma, neuroendocrine tumor hepatic metastases, and hepatic epithelioid hemangioendothelioma. As the indications for LT expand and post-transplant survival improves, pulmonary recurrence has emerged as the predominant pattern of extrahepatic relapse across histologies. Yet no standardized surgical guidelines exist for managing lung metastase

Global, regional, and national burden and changing trends of hepatoblastoma and retinoblastoma from 1990 to 2021, and projections until 2050: findings from the Global Burden of Disease Study 2021.

BACKGROUND: Hepatoblastoma and retinoblastoma, two common extracranial embryonal tumors, are the most prevalent primary cancer of liver and eye in childhood, respectively. Despite high survival in high-income regions, outcomes remain extremely poor in low-income regions, underscoring global health inequities and the need for evidence-based resource allocation. To address this gap, we explored the global, regional, and national burden of the two diseases from 1990 to 2021 and projected trends until 2050. METHODS: We extracted data on incidence, prevalence, deaths, and disability-adjusted life-
