Management of burn patients is linked to the development of anti-HLA antibodies in patients receiving cryopreserved skin allografts: A retrospective cohort.

INTRODUCTION: Skin allografts are used to cover wounds in patients with severe burns. Owing to the allogeneic nature of the tissue, rejection often limits its effectiveness to a few weeks. The aim of this study was to analyse the factors influencing sensitisation in skin allograft recipients and to evaluate the role of donor-recipient HLA mismatches in the humoral response of sensitised patients. PATIENTS AND METHODS: Eighteen patients were enrolled in this single-centre, retrospective cohort study. Adult burned skin allograft recipients were selected through a retrospective chart review (201

Integrated photoprotection and senescence modulation: A topical formulation counteracts cellular aging in human skin cells.

BACKGROUND: Skin aging is driven by the accumulation of senescent cells that release a pro-inflammatory mediators, which collectively disrupt tissue homeostasis. Beside ultraviolet A (UVA) and ultraviolet B (UVB) radiation, high-energy visible blue-violet (HEV) light contributes to oxidative stress, mitochondrial dysfunction and premature keratinocyte senescence. A novel photoprotective topical formulation comprising hydroxyapatite, fermented oils, baicalin and crocin was developed to counteract these effects. This study evaluated its biological effect in human skin cell models under basal con

Is Fel d1 Based Component Resolved Diagnosis Associated With Airway Inflammation in Cat Sensitized Children?

In cat sensitized children, decision to eliminate exposure to cats depends on the clinical outcome. In this study, we aimed to examine the relationship between molecular cat allergen sensitization and exhaled breath condensate (EBC) IL-4, IL-5, and IL-13 as markers of allergic airway inflammation. We enrolled subjects with skin prick test diagnosis of cat allergen sensitization aged 4-17 years. Age, sex, diagnosis, and clinical symptoms as well as family history of allergic diseases were recorded at enrollment along with the association of symptoms and cat exposure. Asthma severity and allergi

Predicting Role of Interleukin-33 in Determining the Development and Severity of Atopic Dermatitis.

INTRODUCTION: Atopic dermatitis (AD), also referred to as atopic eczema, is one of the most common immunological disorders in children. Previous studies have suggested potential roles of interleukin-33 (IL-33) in the onset and progression of AD. This study investigated whether serum IL-33 levels in children with AD are associated with disease severity, immunoglobulin E (IgE) sensitization to food or environmental allergens, and serum IgE concentration. METHODS: The study included 62 children with newly diagnosed AD and 30 age-matched healthy controls. Following an interview and confirmation o

In vitro testing for the diagnosis of polysensitization to vespids: CAP inhibition.

Background. In Southern Europe countries sensitization to both Vespula spp. and Polistes spp. can be observed in 50.5 to 61.5% of anaphylactic sting reactions, hindering the distinction between vespid cross-reactivity and genuine sensitization. CAP-inhibition is regarded as the reference method in such cases. Methods. Retrospective study, including candidates to VIT with double sensitization (skin testing, specific IgE to whole venom extracts and to molecular components) to at least two different vespid venoms (Vespula spp., Polistes spp. and/or Vespa velutina). CAP-inhibition was done in all

Diagnosing Nonsteroidal Anti-Inflammatory Drug-Hypersensitivity/Allergy and Nonsteroidal Anti-Inflammatory Drug-Exacerbated or Induced Food Allergy Phenotypes in Children and Adolescents.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used in children. Hypersensitivity reactions (HRs) to NSAIDs represent a diagnostic challenge, particularly because NSAIDs may act as aggravating factors or cofactors as in two phenotypes recently named NSAID-exacerbated food allergy (NEFA) and NSAID-induced food allergy (NIFA). OBJECTIVE: To diagnose NSAID hypersensitivity/allergy (NH/A) and NEFA/NIFA phenotypes in children and adolescents, classify HRs to NSAIDs according to updated international guidelines, and identify clinical and immunologic predictors of true NH/A. Pantothenic Acid as an Unexpected Cause of Hypersensitivity to Sensitive or Atopic Skin Care Products-A Narrative Review.

Pantothenic acid (PA; vitamin B5) is an essential component of many key metabolic processes. Vitamin B5 deficiency causes dysfunction in various systems and organs. Humans do not produce vitamin B5, so it must be constantly replenished through diet or supplementation. Pantothenic acid is synthesized by plants, fungi, and bacteria, so a well-balanced diet is a good source of pantothenic acid. Pantothenates have beneficial effects on the skin (regenerative, healing, and anti-inflammatory effects). They are readily absorbed through the skin, making them a common active ingredient in cosmetics and

Wheat allergy, a new and emerging threat of food allergy for children.

Compared to cow's milk, eggs and peanut, knowledge on wheat allergy is limited. During the past 2 decades, IgE-mediated wheat allergy in children has been frequently observed all over the World, especially in Asia. Wheat allergy usually presents in infants between 6 and 12 months of age. Common clinical manifestations are skin rashes (maculopapular rash, urticaria and angioedema). Atopic dermatitis is a less common presenting manifestation in wheat allergy compared to those with egg and cow's milk allergy. Anaphylaxis occurs in up to 50% of wheat allergy in children. Wheat-dependent, exercise-

Exposome-driven molecular sensitization signatures in pediatric rhinitis and asthma across Spanish regions.

BACKGROUND: Allergic rhinitis and asthma in childhood arise from dynamic interactions between genetic predisposition and the exposome. However, how environmental heterogeneity shapes molecular IgE sensitization profiles during early life remains insufficiently characterized. METHODS: We conducted a prospective, observational, multicentre, cross-sectional study including 354 children (0-15 years) with allergic rhinitis and/or asthma recruited across eight Spanish cities contrasting regional climatic contexts. All patients were skin prick test-positive to at least one aeroallergen and underwent

Unusual Causes of Food-Induced Anaphylaxis - A Review.

Anaphylaxis is a potentially fatal, acute, multisystemic hypersensitivity reaction. Food-induced anaphylaxis (FIA) is typically an IgE-mediated reaction. Although any food has the potential to cause FIA, there are some less well-known causes that have unusual cofactors/triggers, which pose diagnostic challenges and are often classified as idiopathic anaphylaxis. We describe three unusual causes of FIA in this review and discuss testing strategies in resource-limited settings. These include wheat-dependent exercise-induced anaphylaxis (WDEIA), alpha-gal syndrome, and pork-cat syndrome. WDEIA de

Sensitization to Alternaria alternata, household environmental exposure and respiratory disease control in a Mediterranean agricultural area.

BACKGROUND: Environmental fungi play an important role in allergic respiratory diseases. Alternaria alternata sensitization has been associated with more severe respiratory phenotypes and poorer asthma control. Mediterranean agricultural environments may favor environmental exposure because of their climatic conditions and intensive agricultural activity. OBJECTIVE: To analyze the prevalence of sensitization to Alternaria alternata, evaluate its association with asthma control and quality of life, and describe household environmental findings in a Mediterranean agricultural population. The latex allergy dogma in spina bifida - how generalisable is the evidence? A cross-sectional study from Southern India challenging Western benchmarks.

PURPOSE: Children with spinal dysraphism (SD), and particularly those with open spinal dysraphism (OSD; myelomeningocele/myeloschisis), have historically been classified as a uniquely high-risk population for latex allergy, with most published evidence derived from Western cohorts reporting prevalence rates of 26-70%. Data from low- and middle-income settings remain scarce. This study aimed to determine the prevalence of latex sensitisation and clinical latex allergy in children with SD at a tertiary care centre in southern India and to identify associated risk factors. METHODS: In this cross

Adult IgE-mediated food allergy: clinical characteristics, predictors of severe reactions, and total IgE cut-off.

BACKGROUND: Adult immunoglobulin E (IgE)-mediated food allergy (FA) is increasingly recognized, yet it has remained under-characterized, compared with pediatric FA. We described clinical features, diagnostic profiles, predictors of severe reactions and explored a pragmatic total IgE cut-off in an adult cohort. METHOD: We retrospectively reviewed 423 consecutive adults evaluated for suspected IgE-mediated FA at a tertiary center. Demographics, comorbid atopy, index culprit foods, diagnostic testing (skin prick/prick-to-prick, serum-specific IgE, component-resolved diagnostics), laboratory para

Outcomes of oral food challenge in children with IgE-mediated food allergy.

BACKGROUND: Oral food challenges (OFCs) are the gold standard for diagnosing IgE-mediated food allergies in children, although they carry an inherent risk of inducing allergic reactions, including anaphylaxis. This study examined clinical outcomes and predictors of positive OFC results in a pediatric cohort from a region with high exposure to sesame and tree nuts. METHODS: We retrospectively analyzed 310 OFCs in 211 children (median age: 21 months [IQR: 14-37.3]) at a tertiary pediatric allergy center from January 1 to December 31, 2023. Reaction patterns and predictors of OFC positivity, inc

Differentiating Peanut Allergy from Sensitization in Polish Children: A Real-Life Diagnostic Model.

Peanut allergy (PA) remains a major diagnostic challenge in pediatric allergy, largely due to the frequent discrepancy between immunological sensitization and clinically relevant disease. This study aimed to develop a real-life diagnostic prediction model to distinguish true peanut allergy from asymptomatic peanut sensitization in children referred for evaluation of suspected PA. In this cross-sectional study, 80 children aged 1-18 years were assessed in a tertiary allergy center in Poland. Sixty-five children with peanut sensitization underwent detailed clinical history assessment, skin prick

Acanthamoeba profilin as a novel airway allergen with diverse sensitization patterns including pollen cross-reactivity.

Profilins are ubiquitous pan-allergens responsible for cross-reactivity between pollens and plant foods. While we previously demonstrated that recombinant Acanthamoeba profilin (rAc-PF) drives allergic airway inflammation via Th2/Th17 pathways in murine models, its clinical relevance in human allergic disease remains unclear. This study investigated rAc-PF as a novel inhalant allergen and its immunological impact on patients with allergic airway diseases. A total of 176 patients with allergic airway diseases underwent skin prick tests with rAc-PF and a standard panel of 55 common aeroallergens
