A roadmap towards clinical use of molecular biomarkers in acute ischaemic stroke.

Acute ischaemic stroke remains a leading cause of death and disability worldwide, and even highly effective reperfusion therapies benefit only a fraction of patients. Current clinical decision-making relies largely on clinical examination and imaging, which provide limited insight into the molecular mechanisms that drive injury progression, treatment response and recovery. Molecular biomarkers offer a complementary window into this biology: they can resolve pathway activity at the cellular level, detect pre-morphological tissue stress, report on multi-organ processes and enable high-frequency,

Assessing the American public's preferences for reforms to teen driving licensure systems: a discrete choice experiment.

OBJECTIVES: To analyse factors influencing the American public's preferences for changes to teenage driver licensing requirements. METHODS: We employed a discrete choice experiment (DCE) with 808 participants from National Opinion Research Center's AmeriSpeak panel to assess preferences for two existing elements (on-road testing and intermediate licensure period) and a new feature (driver monitoring with telematics during the intermediate licensure period) of licensing system. Multinomial and mixed logit models were used to estimate preference weights, marginal rates of substitution and the r

Feasibility and discriminatory properties of a simple fitness-to-drive assessment using a driving simulator placed in an orthopaedic outpatient department: a feasibility study.

INTRODUCTION: Safe return to driving after orthopaedic injury or surgery is important, but standardised and feasible in-hospital assessments are lacking. We evaluated the feasibility of a simple simulator-based fitness-to-drive assessment in an orthopaedic outpatient department and its ability to discriminate between orthopaedic patients and professional drivers. METHODS: In this prospective feasibility study (January 2024-January 2025), two identical driving simulators were installed in an orthopaedic outpatient department and a vocational training centre for professional drivers. Artificial intelligence in medical ethics education: a descriptive study of eight models in multiple choice question generation.

INTRODUCTION: Integrating artificial intelligence (AI) into medical education poses barriers and opportunities for medical educators. One of those opportunities is content creation for medical ethics assessment. Assessing the performance of AI in generating multiple choice questions (MCQs) in ethics that aligns with United States Medical Licensing Examination (USMLE) is important. The present study evaluates the performance of eight AI models-GPT-4 Turbo, GPT-3.5 Turbo 0125, o1 Mini, o1 Preview, GPT-4, Claude 3.5 Sonnet, Claude 3 Opus, and Gemini-in regard to the relevance, clarity, and accura

Reforming the Driving Examination Process Evidence Gaps and Data Needs.

Every US state requires drivers to pass a knowledge and skills examination before obtaining their driver's license. Surprisingly little is known about the progression of people through the examination process. To date, no research has been conducted on the proportion of individuals who pass from one stage of the driver examination process to the next compared to those who fail and are required to be reexamined. The purpose of this study was to follow the progression of a cohort of new drivers in Washington State as they advanced through the driver examination process. We reviewed driving recor

The effect of maropitant on sedation, propofol requirements, and cardiovascular function at induction of anesthesia in healthy cats.

The objective of this study was to evaluate the sedative effects, propofol requirements, and cardiorespiratory effects following maropitant administration in healthy cats. Maropitant is a selective neurokinin-1 receptor antagonist licensed for use as an antiemetic in dogs and cats. Recent studies have highlighted the ability of maropitant to reduce inhalational anesthetic requirements during surgery from presumptive analgesic effects. Sedative properties of maropitant and its possible reduction of injectable anesthetic requirements have not yet been evaluated. Twenty adult purpose-bred domesti

How should drivers' use of Automated Lane Keeping Systems (ALKS) be assessed? A study with experienced driving assessors in a Wizard-of-Oz vehicle.

This study aims to contribute to guidelines for driver licensing organizations on assessing driver competence in using Level 3 Automated Lane Keeping Systems (ALKS), based on an on-road experiment with eight professional driving assessors (i.e., expert driving examiners who train examiner candidates; 6 males, 2 females, all driving more than 20,000 km per year) in a Wizard-of-Oz vehicle. Using a think-aloud protocol, we captured cognitive processes during system supervision and take-over requests (TORs) in real-world traffic jams. [Fitness to drive: legal context and new opportunities for cognitive assessment].

Decisions regarding driving licence eligibility represents a complex medical and legal challenge, requiring careful consideration of both public safety and individual interests. The present paper provides a brief overview of the legal and administrative framework of fitness-to-drive evaluation, the roles of the professionals involved in the process, and key issues related to cognitive assessment. While physical symptoms associated with neurological conditions are generally objectively identifiable, cognitive impairments often remain less apparent and may significantly affect driving ability ev

The Examination of Factors Contributing to Delays in Obtaining a Driver's License for Youth With Attention-Deficit/Hyperactivity Disorder.

BACKGROUND: Youth with attention-deficit/hyperactivity disorder (ADHD) are more likely to experience delays in obtaining their driver's license relative to their same-aged peers. However, despite these delays, there is no evidence of improved driving outcomes for these youth. Additionally, graduated driver's licensing laws typically terminate around the age of 18, giving some youth less structure prior to independently transitioning to the road. To date, no study has examined the cognitive and parenting factors that are related to delays in licensure for adolescents with ADHD within the contex

Screening effects of China's driver's license test reform: evidence from regression discontinuity design.

Driver's license testing determines who may legally operate a motor vehicle and is therefore a central screening instrument for road safety. This study develops a screening-theoretic framework for driver licensing and uses microscopic traffic accident data from Guangdong Province, China, to evaluate the 2004 reform of China's driver's license test system. We apply a regression discontinuity design based on license issuance dates, using January 1, 2005 as the empirical cutoff that reflects the transition between policy implementation and license issuance. The results show that drivers licensed

Examination of driver license renewal policies and older driver crash and injury rates by rurality.

BACKGROUND: Among adults aged 65 and older, motor-vehicle crashes are the second leading cause of injury-related death-following falls. State driver license renewal laws commonly have provisions targeting older drivers, but limited evidence exists on their effectiveness in reducing crash and injury rates and how this may vary by rurality. This study aimed to investigate the impact of state driver license renewal policies on older driver crash and injury outcomes, by rurality. METHODS: Crash data, license renewal policies, and other relevant state policies were drawn from 13 U.S. states for th

[Patient's criminal liability for providing false medical information in the procedure for applying for a driving license].

The case report concerns a patient who, during a medical examination conducted to asses medical fitness to drive motor vehicles, concealed the truth about his health condition. Particular attention was paid to the discussion of the patient's responsibility for the content of information provided in the interview. The article discusses criminal liability for making a false statement, for obtaining false certification by fraud and for using a document obtained by fraud. Attention was also drawn to the social obligation to denounce, which may also apply to physicians involved in medical certifica

Inhibition of Neisseria gonorrhoeae complement-mediated killing during acute gonorrhea is dependent upon the IgG2:IgG3 antibody ratio.

UNLABELLED: Excessive binding of antibodies to the bacterial cell surface can paradoxically increase resistance of some Gram-negative pathogens to complement-mediated killing (CMK). We examined the CMK of 336 Neisseria gonorrhoeae isolates from 283 participants recruited to a clinical trial. Serum bactericidal assays revealed 3% (9/283) of the autologous participant sera blocked CMK. Gonococci isolated from these participants were resistant to the autologous host serum, but sensitive to pooled healthy control sera (HCS) and protected by autologous host serum in a 1:1 mixture with HCS.
