Tele-Oncology in the Post-Pandemic Era: Clinical Integration, Access Disparities and Medico-Legal Accountability.

PURPOSE OF THE REVIEW: Tele-health has evolved from a marginal tool confined to rural populations and selected follow-up programs into a structurally integrated component of modern cancer care. Prior to COVID-19, its adoption was constrained by regulatory fragmentation, non-uniform reimbursement, and licensure barriers. This narrative review evaluates the evolutionary integration of tele-health in oncology post-COVID-19, examines digital disparities across patient populations, and addresses the medico-legal implications of this integration, with the objective of providing a comprehensive and c

National Consensus on the Assessment of Visual Function for Driving in Switzerland.

PURPOSE: To establish a national consensus on assessing visual function for fitness to drive in Switzerland. METHODS: The minimum medical requirements for visual function for fitness to drive are regulated by Swiss Federal Law, namely, by the Traffic Licensing Ordinance (TLO). The medical examination techniques relevant in this context and their assessment are not further specified therein, which leads to legal inequality among drivers and uncertainty among examiners. We established a study group of representatives of the Traffic Medicine Section of the Swiss Society of Forensic Medicine and

Arms Race Control Score Standardizes Residency Applicant Publication Assessment.

BACKGROUND AND OBJECTIVES: The publication "arms race" refers to the recent, exponential increase in publications among residency "Match" applicants. Total number of residency applicant publications is the strongest predictor of "Match" success in neurosurgery beyond licensing examination scores. This study sought to introduce an objective metric by which publications may be scored to assess residency applicant authorship effort. METHODS: A retrospective, quantitative assessment of 537 publications by 57 interviewed applicants at a neurosurgery residency program was conducted from the 2022 to

Assessment of Candidates' Performance Pre- and Post-Adoption of Standard Setting in College Examinations between 2016 and 2023.

BACKGROUND: Assessment drives candidates' learning and focuses on the main goals and objectives of the course. Over the years, determination of pass scores in medical education has evolved from the arbitrary 50% to the actual determination of the cut-off scores in the standard setting process in order to make a decision to pass/fail candidates presenting for licensure or credentialing purposes. MATERIALS AND METHODS: The study aimed at assessing and comparing candidates' performance pre- and post-adoption of standard setting in the college. This was a comparative cross-sectional study with a

Supporting PharmD graduates after unsuccessful licensing examination attempts.

Not passing the North American Pharmacy Licensure Examination (NAPLEX) is the reality for a significant number of PharmD graduates. The average first time pass rates for the NAPLEX have experienced significant drops since 2017, hitting an all-time low (76.6 %) for the Class of 2023. While most candidates eventually pass the NAPLEX, a staggering 3228 PharmD graduates remain unlicensed due to unsuccessful licensure examinations from cycle years 2022 to 2024. Further decline is anticipated with impending changes to the structure of the NAPLEX in 2025. Unfortunately, unlicensed graduates are unabl

[Driving with cognitive impairments: Results of the population-based Digital Dementia Registry Bavaria - digiDEM Bayern].

BACKGROUND: In Germany, the number of drivers aged 65 and over is increasing. However, the risk of physical and cognitive impairments that affect the ability to drive rises with increasing age. Those who give up driving are often confronted with constraints of their autonomy and quality of life. The study aims to identify socio-demographic and health-related determinants of non-driving in people with mild cognitive impairment (MCI) or mild to moderate dementia. METHODS: The data basis is the baseline survey of the multicentre, prospective registry study "Digital Dementia Register Bavaria - di

Individuals with severe visual field loss from stroke and glaucoma could have on-road driving safety comparable to normally sighted drivers.

BACKGROUND: Vision is a critical component of safe driving, yet establishing effective legal vision requirements for driver licensing remains challenging. Current EU regulations mandate minimum standards for visual acuity and visual fields, but also allow exemptions based on practical driving tests. This study investigates the on-road driving performance of individuals with visual field loss (VFL) who regained their licences after passing a simulator-based assessment, compared to age-matched and younger, normally sighted controls. METHOD: The study included 72 individuals with VFL who had suc

Glycerophospholipid metabolism licenses IgE-mediated mast cell degranulation.

Immunoglobulin E (IgE) antibodies and mast cells have been extensively recognized to dictate the pathophysiology of anaphylaxis and allergic reactions; nevertheless, the pivotal cues driving IgE-mediated mast cell degranulation remain enigmatic. Here, we demonstrate that FcεRI aggregation-initiated p38α signaling stimulates Ets-1 transcription by recruitment of the SWI-SNF chromatin-remodeling complex, contributing to Pcyt1a expression and glycerophospholipid metabolism in IgE-stimulated mast cells. Most importantly, Pcyt1a-mediated glycerophospholipid metabolism facilitates mast cell degranul

Anti-pA137R antibodies exacerbate the pathogenicity of African swine fever virus in pigs.

African swine fever virus (ASFV) is the causative agent of African swine fever (ASF), a highly contagious disease with a mortality of up to 100% in pigs. Currently, no safe and effective vaccines are available globally, except for two licensed vaccines in Vietnam. One of the factors impeding the development of vaccines is antibody-dependent enhancement (ADE). Our previous study showed that the antibodies against the ASFV protein A137R (pA137R) could promote ADE of ASFV infection in primary porcine alveolar macrophages. However, direct evidence of ADE in target animals remains limited. Here, we

Use of a formative mock examination in predicting Australian general practice licensure examination performance.

The Royal Australian College of General Practitioners (RACGP) administers high-stakes summative licensure examinations for unsupervised independent Australian general practice. Examination failure can have adverse consequences including psychological stress and financial implications. In-training assessments as predictors of summative or licensure exam outcome are frequently administered as early-training stage formative assessments. Assessments more proximate to the time of licensure examinations may also be of utility. The Mock-AKT, a formative assessment, was available to registrars of an A

Validation of a performance-based driving assessment: Implications for licensing young drivers.

INTRODUCTION: The on-road behind-the-wheel "practical" license test is the gate by which aspiring drivers must pass in order to drive independently in many jurisdictions. Evidence linking practical test performance and future driving outcomes is mixed. In the United States, license tests are characterized by high pass rates. A more rigorous test might dissuade applicants who are not ready from attempting the test, encourage better preparation, and also sensitize aspirational drivers to their areas of strengths and weaknesses; this is especially important for young, novice drivers. Predicting COMLEX-USA Level 2-CE using medical school performance and use for student advising.

CONTEXT: As Comprehensive Osteopathic Medical Licensing Examination-USA (COMLEX-USA) Level 1 has changed to Pass/Fail scoring, residency programs that required minimum Level 1 scores for applicant consideration may choose to focus on COMLEX-USA Level 2-Cognitive Evaluation (Level 2-CE) target scores for applicant selection. Therefore, finding ways to predict passing and high Level 2-CE performance based on students' past performance and to guide their study accordingly is essential for helping students succeed in and beyond medical school. OBJECTIVES: The purpose of this retrospective study i
