Masquerade Syndrome Presenting as Optic Neuritis and Retinitis Due to Pituitary Germinoma.

Masquerade syndrome is a collective term for various ocular diseases presenting with clinical features resembling intraocular inflammation. Delays in definitive diagnosis and treatment can not only lead to irreversible vision loss but also impact the patient's prognosis regarding survival. We report the case of a 26-year-old male patient who presented with impaired vision in the right eye, which had persisted for six months; neuroretinitis was diagnosed. Although infectious neuroretinitis was suspected, tests for pathogens such as Mycobacterium tuberculosis, Toxoplasma, Toxocara, and Bartonell

Comparative Evaluation of Routine and Special Staining Techniques in Fine Needle Aspiration Cytology of Lymph Nodes.

BACKGROUND AND OBJECTIVE: Fine needle aspiration cytology (FNAC) is a fast, inexpensive, and least invasive form of diagnosis, which is commonly used in the assessment of lymphadenopathy. In developing nations such as India, malignancies and tuberculosis (TB) are some of the most frequent causes of the enlargement of the lymph nodes. Although regular cytology stains are commonly used, they are not always sensitive to some infections and neoplastic diseases. The objectives of this research were to compare and evaluate diagnostic using routine and special staining methods in FNAC of lymph nodes.

Diagnostic utility of Xpert MTB/RIF ultra in clinically suspected tuberculous lymphadenitis in low-burden settings: a single center experience from Saudi Arabia.

BACKGROUND: Tuberculosis (TB) is a major global health issue, particularly among HIV-positive individuals. In Saudi Arabia, the incidence has declined to 8.4 per 100,000 but remains a concern among high-risk groups. Extrapulmonary TB, especially tuberculous lymphadenitis (TBL), is challenging to diagnose due to the low sensitivity of smear, culture, and histopathology. Xpert MTB/RIF Ultra (Xpert-Ultra) offers improved detection in paucibacillary disease, though local evidence remains limited. METHOD: We retrospectively reviewed 85 adult patients with suspected TBL at King Abdulaziz Medical Ci

Therapeutic drug monitoring of first-line anti-tuberculosis drugs: impact of dose modification on plasma drug concentrations in pulmonary and extrapulmonary tuberculosis.

BACKGROUND: Standard weight-based dosing of first-line anti-tuberculosis drugs does not consistently achieve therapeutic plasma concentrations in all patients. Inadequate drug exposure may contribute to poor treatment response, particularly in extrapulmonary and disseminated tuberculosis, where pharmacokinetic variability is more pronounced. Therapeutic drug monitoring (TDM) offers a strategy to individualize dosing and optimize plasma drug concentrations. METHODS: This observational analytical study enrolled 100 patients with pulmonary, extrapulmonary, or disseminated tuberculosis, of whom 8

Prevalence and spectrum of fungal pathogens in pulmonary tuberculosis patients.

Pulmonary fungal infections are increasingly recognized as significant co-infections in patients with pulmonary tuberculosis (PTB). Therefore, it is of interest to determine the prevalence and spectrum of fungal pathogens in patients with suspected and confirmed PTB. A cross-sectional study was conducted on 180 patients at MGM Medical College, Indore, India, with sputum samples processed for fungal culture. Fungal culture positivity was observed in 20% of the patients, with aspergillus species being the most commonly isolated pathogen. Thus, we show the need for routine mycological examination

Tuberculosis in Small Ruminants in Portugal: A Retrospective Laboratory-Based Study (2012-2023).

Animal tuberculosis caused by mycobacteria of the Mycobacterium tuberculosis complex (MTBC) represents a significant challenge to both public and animal health. Although eradication programs focus predominantly on cattle, small ruminants, especially goats, may play a relevant role as reservoirs. This study aimed to characterize tuberculosis cases in small ruminants in Portugal diagnosed at the national reference laboratory for Tuberculosis (Instituto Nacional de Investigação Agrária e Veterinária) over the period from April 2012 to July 2023. In this study, samples from 79 animals suspected of

Risk Factors Influencing the Specificity of Skin Tests for Bovine Tuberculosis in a Low-Prevalence Region.

BACKGROUND: Skin tests are the main official diagnostic method for granting, maintaining, and regaining the officially tuberculosis free (OTF) herd status in the European Union. Although highly specific, repeated testing may lead to the culling of animals not infected with bovine tuberculosis (bTB) and the suspension of OTF status, a problem especially relevant in low-prevalence regions where most herds are OTF. Identifying risk factors that affect the specificity of the single (SIT) and comparative (CIT) intradermal tuberculin tests is therefore essential to optimize control measures. MATERI

Diagnostic accuracy of stool Xpert MTB/RIF assay for adult pulmonary tuberculosis: a systematic review and meta-analysis.

BACKGROUND: The etiological diagnosis of adult pulmonary tuberculosis (PTB) mainly relies on sputum or other respiratory specimens. However, some suspected patients cannot provide adequate sputum, limiting conventional diagnostic methods. Stool is an easily obtainable, non-invasive specimen and, combined with nucleic acid amplification techniques, may offer a new adjunct approach for rapid PTB diagnosis. OBJECTIVE: To systematically evaluate the diagnostic accuracy of stool Xpert MTB/RIF for adult PTB. METHODS: PubMed, Web of Science, Embase, and the Cochrane Library were searched through Ap

Diagnostic Challenges and Scientific Management of Suspected Recurrent Kikuchi-Fujimoto Disease: A Case Report.

BACKGROUND Kikuchi-Fujimoto disease is a rare, benign, and self-limiting disease typically characterized by persistent fever and lymphadenopathy. The clinical manifestations are nonspecific, and consensus diagnostic criteria are currently unavailable; therefore, it is easily misdiagnosed as lymphoma, tuberculosis, or autoimmune diseases. Definite diagnoses rely on lymph node biopsy results. Recurrence of Kikuchi-Fujimoto disease is not uncommon, with recurrence rates varying across different age groups, ranging from 3% to 42.4%. Most patients have a favorable prognosis, while some progress to

Associations Between QuantiFERON-TB Gold Plus IFNγ Concentrations and Progression to Symptomatic Tuberculosis in Global High-Burden TB Settings.

BACKGROUND: Predictive biomarkers for symptomatic tuberculosis (TB) progression would transform targeted prevention efforts. Although interferon-gamma release assays (IGRAs), including QuantiFERON® TB-Gold Plus (QFT-Plus), have been studied for this purpose, systematic evaluation of the QFT-Plus TB1 and TB2 Interferon-Gamma (IFNγ) concentrations remains limited, particularly in high-burden TB settings. METHODS: Baseline TB1 and TB2 IFNγ concentrations from 5246 participants (ages 15-34 years) in TB-endemic regions were analyzed in relation to subsequent TB outcomes over a median of 525 days f
