Anomalous Right Coronary Artery in the Setting of Active Tuberculosis: A Multidisciplinary Management Challenge.

Anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA) is a rare congenital coronary anomaly, with an uncertain prevalence and often diagnosed incidentally. This case report presents a 62-year-old male with ARCAPA diagnosed during an evaluation for chest surgery. The patient had a history of colon cancer and active tuberculosis, complicating the clinical management. He reported chest pain, shortness of breath, and palpitations, with atrial fibrillation observed on a 24 h Holter ECG. Coronary angiography revealed robust collateral circulation and a suspected anomalous

Diagnostic utility of standardized assessment of ciliary ultrastructure by transmission electron microscopy in primary ciliary dyskinesia: a Japanese perspective.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare genetic disorder associated with structural or functional ciliary defects. Its diagnosis usually requires a combination of tests, including observation of the cilium structure by transmission electron microscopy (TEM). In Japan, when TEM was previously the primary method for diagnosing PCD, there were no standardized methods for evaluating TEM findings, and the correlations with PCD-causing genes remained unclear. METHODS: Here, we evaluated 101 patients with suspected PCD using a standardized assessment of ciliary ultrastructure by TEM,

Tuberculous Meningitis in a One-Year-Old Child With Isoniazid-Induced Hepatotoxicity: A Case Report.

Tuberculous meningitis (TBM) is predominantly observed in developing countries but remains relatively rare in developed countries. Therefore, if a clinician does not suspect TBM, its diagnosis may be delayed. Furthermore, drug-induced hepatotoxicity is common and can become severe during TBM treatment. Given the importance of multidrug regimens for TBM management, alternative drugs with favorable cerebrospinal fluid (CSF) penetration and high safety in terms of side effects are urgently required. We report a case of a one-year and 10-month-old Japanese boy who presented with an eight-day histo

Isolated splenic tuberculosis in a patient with rheumatoid arthritis.

Isolated splenic tuberculosis (TB) is rare. Here, we report a case of isolated splenic TB in a 70-year-old man with rheumatoid arthritis (RA). The patient presented to the emergency department with a 3-day history of epigastric pain and hematemesis. For RA treatment, a combination of methotrexate (8 mg) and baricitinib (4 mg) had been initiated 2 years prior. Abdominal computed tomography (CT) scan revealed upper gastrointestinal hemorrhage and intrasplenic involvement. Following endoscopic hemostasis, we performed endoscopic ultrasound-guided fine-needle aspiration via the stomach due to susp

Correlation between Vitamin D Levels and Troponin in Patients Suspected of Acute Coronary Syndrome: A Cross-sectional Intensive Care Unit Study.

INTRODUCTION: Vitamin D (VD) deficiency has become a global epidemic in the past 2 decades. Cardiac troponin is a specific biomarker for detecting myocardial injury, particularly in the context of myocardial infarction (MI), where elevated levels are indicative of myocardial necrosis. This study aimed to investigate the relationship between VD levels and troponin values in patients admitted to the Intensive Care Unit under suspicion of acute coronary syndrome, including comparisons with patients ultimately not diagnosed with ACS. MATERIALS AND METHODS: This cross-sectional study included a gr

Differentiating proven progressive disseminated histoplasmosis from other diagnoses in hospitalized persons with HIV and suspected progressive disseminated histoplasmosis: Findings from a clinical and demographic study in Mexico.

BACKGROUND: Progressive disseminated histoplasmosis (PDH) is a leading cause of morbidity and mortality among persons with HIV (PWH) in the Americas. Clinical presentation often overlaps with other opportunistic infections -especially tuberculosis (TB)- and sensitive diagnostics are frequently unavailable. In Mexico, epidemiological data on histoplasmosis in PWH are scarce. This study aims to describe the clinical and demographic characteristics along with final diagnosis of hospitalized PWH who had clinical suspicion of progressive disseminated histoplasmosis. METHODOLOGY/PRINCIPAL FINDINGS:

Clinical and immunological predictors of chronicity in erythema nodosum.

Background Erythema nodosum (EN) is a painful inflammatory disorder of the subcutaneous fat and the most common type of septal panniculitis. It often presents as erythematous, tender subcutaneous nodules, typically affecting the shins. EN is frequently a hypersensitivity reaction that can serve as a warning sign for an underlying systemic disorder, making it crucial to identify the cause. Though self-limiting in many cases, EN can sometimes become chronic, leading to significant discomfort. The pathogenesis of EN and the predictors of chronicity remain unclear, necessitating further investigat

A 30-year-old previously healthy man presented with fever and headache. HIV tests yielded negative results. Cerebrospinal fluid (CSF) analysis revealed pleocytosis (619/µL), elevated protein (210.3 mg/dL) and adenosine deaminase levels, and decreased glucose levels. A positive CSF culture for tuberculosis confirmed the patient had tuberculous meningitis (TBM). He was treated with methylprednisolone, isoniazid, rifampicin, pyrazinamide, and ethambutol (all highly sensitive). His compliance with medication was good. After six weeks of treatment, he was discharged in stable condition. Eight weeks

Bilateral Central Serous Chorioretinopathy Associated With Tuberculosis in a Female Patient.

We present a novel case of central serous chorioretinopathy (CSCR) associated with tuberculosis in a female patient. A 29-year-old female was referred with bilateral atypical CSCR following steroid treatment for suspected optic neuritis. Examination showed bilateral hypermetropic shift and foveal subretinal fluid (SRF). Optical coherence tomography (OCT) confirmed bilateral serous retinal detachment, with associated pigment epithelial detachments in the right eye. A raised erythrocyte sedimentation rate prompted multidisciplinary investigation, eliciting radiological and serological findings c

Immune responses of cattle vaccinated by various routes with Mycobacterium bovis Bacillus Calmette-Guérin (BCG).

BACKGROUND: Mycobacterium bovis BCG is the human tuberculosis vaccine and is the oldest vaccine still in use today with over 4 billion people vaccinated since 1921. The BCG vaccine has also been investigated experimentally in cattle and wildlife by various routes including oral and parenteral. Thus far, oral vaccination studies of cattle have involved liquid BCG or liquid BCG incorporated into a lipid matrix. Lyophilization is an established technique used for stabilizing bioproducts such as vaccines. METHODS: In the current study, cattle were vaccinated in two phases. In each phase, cattle w
