Incidence and outcomes of childhood status epilepticus in Kano, northern Nigeria.

OBJECTIVE: Convulsive status epilepticus (CSE) is a major neurological emergency in childhood, but population-based data from low- and middle-income countries remain limited. We estimated the incidence, mortality, neurological outcomes, and predictors of adverse outcomes among children with CSE in Kano, northern Nigeria. METHODS: We conducted a review of one-year surveillance data on childhood CSE across eight hospitals and a random sample of eight primary health centers in the Kano metropolis. Cases were identified using standardized definitions and weighted to account for sampling at primar

MassARRAY-based targeted detection of SLC22A5 mutations: A feasibility study for secondary screening of primary carnitine deficiency in newborns.

This study aimed to evaluate the feasibility of using MassARRAY molecular mass spectrometry to detect SLC22A5 gene mutations in second-tier screening for primary carnitine deficiency (PCD) among newborns. The goal is to provide technical evidence to support improvements in PCD screening and to reduce diagnostic delays. We first reviewed literature and databases to identify commonly reported pathogenic variants in the SLC22A5 gene among the Chinese population and established a hotspot mutation panel. MassARRAY analysis was conducted on dried blood spot samples using multiplex PCR, single-base e

Nocturnal blood pressure patterns in pediatric obstructive sleep apnea: The Kids Trial study.

UNLABELLED: Obstructive sleep apnea (OSA) in children is associated with cardiovascular risk, but its impact on sleep blood pressure (BP) remains incompletely understood. OBJECTIVE: To assess the association between the severity of OSA, stratified by the obstructive apnea-hypopnea index (OAHI), and altered sleep BP patterns, specifically the non-dipping pattern (NDP), using ambulatory 24-h BP monitoring (ABPM). METHODS: The Kids-Trial is a multicenter observational trial (NCT03696654) conducted in Spain, including children with suspected OSA that underwent polysomnography for its diagnosis.

EEG in nonconvulsive status epilepticus: The influence of clinical information.

INTRODUCTION: Blind EEG analysis, especially in the case of suspected nonconvulsive status epilepticus (NCSE), can lead to misdiagnosis. It is important to consider patient demographic and clinical information when interpreting EEG recordings, which constitutes a limitation of automatic seizure detectors. This study aims to quantify the impact of this information on EEG interpretation in cases of suspected NCSE. METHODS: EEG recordings from patients with suspected NCSE were analysed by 2 independent reviewers, first without and subsequently with access to clinical data. Inter-rater agreement

Sex-based differences in emergency department treatment times for acute ischaemic stroke: evidence from a large Italian cohort.

INTRODUCTION: Sex-related disparities in acute ischaemic stroke (AIS) care have been widely reported. However, evidence from Italy remains limited. We aimed to evaluate sex differences in access to revascularisation treatments (RTs) and key time performance indicators in a large Italian cohort. PATIENTS AND METHODS: We conducted a single-centre, retrospective, observational study including all adults admitted to the emergency department of a comprehensive stroke centre in Rome between January 2015 and December 2022 for suspected stroke. Clinical and demographic characteristics, comorbidities,

Molecular Characterization of Syndromic Hearing Loss in North African Moroccan Families.

Hearing loss (HL) is a common sensory disorder, with syndromic forms accounting for ~30% of genetic cases. Due to phenotypic and genetic heterogeneity, accurate diagnosis remains challenging. Exome sequencing (ES) offers a powerful tool to uncover the underlying genetic causes. This study aimed to investigate the genetic basis of syndromic hearing loss (SHL) in North African Moroccan patients through ES. Seven individuals with suspected SHL were recruited from Hassan II University Hospital, Fez. After clinical and audiological assessments, ES was performed to identify causal genetic variants.

Optimizing Screening for Obstructive Sleep Apnea: Comparative Assessment of STOP and STOP-BANG Questionnaires in Croatia, Türkiye, and Greece.

Background and Objectives: Obstructive sleep apnea (OSA) is a common disorder associated with significant cardiovascular, metabolic, and neurocognitive consequences. The STOP and STOP-BANG questionnaires are widely used screening tools for identifying individuals at increased risk of OSA. However, their performance may vary across populations. This variability is due to demographic and anthropometric differences. We aimed to analyze the screening accuracy of the STOP and STOP-BANG questionnaires across three distinct Mediterranean populations: Croatia, Greece, and Türkiye. Additionally, we aim

Reduced baroreflex sensitivity in situational syncope compared with vasovagal syncope: Evidence for different baroreflex profile.

BACKGROUND: Baroreflex sensitivity (BRS) reflects short-term cardiovascular autonomic regulation and contributes to blood pressure stability, a key determinant of cerebral perfusion. We aimed to assess BRS in patients with situational syncope (SS) and to compare it with that of patients with vasovagal syncope (VVS) under standardized conditions. METHODS AND RESULTS: We prospectively evaluated consecutive patients with suspected reflex syncope. Patients with confirmed SS or VVS and available BRS assessment at rest and during early orthostatic phase were included. The nearest neighbor propensit

Nocturnal Hypoxemia and Incident Coronary Artery Disease in Obstructive Sleep Apnea: Results from the Sleep Apnea Patients in Skaraborg Study.

BACKGROUND: The relationship between obstructive sleep apnea (OSA) and coronary artery disease (CAD) remains controversial. Although observational studies have suggested that OSA is associated with an increased risk of coronary events, substantial confounding and neutral findings from treatment trials have raised uncertainty regarding which aspects of OSA are most strongly linked to CAD risk. AIMS: This study aimed to examine the association between nocturnal hypoxemia, assessed using the oxygen desaturation index (ODI), and incident CAD. STUDY DESIGN: Prospective cohort study. METHODS: Adu

Assessing pragmatic abilities of small children with and without ASD diagnosis.

PURPOSE: DSM 5 indicates social communication deficit as one of the necessary criteria for the diagnosis of autism spectrum disorder. However, the pragmatic assessment of language is not as commonly considered as the formal aspects of language. The presented guide aims to consider communicative elements that include nonverbal aspects of language and the intentionality of communicative acts to identify the first signs of pragmatic alteration. METHODS: The ROLPP (Observation Script of Language from a Pragmatic Perspective) is an observation script for socio-cognitive and pre-verbal language ski

Perinatal arterial ischemic stroke - Is amplitude integrated EEG a helpful tool for the clinician?

BACKGROUND: Clinical symptoms of perinatal arterial ischemic stroke (PAIS) are subtle and include mostly seizures, apneas or focal neurologic deficits thus diagnosis is easily delayed or missed. PAIS is reliably diagnosed by magnetic resonance imaging (MRI) showing focal infarction in an arterial vascular territory. PATIENTS: We included 6 neonates with diagnosis of PAIS to the cohort of our study. METHODS: We investigated amplitude-integrated electroencephalography (aEEG) of six infants with PAIS as a potential tool for the clinician to facilitate early diagnosis of PAIS in this retrospecti

Current Treatment Options for Adult Grade 2 IDH-mutant Diffuse Gliomas in the Vorasidenib Era: Patient Selection, Sequencing, and Practical Management.

Management of adult WHO CNS grade 2 IDH-mutant diffuse gliomas has entered a more precise postoperative era. Vorasidenib has changed the discussion because it can slow progression and delay the next intervention in selected patients, but the key therapeutic question is not whether mutant IDH inhibition is active; it is where it should be placed relative to observation, reoperation, radiotherapy, and alkylating chemotherapy. We believe treatment should begin with maximal safe resection, integrated molecular diagnosis, and a careful assessment of residual non-enhancing disease, growth kinetics,
