Uncovering dual molecular diagnoses in families with complex phenotypes through structural and clinical studies of novel COL4A6 variants.

BACKGROUND: The relationship between observed clinical phenotypes and underlying genotypes is blended or skewed in multiple molecular diagnoses, complicating a comprehensive molecular genetic diagnosis. AIM: We report two families with dual diagnoses, using the deafness-associated gene, COL4A6, to exemplify its contribution to blended, complex clinical presentations. DESIGN: This is an observational study within a large, ethnically diverse rare disease cohort, focusing on families with hearing loss and suspected dual diagnoses, followed by functional and structural studies of novel variants.

[How I explore … diaphragmatic dysfunction].

Diaphragmatic dysfunction, whether unilateral or bilateral, partial or complete, is often an overlooked cause of dyspnea, sleep disturbances, and, in more advanced stages, chronic alveolar hypoventilation. It may result from injury to the phrenic nerve, the diaphragm muscle itself, or central nervous system pathways. The diagnosis is frequently suspected when an elevated hemidiaphragm is incidentally observed on chest imaging. A comprehensive assessment combining clinical examination, pulmonary function tests, arterial blood gas analysis, and targeted imaging is essential to confirm the dysfun

Presence of Autoantibodies against the Autonomic Nervous System in Primary Achalasia.

BACKGROUND: Primary achalasia is a rare disorder but a significant cause of esophageal motor dysfunction. The pathophysiology of achalasia is still unknown, although an autoimmune etiology is suspected. OBJECTIVES: To examine the presence of autoantibodies against autonomic nervous system receptors among primary achalasia patients. METHODS: In this observational cross-sectional study, we measure the levels of serum autoantibodies targeting G protein-coupled receptors of the autonomic nervous system, including adrenergic, muscarinic, endothelin, and angiotensin receptors. The study included 4

Peripheral and central nervous system toxicities of Bruton tyrosine kinase Inhibitors: a real-world pharmacovigilance study.

BACKGROUND: Bruton's tyrosine kinase (BTK) inhibitors have revolutionized the management of B-cell malignancies, yet emerging evidence indicates that they may be associated with nervous system disorders adverse events (AEs). Given their expanding therapeutic applications, especially in nervous system disorders, understanding their neurotoxicity profiles is of critical clinical importance. METHODS: We conducted a retrospective pharmacovigilance analysis using data from the United States. FDA Adverse Event Reporting System (FAERS) covering Q1 2013 to Q4 2024. Mitochondrial Dysfunction in Monogenic Developmental and Epileptic Encephalopathies.

BACKGROUND: Before diagnostic whole exome sequencing, monogenic/chromosomal developmental and epileptic encephalopathies (DEEs) were frequently misdiagnosed as mitochondrial disorders (MDs) with epilepsy, due to overlapping clinical and biochemical features. Assessing muscle functional assays in patients with a genetic diagnosis and epilepsy offers a unique opportunity to explore mitochondrial dysfunction in monogenic/chromosomal DEEs, in comparison to the mitochondrial dysfunction observed in genetically confirmed MDs. METHODS: In this retrospective cohort study, clinical and biochemical dat

Patient-derived glioblastoma cultures preserve respiration phenotypes during ex vivo maintenance and show sex-associated differences in migration.

Glioblastoma (GBM) is a highly heterogeneous, invasive brain tumor with profound metabolic plasticity. Patient-derived cultures provide valuable preclinical models, yet the stability and clinical relevance of their metabolic phenotypes during ex vivo maintenance remain unclear. Here, we performed longitudinal functional profiling of mitochondrial respiration, substrate utilization, and migration in primary patient-derived GBM cultures at one and five weeks ex vivo using Seahorse Bioanalyzer assays and wound-healing assays, complemented by exploratory integration with routine clinical parameter

A decade of change in delirium management: Temporal trends in psychotropic recommendations in a tertiary care hospital.

BACKGROUND: Delirium is a common acute neuropsychiatric syndrome in hospitalized adults. Emerging evidence and updated guidelines increasingly discourage routine antipsychotic use in delirium due to safety concerns. This study examined temporal trends in psychotropic recommendations for delirium across two three-year epochs in a tertiary-care hospital. METHODS: This retrospective study analyzed 1812 psychiatric consultations for suspected delirium-of which 1657 were confirmed-comparing two epochs (2016-2019 vs. 2022-2025). Psychotropic recommendations, haloperidol administration routes, and c

Recognition of giant cell arteritis and calcified carotid artery atheromas in dental practice: systematic review and meta-analysis.

OBJECTIVE: Giant cell arteritis (GCA) is a granulomatous vasculitis affecting medium- and large-sized arteries, most commonly the temporal arteries. Delayed recognition can lead to severe complications, including irreversible vision loss and stroke. Dentists routinely obtain panoramic radiographs for dental assessment, where calcified carotid artery atheromas (CCAA) may appear as incidental findings in the cervical region. This study systematically evaluated the incidence of suspected CCAA detected on panoramic radiographs obtained for dental indications and considered the clinical relevance o
