Telemedical care system for the treatment of arterial hypertension using recommendation algorithms to support diagnostic and therapeutic decisions-study protocol for a single-center, randomized, controlled, open-label SOT-ART-HT trial.

BACKGROUND: Arterial hypertension is a global issue and achieving the blood pressure control is a challenge. It has been suggested that telemonitoring benefits these patients but there is a lack of research evaluating tools to assist doctors in caring for patients with arterial hypertension. Herein, we detail a protocol for a single-center, randomized, open-label study with a control group to develop a blood pressure monitoring system with a built-in algorithm supporting the physician's decisions. METHODS: Two hundred patients aged 18-80 with a confirmed diagnosis of arterial hypertension and

A patient with patent foramen ovale associated ischemic stroke combined with hemorrhagic transformation: A case report.

RATIONALE: Magnetic resonance imaging (MRI) of patients with patent foramen ovale (PFO)-associated stroke often shows scattered, multiple ischemic lesions, mostly located in the cerebral cortex or vertebrobasilar artery. PFO occlusion is recommended for patients with large right-to-left shunt signals (>4 mm), moderate to large right-to-left shunts, and multiple ischemic lesions on imaging examination if an embolic event is suspected. PATIENT CONCERNS: A 66-year-old male was admitted to the hospital with dizziness and blurred vision for 16 hours. No other symptoms or signs were observed. DIAG

Cardiovascular safety of propranolol oral solution in infantile hemangiomas: a disproportionality analysis based on the FAERS database.

This study aims to explore the potential association between propranolol and cardiovascular adverse events (AEs) through real-world evidence. Reports from the FDA Adverse Event Reporting System (FAERS) database spanning from January 2014 to September 2024, identifying propranolol oral solution as the primary suspected (PS) drug, were analyzed. Disproportionality analysis employed four key metrics: Reporting Odds Ratio (ROR), Proportional Reporting Ratio (PRR), Bayesian Confidence Propagation Neural Network (BCPNN), and Multi-item Gamma-Poisson Shrinkage (MGPS) to identify positive signals of c

Cardiac cycle weighted ΜQFR for the assessment of functional relevance of the myocardial bridge.

Myocardial bridging (MB) causes coronary artery compression in systole, potentially resulting in angina and arrhythmias. Stent implantation in patients with MB may offer short-term improvement of symptoms, but carries a high risk of restenosis. We present a symptomatic patient with MB who was assessed by a novel angiography-based functional index that incorporates both the diastolic and systolic phases of the heart cycle - termed "cardiac cycle weighted µQFR" (µQFRccw). In addition to the observed dynamic systolic narrowing of the left anterior descending artery (LAD), a fixed stenotic (athero

A cohort study in HigAshi-nippon of Pulmonary hyPertensIoN in systEmic SclerosiS (HAPPINESS study): protocol and baseline data for an observational study.

BACKGROUND: Pulmonary hypertension (PH) is the leading cause of death among patients with systemic sclerosis (SSc). Recently, early therapeutic intervention to improve the prognosis was suggested, and the definition of PH was recently revised by lowering the cut-off value of mean pulmonary arterial pressure (mPAP) from ≥ 25 to > 20 mmHg. However, the optimal threshold for therapeutic intervention remains unclear. We aim to evaluate the prognosis of patients with SSc and its relationship with mPAP. METHODS: For this non-interventional retrospective and prospective cohort study, we enrolled pat

Association between red blood cell transfusion and subsequent cardiovascular events in patients admitted to the cardiovascular intensive care unit: a single-center retrospective study.

Anemia can worsen the prognosis of patients with acute cardiovascular (CV) disease; however, the effect of red blood cell (RBC) transfusion on mid-term outcomes in such patients requiring intensive care remains unclear. Therefore, this study investigated the association between RBC transfusions during hospitalization and subsequent CV events (all-cause mortality, non-fatal myocardial infarction or stroke, admission for acute heart failure [AHF], unstable angina, and other CV events) after hospital discharge in patients admitted to the cardiovascular intensive care unit (CICU). We retrospective

Mexican Patients With Suspected 22q11.2 Deletion Syndrome: Clinical Characterization and Molecular Findings by Fluorescence In Situ Hybridization and Multiplex Ligation-Dependent Probe Amplification.

BACKGROUND: The 22q11.2 deletion syndrome (22q11.2DS) is mostly caused by deletions of 3 and 1.5 Mb, referred to as typical deletions, although atypical deletions have also been reported. The commonest features are congenital heart disease, immunodeficiency, facial dysmorphism, and developmental delay. However, phenotypic variability is remarkable, and the underlying mechanisms remain poorly understood. OBJECTIVE: To determine copy number variations (CNVs) in the 22q11.2 region and their association with clinical manifestations in Mexican patients with suspected 22q11.2DS. METHODS: Fluoresce

Associating electrocardiographic abnormalities with coronary artery disease: insights into microvascular dysfunction from dynamic CT perfusion.

OBJECTIVES: Electrocardiographic (ECG) abnormalities serve as important predictors of future cardiovascular events. However, the specific cardiac abnormalities that bridge the gap between ECG abnormalities and subsequent events remain poorly understood. This study aimed to evaluate the relationship between ECG abnormalities and the prevalence of coronary artery disease (CAD) in propensity score-matched patients with suspected CAD who underwent comprehensive cardiac computed tomography (CT). MATERIALS AND METHODS: A total of 357 patients suspected of CAD underwent ECG and cardiac CT assessment

Intraplaque Hemorrhage Volume and Ischemic Stroke Risk.

BACKGROUND AND PURPOSE: Intraplaque hemorrhage (IPH) in carotid atherosclerotic plaques is the best established biomarker of plaque vulnerability. However, the relationship between IPH volume and ischemic neurologic symptoms remains scarcely studied. This study explored the association between carotid IPH volume and ischemic event severity. MATERIALS AND METHODS: A retrospective analysis was conducted on consecutive patients with suspected carotid atherosclerosis evaluated from December 2015 to January 2021. Patients underwent carotid plaque MRI using T1-weighted imaging with fat suppression

Diagnostic value of bone scintigraphy versus cardiovascular magnetic resonance in cardiac amyloidosis.

BACKGROUND: Accurate diagnosis of transthyretin amyloidosis cardiomyopathy (ATTR-CM) and its differentiation from light-chain (AL) cardiac amyloidosis (CA) cases (AL-CM) are of paramount importance. Surprisingly, comparative imaging data based on concurrent cardiovascular magnetic resonance (CMR) and bone scintigraphy in the same patients with biopsy-proven diagnosis of CA are still rare. METHODS: This was a real-world retrospective single-center study based on a local clinical care pipeline and we carefully analyzed clinical, laboratory, CMR, and bone scintigraphy data (and if necessary addi

Proposal for Diagnostic Criteria for Manifestations of Chronic Graft-versus-Host Disease in the Central Nervous System.

Atypical manifestations of chronic graft-versus-host disease (cGVHD) can affect both the central nervous system (CNS) and peripheral nervous systems presenting with diverse clinical and paraclinical features. These manifestations are often difficult to distinguish from other neurologic conditions, particularly those resulting from drug toxicity and opportunistic infections. To date, only rough diagnostic criteria for these rare manifestations have been proposed. The objective of this study was to contribute additional data to the existing literature and to develop more specific diagnostic crit

Longitudinal changes on cranial magnetic resonance imaging in relapsing giant cell arteritis.

OBJECTIVE: There is a need for better tools to monitor disease activity in giant cell arteritis (GCA). Prior studies demonstrated that vascular enhancement on cranial vessel wall magnetic resonance imaging (vw-MRI) decreases with treatment of GCA, but whether enhancement increases during relapse is not well known. This study examined changes on vw-MRI during relapse of cranial GCA. METHODS: Patients with active GCA acquired cranial vw-MRIs at enrollment and months 1, 6, and 12 and if suspected relapse occurred. Neuroradiologists graded vw-MRI enhancement for several structures. Changes in MRI

A disproportionality analysis of real-world adverse events associated with tirofiban from the FAERS database.

Tirofiban is a glycoprotein IIb/IIIa receptor blocker mainly used alongside other blood thinners in patients with acute coronary syndrome (ACS) undergoing procedures like angioplasty (PCI). Emerging clinical evidence suggests it may also be beneficial during endovascular treatment for ischemic stroke (IS). Because Tirofiban prevents blood clotting, it is important to assess how often it causes serious bleeding or other side effects when used in real-life settings. We used disproportionality analysis to review all FDA-reported adverse events linked to Tirofiban (from 2004 onward), focusing on c
