Pyogenic Sacroiliitis in Older Adults Presenting With Massive Retroperitoneal Abscess: A Case Report.

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

[Clinical analysis of 369 deaths within 30 days following surgical treatment of proximal femur fracture].

BACKGROUND: The 30-day mortality rate after surgical treatment of proximal femur fractures (PF) is reported in the literature to be up to 10%. Clinical studies in German-speaking countries on the suspected causes or underlying diagnoses have not yet been available, which is the reason for this study. MATERIALS AND METHODS: The retrospective single-center cohort study is based on a database that records PF. Patients who died within 30 days postoperatively between 2006 and 2025 were included. The primary study endpoint was the causes of death, as documented in the final discharge summary. The s

The Landscape of Tumor Dissemination in Leptomeningeal Disease: Mapping CTC Plasticity and Regional Heterogeneity Across Blood and CSF Compartments.

Background/Objectives: Leptomeningeal disease (LMD) is among the most severe complications of metastatic breast cancer, with median survival measured in weeks. Definitive diagnosis relies on neuroimaging and cerebrospinal fluid (CSF) cytology, which carries a false-negative rate of 40-50% at a single lumbar puncture. Improved tools for detecting and characterizing tumor cells across systemic and central nervous system (CNS) compartments are urgently needed. Methods: We applied an enrichment-free, fluorescence whole-slide imaging (fWSI)-based liquid biopsy platform to ante-mortem and post-morte

Laparoscopic resection of a low-grade appendiceal mucinous neoplasm in a patient with autosomal dominant polycystic kidney disease: a case report.

INTRODUCTION AND IMPORTANCE: Appendiceal mucinous neoplasms (AMNs) are rare, and preoperative diagnosis can be challenging, particularly when other abdominal cystic diseases coexist. Autosomal dominant polycystic kidney disease (ADPKD), which is characterized by multiple cystic formations, can complicate both differential diagnosis and surgical planning. CASE PRESENTATION: A 77-year-old man with ADPKD and chronic renal failure was referred to our hospital following a positive fecal occult blood test during an evaluation for anemia. Colonoscopy revealed a large submucosal tumor-like elevation

Quantification of plaque and detection of high-risk plaque characteristics based on coronary computed tomography angiography: comparison between superresolution deep learning reconstruction and hybrid iterative reconstruction.

BACKGROUND: In addition to stenosis evaluation, coronary computed tomography angiography (CCTA) also allows for noninvasive plaque quantification and characterization, which informs risk stratification and management strategies. This study aimed to compare a novel superresolution deep learning reconstruction (SR-DLR) technique with a conventional hybrid iterative reconstruction (HIR) method in quantifying CCTA-derived plaque and detecting high-risk plaque (HRP) characteristics. METHODS: A total of 255 consecutive patients (mean age 63.7±9.6 years; 184 men and 71 women) who underwent clinicall

Probable vestibular paroxysmia mimicking benign paroxysmal positional vertigo of the superior semicircular canal in the presence of a neurovascular conflict: a case report and review of the literature.

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment. CASE PRESENTATION: We describe the case of a 52-year-old French woman p

Use of lung ultrasound surface wave elastography in patients with interstitial lung disease.

OBJECTIVES: To assess whether lung ultrasound surface wave elastography (LUSWE), a non-invasive measure of lung stiffness, correlates with pulmonary function and high-resolution CT (HRCT) patterns in suspected interstitial lung disease (ILD). We hypothesised that LUSWE-derived lung stiffness would be associated with functional impairment and radiological ILD patterns. SETTING: Prospective, single-centre pilot study conducted in a tertiary referral ILD unit in Barcelona, Spain. PARTICIPANTS: 68 consecutive patients (41 men and 27 women) with suspected ILD were enrolled between June 2020 and F

18F-FDG PET/CT Metabolic Patterns in Clinically Probable Seronegative Autoimmune Encephalitis with Normal MRI Findings: A Retrospective Observational Study.

Seronegative autoimmune encephalitis (AIE) poses a significant diagnostic challenge because of the absence of identifiable neuronal antibodies and frequently normal structural neuroimaging. 18F-FDG PET/CT has emerged as a sensitive functional imaging modality for detecting metabolic abnormalities in such patients. We conducted this study to evaluate the metabolic patterns observed on 18F-FDG PET/CT in patients with clinically suspected seronegative AIE and to assess the diagnostic utility of 18F-FDG PET/CT when MRI findings are normal. Methods: This retrospective observational study included 1

Disease spectrum and genetic profile of high-risk children with severe carnitine deficiency.

BACKGROUND: Severe free carnitine deficiency (C0 ≤5 μmol/L) is frequently observed in pediatric patients suspected of primary carnitine deficiency (PCD), though confirmed diagnoses remain rare. This study aimed to analyze the disease spectrum and genetic profile of high-risk children with severe reduced carnitine levels and to enable targeted treatment. METHODS: We retrospectively analyzed blood acylcarnitine profiles (ACP), urine organic acids, genetic results, and clinical features of high-risk pediatric patients with significantly low C0 levels, which led to a diagnosis. RESULTS: Among 56

Haemaphysalis longicornis ticks as a competent vector for 'Candidatus Mycoplasma haemobos' in experimental transmission studies.

'Candidatus Mycoplasma haemobos' ('Ca. M. haemobos') is an emerging hemotrophic mycoplasma affecting cattle and other animals worldwide. While arthropod vectors have been suspected in its transmission, experimental evidence confirming vector competence remains limited. We investigated the ability of the hard tick Haemaphysalis longicornis (H. longicornis) to acquire, maintain, and transmit 'Ca. M. haemobos' under laboratory conditions. Ticks were exposed to the pathogen via microinjection or by feeding on infected BALB/c mice. Transovarial and transstadial transmission were assessed using quan

Do Labial Minor Salivary Gland Biopsies Maintain Diagnostic Sensitivity and Specificity for Sjögren's Disease - An Institutional Study.

BACKGROUND: Current American College of Rheumatology/European League Against Rheumatism (ACR-EULAR) guidelines indicate a positive biopsy with a score of ≥1 using focus scores (FS) to quantify lymphocytic foci presence. Accurate labial minor salivary gland biopsy (LMSGB) diagnosis depends on proper biopsy sampling, observer interpretation, and FS. There are fewer studies that evaluate the sensitivity and specificity of histology and serological testing. There was a substantial association between FS and the serological test, according to one study, but not in another. AIM: To determine the di

Diagnostic Accuracy of Point-of-Care Ultrasound (POCUS) for Suspected Acute Appendicitis in Pediatric and Adult Emergency Departments: A Systematic Review and Meta-Analysis.

Acute appendicitis is a common surgical emergency that often requires diagnostic imaging. While computed tomography (CT) is highly accurate, it involves ionizing radiation and potential delays. Point-of-care ultrasound (POCUS), performed by emergency physicians, offers a rapid, radiation-free alternative, but its diagnostic accuracy varies across studies. This systematic review and meta-analysis evaluates the diagnostic performance of POCUS for acute appendicitis in pediatric and adult populations. Major databases were searched for prospective and retrospective studies evaluating POCUS for app
