Barriers and Facilitators to Point-of-Care Ultrasound Utilization Among Emergency Medicine Residents and Emergency Nurse Practitioners Following Institutional Training.

PURPOSE: Point-of-care ultrasound (POCUS) serves as a critical diagnostic tool in emergency medicine; however, a persistent gap remains between competency acquired during training and consistent clinical application. This study sought to identify barriers and facilitators influencing POCUS examination frequency among emergency medicine (EM) residents and emergency nurse practitioners after institutional training, and to examine the association between individual-level factors and the frequency of POCUS examinations. PATIENTS AND METHODS: This retrospective study was conducted in the Departmen

Relative importance of fall risk factors in identifying falls among community-dwelling older adults: A cross-sectional retrospective study.

This study determined the relative strength of the association between fall risk factors and falls among community-dwelling older adults. Ninety-four older adults participated in this cross-sectional study. Their fall history in the past year and four categories of fall risk factors, including sensory (foot tactile sensation), motor (muscle strength and power), physical (balance, mobility, ankle range of motion, and physical activity level), and mental (fear of falling and cognitive functions), were collected. Logistic regression indicated that sensation, leg muscle power, and knee extensor st

Competing narratives and public support for drink-drive policy reform: a framing experiment in England and Wales.

Lowering legal blood‑alcohol concentration limits for driving is an evidence‑based policy to prevent harms. In England and Wales, proposals to reduce the drink-drive limit from 80 mg to 50 mg alcohol per 100 ml of blood have generated competing narratives, including alcohol-industry claims about harm to rural pubs and public-health arguments emphasising road safety. We conducted a pre‑registered online experiment with adults in England and Wales (N = 495) to test how these narratives shape policy support. Participants read one of three news‑style articles framing the proposed reduction as alco

Longitudinal Non-interventional Changes of the FORTA Score are Associated with Changes of Cognitive and Physical Function Tests in Community-Dwelling Older People.

BACKGROUND: Listing tools were found to ameliorate drug treatment in older people; the FORTA (Fit-fOR-The-Aged) list is a clinically validated positive-negative list of medication appropriateness. Here, we retrospectively analyze longitudinal correlations between the FORTA score and key measures of physical and cognitive function in older people. METHODS: 504 participants of a multi-center cohort study (AgeCoDe/AgeQualiDe) for whom the FORTA score (sum of over- and under-treatment errors) had been assessed were studied at three follow-up (FU) time points (FU 6-8; mean age range 87.9-89.7 year

Global prevalence of underweight and overweight/obesity among migrant workers: a systematic review and meta-analysis.

BACKGROUND: Migrant workers are increasingly recognised as a nutritionally vulnerable population due to unstable working conditions, disrupted food environments, and limited access to health services. However, global estimates of malnutrition in this group remain fragmented. We aimed to identify the global prevalence of underweight and overweight/obesity among migrant workers and identify key moderating factors. METHODS: We conducted a systematic review and meta-analysis following PRISMA guidelines. We searched seven databases without restrictions on language, region, or year of publication.

Artificial intelligence in medical education: Performance of ChatGPT versus medical students on medical examination.

Attention toward Chat Generative Pre-trained Transformer (ChatGPT) has greatly increased, including in the medical field, but concerns about its problem-solving ability, accuracy, reliability, and limitations in medical problems always coexist. This study aimed to evaluate ChatGPT's performance in solving a medical school examination. We used 50-question examinations from the preventive medicine and psychiatry fields applied to 41 fourth-year medical students. Forty-nine questions were separately entered (one was excluded for being image-based) into the text box of ChatGPT models 3.5 and 4.0 w

Lower-Limb muscle power Determines reactive balance stability following Slip-Like perturbations in healthy young and older adults.

Lower-limb muscle power is considered a key determinant of neuromuscular performance, yet its relationship with reactive balance remains poorly understood across the adult lifespan. This study examined the association between countermovement jump (CMJ)-derived peak muscle power and center-of-mass (CoM) stability following a slip-like balance perturbation in young and older adults. Fifty-two healthy adults (27 older, ≥65 years; 25 younger, <65 years) underwent CMJ testing and unexpected platform perturbations. Reactive balance performance was quantified as CoM stability at first post-perturbati

Muscle force responses during initial-contact gait perturbations and recovery steps.

BACKGROUND: Responses to gait perturbations are critical for maintaining dynamic stability and preventing falls, yet muscle-level mechanisms underlying these responses remain insufficiently understood, particularly during the initial contact (IC) phase. RESEARCH QUESTION: How do individual lower-limb muscle forces adapt during perturbations at IC and in the subsequent recovery step? METHODS: Twenty-one healthy young women walked on a dual-belt treadmill while perturbations were induced via sudden belt deceleration at IC. Motion capture and ground reaction force data were processed in OpenSim

Does Liposomal Bupivacaine Provide Superior Postoperative Analgesia Compared With Ropivacaine With Dexamethasone in Suprainguinal Fascia Iliaca Compartment Block for THA? A Randomized Controlled Trial.

BACKGROUND: Choosing the most effective pain management is important for enhancing functional recovery after surgery. The fascia iliaca compartment block (FICB) is recommended for analgesia after THA, but whether liposomal bupivacaine is more effective for this application remains uncertain. QUESTIONS/PURPOSES: (1) Did a single-injection suprainguinal FICB using liposomal bupivacaine provide superior pain control by a clinically important margin? (2) What was the effect of liposomal bupivacaine on perioperative opioid consumption? (3) How did liposomal bupivacaine affect the duration of the s

Grab bar loading resulting from unexpected balance loss during bathing transfers.

BACKGROUND: Bathtub transfers can be challenging for older adults, and may result in balance loss. While grab bars can be used to help prevent a fall during a balance perturbation, it is unclear what loading demands are placed on grab bars during balance recovery, and how loading differs between proactive and reactive grasp strategies, or between older and younger adults. METHODS: Twenty healthy younger adults (20-33 years) and 10 healthy older adults (65-74 years) underwent simulated bathing transfers in a slippery bathtub. The younger adults used proactive or reactive grasp strategies, and

Examination of driver license renewal policies and older driver crash and injury rates by rurality.

BACKGROUND: Among adults aged 65 and older, motor-vehicle crashes are the second leading cause of injury-related death-following falls. State driver license renewal laws commonly have provisions targeting older drivers, but limited evidence exists on their effectiveness in reducing crash and injury rates and how this may vary by rurality. This study aimed to investigate the impact of state driver license renewal policies on older driver crash and injury outcomes, by rurality. METHODS: Crash data, license renewal policies, and other relevant state policies were drawn from 13 U.S. states for th

Correlation between BAC and abdominal muscle alcohol concentration: a promising parachute matrix in cases of severe injury.

Ethanol represents the most abused drug worldwide, and among the most frequently evidenced in toxicological analysis on drug-induced/related deaths. When a blood sample is not available in a death characterized by severe trauma an alternative matrix is needed for an estimation of impairment. Presented is the correlation of postmortem blood and rectus abdominis muscle ethanol concentration. Ethanol concentrations in both blood (BAC) and muscle were determined by head space gas chromatography/mass spectrometry. Twenty-nine forensic cases with positive BAC were selected for the correlation study.

Association of intradialytic hypotension and physical performance with fall and fracture incidence in hemodialysis patients: The REPnet-HD study.

Although hypotension has been linked to fall-related injuries in older adults, evidence in hemodialysis (HD) patients remains limited. This study examined the effects of intradialytic hypotension (IDH) and physical performance on the risk of subsequent falls and fractures. In this prospective cohort study, 518 ambulatory HD patients were observed over 1 year to track the occurrence of accidental falls, severe falls (those requiring medical intervention), and fractures. Baseline assessments included IDH (defined as nadir systolic blood pressure [BP] during dialysis <90 mmHg or intervention for

Mental health after a critical incident: the role of parental status and prior training in firefighters.

INTRODUCTION: In this brief research report, we postulated that operations involving children can increase stress for first responders. Here, a traffic accident in which a mother and her child were fatally wounded was investigated. We postulated that firefighters with children and firefighters without pre-training experience would experience greater psychological impact when responding to this incident. METHODS: All firefighters who responded to the incident could participate voluntarily. Participants (n = 49) were categorized by parental status (n = 24 parents; n = 25 non-parents) and pre-de

Isomer Composition Assessment of Synthetic Phosphatidylethanol by Collision-Induced and Ozone-Induced Dissociation Mass Spectrometry.

The direct blood-alcohol biomarker phosphatidylethanol (PEth), especially its most abundant analogue 1-palmitoyl-2-oleoyl-sn-phosphatidylethanol (PEth 16:0/18:1) has gained increasing relevance in clinical and forensic applications for assessing alcohol consumption. Accurate quantification of PEth is essential to reliably differentiate between abstinence, moderate alcohol consumption and excessive alcohol intake. Measurement accuracy of PEth 16:0/18:1 by well-established liquid chromatography-tandem mass spectrometry (LC-MS/MS) approaches such as multiple reaction monitoring (MRM) can be confo

Maternal deprivation and adolescent alcohol exposure induce sex-dependent alterations in stress-related behavior and lipid signaling in rats.

BACKGROUND: Early-life stress constitutes a major risk factor for the development of neuropsychiatric and substance use disorders, exerting enduring effects on stress responsivity and emotional behavior. Maternal deprivation (MD) is a well-established model of early adversity that induces persistent neurobiological alterations. During adolescence, alcohol exposure presents an additional challenge that may interact with early-life stress, potentially influencing long-term adaptations in a sex-dependent manner. Among the systems involved, lipid signaling pathways, including the endocannabinoid s

Differential modulation of haematopoietic and oxidative injury by PARP-1 and ATR kinase inhibition in a murine model of acute irradiation.

UNLABELLED: Temporal Modulation of Acute Radiation Injury by Post-Exposure PARP-1 versus ATR Kinase Inhibition. BACKGROUND: PARP-1 and ATR inhibitors have been employed as radiosensitizers in cancer therapy. In addition to this, there is little evidence about the role of these chemicals after a dose of radiation, specifically in relation to reducing damage to healthy cells rather than increasing it. OBJECTIVE: The goal of this study was to determine if PARP-1 or ATR kinase inhibitors, when delayed administered, could decrease the severity of the condition caused by an acute dose of radiation
