Systems analysis of clinical incidents: development of a new edition of the London Protocol.

The investigation of incidents and accidents, together with subsequent reflection and action, is an essential component of safety management in every safety-critical industry, including healthcare. A number of formal methods of incident analysis were developed in the early days of risk management and patient safety, including the London Protocol which was published in 2004. In this paper, we describe the development of a new edition of the London Protocol. We explain the need for a revised and expanded version of the London Protocol, addressing both the changes in healthcare in the last two de

A Case of Displaced Cricoid Cartilage Fracture Successfully Treated With Non-Surgical Intervention.

Laryngeal trauma, particularly cricoid fractures, is rare but potentially life-threatening and often requires invasive airway management. However, factors contributing to successful conservative treatment remain unclear. We report the case of a 40-year-old woman diagnosed with a displaced cricoid cartilage fracture following a motorcycle accident. Despite the risk of upper airway obstruction due to a massive hematoma observed on fiberoptic laryngoscopy and a displaced cricoid cartilage fracture, the patient was successfully managed conservatively without tracheotomy or endotracheal intubation

Scanning electron microscopy and extended viability testing as a tool to evaluate the safety of MALDI-TOF extracts for risk group 3 spore-forming bacteria.

Introduction. Matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) MS for rapid identification of risk group 3 (RG3) bacteria is impeded by the following two main limitations: (a) equipment and maintenance costs for instruments placed within containment and (b) lack of a validated inactivation protocol to move RG3 material to a lower containment level. A validated inactivation method would improve operations of public health laboratories by allowing safe triage of potential RG3 agents. Albeit a validated, zero-risk inactivation protocol is unlikely, scientific interrogation o

Who takes a recovery step? Predictors of step execution after tripping in individuals with stroke.

BACKGROUND: A recovery step is a crucial balance reaction that prevents falls in individuals with stroke. However, the characteristics of this response after tripping remain unclear, particularly regarding which limb is used for stepping and the factors associated with whether a step is taken. RESEARCH QUESTION: This study aimed to identify the factors associated with whether a recovery step was taken following a trip to support the development of effective fall prevention strategies for individuals with stroke. METHODS: A large-scale motion capture database comprising 41,943 gait trials was

Osseointegrated prostheses reduce reactive stability demands during walking in Service members with unilateral transfemoral amputation.

BACKGROUND: Heightened fall risk following lower limb amputation can lead to adaptations during walking that increase proactive stability (e.g., a wider base of support). However, such compensations also reduce maneuverability, increasing reactive stability demands (e.g., corrective steps) as more lateral foot placement necessitates greater lateral impulses. By improving prosthetic control, osseointegration (OI) may mitigate fall risk and reduce the need for proactive stability compensations. Thus, this work aims to characterize proactive stability compensations, via mediolateral margins of st

Prediction of individualised 6-month mortality risk in opioid use disorder.

BACKGROUND: People with opioid use disorder (OUD) have substantially higher standardised mortality rates compared with the general population. However, lack of individualised prognostic information presents challenges in personalisation of addiction treatment delivery. AIMS: To develop and validate the first prognostic models to estimate 6-month all-cause and drug-related mortality risk for people diagnosed with OUD using indicators recorded at baseline assessment in addiction services in England. METHOD: Thirteen candidate prognostic variables, including sociodemographic, injecting status a

Driving performance in the morning after bedtime vornorexant administration: A randomized clinical trial using a driving simulator.

AIM: Sedative hypnotics can impair driving performance to a degree comparable to blood alcohol concentrations of 0.05%-0.08%. This study aimed to evaluate the residual effect of vornorexant, a novel orexin receptor antagonist, on next morning driving performance relative to a predefined clinically meaningful threshold. METHODS: Participants received 10 and 20 mg of vornorexant and placebo for eight consecutive nights and zopiclone on Days 1 and 8 in a four-way crossover and double-blind manner. Driving performance at 9-h post-dose was assessed measuring the standard deviation of the lateral p

Individualized cognitive behavioral therapy for chronic pain in Japan: study protocol for a multicenter randomized controlled trial.

BACKGROUND: Chronic pain imposes a substantial burden on individuals and society. Cognitive behavioral therapy for chronic pain (CBT-CP) has proven effective internationally, but randomized evidence in Japan is scarce. The first Japanese trial was a single-center, 16-session videoconference program that prioritized pain intensity. In contrast, this multicenter trial tests a brief, face-to-face, individualized CBT-CP program with quality of life (QoL) as the primary outcome. METHODS: This open-label, randomized, parallel-group superiority trial enrolls 60 adults aged 20-80 years with chronic p

Examination of the DSM-5 level 2 substance use measure using Rasch modeling with United States parents.

BACKGROUND: The DSM-5 Substance Use Disorder (SUD) Adult Level 2 measure is one of the new Cross Cutting (CC) measures recommended by the American Psychiatric Association to evaluate symptoms across multiple disorders with two levels. While widely used, very little is published on the validity and reliability of the CC measures and none to date on the SUD level 2 measure, which examines the use of substances in the last 7 days. METHODS: Employing a scale development and validation framework, this study examined reliability and validity evidence for the SUD level 2 CC measure with a sample of

A geospatial analysis of subway-related fatalities in New York City (2008-2021).

The New York City (NYC) subway system is one of the largest rapid transit systems in the world. With an annual ridership of 1 billion people, subway-related fatalities present a significant public health concern. Properly contextualizing such fatality data is imperative to understanding relevant risk factors. This paper reviews all subway-related fatalities in NYC occurring between 2008 and 2021 (n = 838). Descriptive statistics for fatalities considered manner of death, decedent demographics, season of death, and day of the week when the death occurred. In addition, a geospatial approach was

Temporal Dynamics of Subtle Cognitive Change: Validation of a User-Friendly Multidomain Digital Assessment Using an Alcohol Challenge.

BACKGROUND: Clinical trials in neurological and psychiatric indications are hampered by poor measurement fidelity in currently used "standardized" rating scales. Digital, repeatable tests that can be remotely administered offer a more fine-grained understanding of the patient's clinical trajectory. Several such tools are being developed, but only a few have been validated in terms of their ability to discern and describe change over time-a critical element of clinical trials. OBJECTIVE: Four cognitive tasks from a digital battery (delivered via tablet) are administered at high frequency follo

Sex- and ALDH2-dependent differences in alcohol metabolism and psychomotor performance: a study in Han Chinese adults after binge drinking.

BACKGROUND: Psychomotor impairments due to alcohol consumption may lead to a series of negative consequences. However, the influence of sex and ALDH2 polymorphism on psychomotor dysfunction has not yet been investigated. METHODS: One-hundred and three participants, genotyped for ALDH2 rs671, were administered a dose of 1.0 g/kg of white spirits. The blood ethanol concentration (BEC) and acetaldehyde concentration (BAAC) were measured at specific time intervals before and after alcohol consumption. Additionally, auditory simple reaction time (ASRT), visual choice reaction time (VCRT), pursuit

Self-underestimation of BAC as a predictor of risky driving in heavy drinkers.

BACKGROUND: Alcohol-related driving fatalities persist as a public health issue in the United States. Recent studies have focused on the role of driver risk-taking in DUI occurrences and consequent driving fatalities. This laboratory study examined simulated driver risk-taking under an acute dose of alcohol and a placebo to test the degree to which individual differences in risk-taking were predicted by drivers' self-appraisal of their intoxication and the degree to which alcohol impaired their inhibitory control. METHODS: Eighty young adult drivers (41 men and 39 women) received 0.65g/kg alc

Mapping acute alcohol effects on bodily sensations: A cross-dimensional interoceptive approach.

BACKGROUND: Interoceptive processes may underlie maladaptive patterns of alcohol use. Bodily sensations experienced during alcohol intoxication could therefore reveal distinct mechanistic components relevant for addiction theory and research. Here, we apply novel tools to examine how intoxication impacts somatic awareness using bodily maps and a cardiac interoception task. METHODS: In a double-blind, within-subjects, placebo-controlled study, social drinkers (n = 37, 17 female) were administered 0.4 g/kg of alcohol. We measured changes in self-reported bodily sensations during the ascending a

Measuring Alcohol-Induced Striatal Dopamine Release in Healthy Humans With [11C]-Raclopride: A Meta-Analysis.

Alcohol consumption is known to affect dopamine (DA) release in the brain, with significant implications for understanding addiction and its neurobiological underpinnings. This meta-analysis examined the effects of acute alcohol administration on striatal DA release in healthy humans as measured with [11C]-raclopride positron emission tomography (PET). Oral alcohol administration was associated with a significant reduction in [11C]-raclopride binding potential (BPND) in the ventral striatum (Cohen's d = -0.76), indicative of increased DA release, particularly at lower blood alcohol concentrati

A within-subject, double-blind, placebo-controlled randomized evaluation of the combined effects of cannabidiol and hydromorphone in a human laboratory pain model.

Preclinical and epidemiological evidence supports that cannabinoids may have opioid-sparing properties and could be one strategy to decrease opioid use and associated harms like overdose and extramedical use. The objective of this within subjects, double-blind, double-dummy, randomized human laboratory trial was to examine whether cannabidiol (CBD) increases opioid analgesic effects and whether there are corresponding increases in other opioid mediated effects. Healthy participants (N = 31) attended 5 outpatient sessions where they received the following drug conditions: (1) placebo + placebo,
