The Rare Case of a Sertoli Cell Tumor of the Adrenal Gland in a Man With Pathological Dexamethasone Suppression Test: A Case Report.

Sertoli cell tumors are rare gonadal stromal tumors. Extragenital manifestations, particularly in the adrenal gland, are exceptionally uncommon. We present a case of an adrenal Sertoli cell tumor in a male patient, presenting with a pathological (1 mg overnight) dexamethasone suppression test. An 82-year-old otherwise healthy man was referred for evaluation of an incidentally detected left adrenal mass. The patient was asymptomatic with no clinical signs of catecholamine excess, hypercortisolism, hyperaldosteronism, or B-symptoms. Initial CT scan revealed a 44 × 42 mm lesion, which increased t

Long-Term Growth Outcomes of Children With Type 1 Diabetes According to Glycemic Control and Use of Continuous Glucose Monitoring: A Retrospective Cohort Study.

BACKGROUND: Chronic diseases such as type 1 diabetes mellitus (T1DM) may alter linear growth; however, reports regarding growth in children with T1DM have been inconsistent. This study aimed to investigate the height and growth velocity of patients with T1DM, and whether they were affected by various factors 5 years after the diagnosis. METHODS: This retrospective study included patients with T1DM between October 2005 and May 2022, with a follow-up period of at least 1 year. Patients with diabetes, thyroid disease, celiac disease, or any other chronic disease were excluded. We compared the me

Pulmonary exacerbations in cystic fibrosis after fatherhood with and without CFTR modulators.

BACKGROUND: Advances in cystic fibrosis (CF) management, quality of life, and fertility treatments have encouraged more men with CF (mwCF) to pursue parenthood. However, data on the impact of fatherhood on CF disease course, particularly in the era of CFTR modulators (CFTRm), remain limited. METHODS: We conducted a multicenter retrospective cohort study in six CF centers (2007-2023). Demographic and clinical parameters, including forced expiratory volume in 1 s (FEV1), body mass index (BMI), bacterial colonization, and pulmonary exacerbation (PEx) rates were compared for the year before and t

Primary Multifocal Hepatic Gastrointestinal Stromal Tumor Mimicking Metastatic Disease: A Diagnostic Challenge With PET-CT and Histopathological Correlation.

Primary hepatic gastrointestinal stromal tumor (PHGIST) is an exceptionally rare extra-gastrointestinal stromal tumor arising within the liver. It poses a significant diagnostic challenge because hepatic involvement in gastrointestinal stromal tumor (GIST) is more commonly metastatic. Multifocal primary hepatic presentation is even less frequently described. We report a case of a 62-year-old male presenting with constitutional symptoms, wherein contrast-enhanced computed tomography (CECT), magnetic resonance imaging (MRI), and PET-CT revealed multiple lobulated solid lesions involving the righ

Evolution of Nerve Conduction Demyelinating Parameters Between Baseline and Treatment Cessation May Not Be Predictive of Relapse in IVIg-Responsive CIDP Patients.

INTRODUCTION/AIMS: Identifying factors associated with short-term relapse is essential for tailoring maintenance treatment on a case-by-case basis in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). We sought to determine whether changes in demyelinating electrophysiological features during treatment could predict relapse after treatment discontinuation in a cohort of CIDP patients. METHODS: We conducted a retrospective study of patients with CIDP treated with intravenous immunoglobulin (IVIg), all of whom had a first nerve conduction study (NCS) at treatment initiation or du

Severe Dermatologic Toxicity During Amivantamab-Lazertinib Treatment in Epidermal Growth Factor Receptor (EGFR)-Mutated Non-Small Cell Lung Cancer.

Amivantamab is an epidermal growth factor receptor (EGFR) and mesenchymal-epithelial transition (MET) antibody. Lazertinib is an EGFR tyrosine kinase inhibitor with central nervous system penetration. Used in combination, this targeted therapy is effective in treating non-small cell lung cancer (NSCLC). However, it has been associated with severe dermatologic adverse reactions. A 61-year-old woman with a history of EGFR-mutated NSCLC with brain metastases on amivantamab-lazertinib therapy presented for follow-up with drainage from her scalp incision after undergoing a right parietal craniotomy

Radiation-Induced Pleomorphic Dermal Sarcoma of the Chest Wall 25 Years After Breast Cancer Radiotherapy: A Case Report and Literature Review.

Radiation-induced sarcoma (RIS) is a rare but aggressive long-term complication of radiotherapy. It typically develops within previously irradiated tissues after a prolonged latency period and is associated with poor prognosis because of its locally invasive behavior and high recurrence rates. Breast cancer survivors who receive adjuvant radiotherapy remain at risk for secondary malignancies, including sarcomas arising within the radiation field. We report the case of a 78-year-old woman who developed a progressively enlarging chest wall mass 25 years after mastectomy and adjuvant radiotherapy

A Case of Drug-Induced Liver Injury Associated With Saw Palmetto.

Saw palmetto (SP) is a widely used herbal supplement for benign prostatic hyperplasia (BPH) and is generally considered safe. However, rare cases of SP-associated drug-induced liver injury (DILI) have been reported. We describe a case of cholestatic hepatitis attributed to SP, highlighting the diagnostic challenges associated with herbal supplement use and polypharmacy. A 70-year-old male with a history of BPH, hypertension, chronic obstructive pulmonary disease, atrial fibrillation, and cholecystectomy presented with one month of jaundice, pruritus, and pale stools. Diagnostic and Therapeutic Perspectives on Parotid Mammary Analog Secretory Carcinoma: Two Case Reports and Literature Review.

Mammary analog secretory carcinoma (MASC) is a rare malignant tumor of the salivary glands that exhibits significant morphological, immunohistochemical, and molecular similarities with secretory carcinoma of the breast, including the characteristic ETV6-NTRK3 gene fusion. The diagnosis of this condition remains challenging due to its histopathologic overlap with other low-grade salivary gland tumors, such as acinic cell carcinoma and mucoepidermoid carcinoma. The following report details two cases of MASC manifesting in the right parotid gland of a 37-year-old male and a 23-year-old female. Bo

Adult Onset Hirschsprung's Disease With Overlapping Presentation of Sigmoid Volvulus: Two Case Reports With Literature Review at Tikur Ambessa Specialized Hospital, AAU, Ethiopia.

Adult-onset Hirschsprung's disease (HSD) is uncommon and frequently leads to diagnostic difficulty after infancy. It is a congenital illness that causes intestinal obstruction due to a lack of ganglion cells. Approximately 1 in 5000 live newborns have this illness, and 95% of HSD cases are identified in infants. This case report will fill a knowledge gap at a low-resource setting where published papers on this topic are extremely rare, and will provide insight into diagnosis and surgical management options that are feasible in resource-limited environments with good outcomes. In this case repo

Thyroid metastasis from lung adenocarcinoma: diagnostic pitfalls and individualized management in a six-patient case series and literature review.

BACKGROUND: Clinically recognized thyroid metastasis from lung adenocarcinoma is rare and may mimic a primary thyroid malignancy because imaging, cytologic, and immunophenotypic findings overlap. We aimed to characterize the diagnostic pitfalls, management, and outcomes of 6 consecutive patients and to define the limited circumstances in which thyroid-directed treatment may be reasonable. CASE DESCRIPTION: Six patients with pathologically confirmed thyroid metastasis from lung adenocarcinoma were treated at an academic tertiary cancer hospital between September 2018 and September 2023. Electromyographic biofeedback or neuromuscular electrical stimulation added to isometric quadriceps exercise does not improve pain and functional outcomes in knee osteoarthritis: a randomized controlled trial.

Quadriceps muscle weakness is a common clinical feature in individuals with knee osteoarthritis (KOA) and contributes to pain, functional limitations, and reduced quality of life. While quadriceps strengthening exercises are well-established, the additional benefits of electromyographic biofeedback (EMG-BFB) and neuromuscular electrical stimulation (NMES) remain inconclusive. This single-blind randomized controlled trial investigated whether EMG-BFB or NMES provides added benefits when combined with isometric quadriceps strengthening exercises in patients with mild-to-moderate KOA. Effects of improved visual appearance on clinical outcomes after total knee arthroplasty: a randomized controlled trial.

BACKGROUND: Despite advancements in total knee arthroplasty (TKA), some patients remain dissatisfied postoperatively, potentially due to subjective perceptions of knee appearance. This study investigated whether visual feedback on improved knee alignment and appearance after TKA influences pain perception and patient-reported outcomes. METHODS: In this prospective, double-blind, randomized controlled trial, 60 patients undergoing primary TKA were randomized into intervention or control groups (n = 30 each). Pre- and postoperative radiographs, photographs, and gait videos were collected. Patie

Hippocampal volume trajectory and its relation to clinical course during and following ECT-treatment in patients with depression: A systematic review and meta-analysis.

BACKGROUND: Neuroimaging studies in humans and translational animal models consistently demonstrate ECT-induced hippocampal volume increases. However, evidence linking hippocampal volume changes to clinical improvement in depression has been inconsistent. This systematic review with meta-analysis extends previous work to investigate whether hippocampal volume changes following the completion of an ECT-index series are associated with the clinical course of depression. METHODS: We conducted a systematic review and meta-analysis following PRISMA guidelines. Studies were eligible if they assesse
