Short-term adjunctive use of a liposomal tear substitute after repeated intravitreal therapy: a randomized pilot study.

PURPOSE: This pilot study investigates the efficacy of a liposomal ophthalmic solution containing cross-linked hyaluronic acid, trehalose, and stearylamine in reducing the occurrence of dry eye-like signs and symptoms in patients undergoing repeated intravitreal treatment. METHODS: Participants aged over 50 years old were randomly assigned to receive either standard therapy alone (Group 1) or standard therapy with additional eye drops (Group 2). Examinations included TBUT, Schirmer's test, fluorescein staining, the Ocular Surface Disease Index (OSDI), and the 5-item Dry Eye Questionnaire (DEQ

Case Report: Antiviral treatment after pterygium surgery based on corneal confocal microscopy.

INTRODUCTION: Pterygium is a chronic inflammatory condition that commonly occurs on the bulbar conjunctiva of the nasal palpebral fissure. Its etiology is multifactorial, and surgical excision remains the primary treatment method. However, recurrence and other complications may affect treatment outcomes. Currently, the diagnosis and treatment of secondary corneal diseases following pterygium excision surgery vary. Therefore, in vivo confocal microscopy of the cornea holds significant clinical importance in clarifying the etiology of the disease. CASE PRESENTATION: An elderly diabetic patient

Acute Bilateral Corneal Oedema Following Suspected Toxic Endothelial Insult: An Intriguing Case Report.

Visual recovery after a suspected toxic endothelial insult can be partial and uneven despite early improvement. We report a 40-year-old male who developed sudden bilateral blurring of vision with mild ocular discomfort after binge alcohol and marijuana intake, with an additional history of prolonged exposure to vehicle-emitted fumes. There was no history of trauma, contact lens use, ocular surgery, topical medication use, fever, discharge, or previous ocular disease. Examination showed severe bilateral visual reduction, epithelial oedema, diffuse stromal corneal oedema, central haze, and promi

Clinical Presentation and Surgical Management of Idiopathic Gingival Fibromatosis: A Case Report.

Idiopathic gingival fibromatosis (IGF) is a rare benign condition characterized by a progressive fibrous enlargement of the gingival tissues in the absence of systemic, pharmacological, or hereditary causes. This case report describes an 11-year-old male presenting with generalized gingival overgrowth causing functional and aesthetic impairment. Clinical examination revealed firm, pale pink, non-hemorrhagic gingival enlargement covering a significant portion of the dental crowns, with pseudo-pocket formation but no evidence of attachment loss. The patient had no relevant medical history, drug

A case report on the co-presentation of pituitary abscess and POEMS-like syndrome.

RATIONALE: POEMS syndrome is a rare paraneoplastic syndrome, manifesting with progressive distal polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, and skin changes, which has multisystem manifestations and chronic cytokine overproduction. Pituitary abscess is also a rare life-threatening disease that could lead to panhypopituitarism. We report a case of pituitary abscess with high suspicion of POEMS syndrome, which to our knowledge has not been reported previously. PATIENT CONCERNS: The co-presentation of pituitary abscess and POEMS-like syndrome is rare and its clinical manif

Acromegaly Discovered After Head Trauma: Multivessel Coronary Disease and Postoperative Meningitis Complicating a Pituitary Macroadenoma.

Acromegaly is a rare endocrine disorder caused by chronic growth hormone (GH) excess, most often from a pituitary somatotroph adenoma. Because the somatic and metabolic features develop slowly, the diagnosis is often delayed for years. We report the case of a 59-year-old man whose acromegaly was identified incidentally after a 1.6 cm pituitary macroadenoma was found on MRI obtained for trauma evaluation following a bicycle accident. History and examination revealed several long-standing features of acromegaly, including widely spaced teeth, mild prognathism, increased shoe and hand size, sever

Collision tumor of poorly differentiated thyroid carcinoma and intrathyroid thymic carcinoma arising in association with a branchial cleft-like cyst: a case report.

BACKGROUND: Primary squamous carcinoma of the thyroid is an exceptionally rare malignancy, and its coexistence with poorly differentiated thyroid carcinoma (PDTC) is exceedingly uncommon. The pathogenesis of thyroid squamous carcinomas remains controversial, particularly when associated with congenital epithelial remnants such as branchial cleft-like cysts or thymic/ultimobranchial remnants. We report an unusual collision tumor composed of PDTC and intrathyroid thymic carcinoma (ITTC) arising in association with a longstanding cystic thyroid lesion. CASE PRESENTATION: A 59-year-old man initia

[Guidelines for the diagnosis and treatment of obstructive sleep apnea in adults (2025)].

Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder with a considerable healthcare burden. Previously, the 2011 guideline for adult obstructive sleep apnea was largely formulated on the basis of expert opinion and consensus, which played an important role in guiding clinical practice. With the growing body of evidence from high-quality clinical studies, however, guideline development models that rely primarily on expert consensus are no longer adequate to support contemporary clinical decision-making. To further standardize the screening, diagnosis, treatment,

Safe surgical management of two concomitant giant hepatic hydatid cysts: a case report.

BACKGROUND: Hydatid disease (HD) is a parasitic infection caused by the larval stage of the tapeworm Echinococcus granulosus. The liver is primary affected, accounting for up to 70% of cases, but it can also affect other organs such as the spleen, pelvic cavity, and bones. Patients can remain asymptomatic for 10 to 15 years. This report, accompanied by an extensive literature review, aims to contribute to the understanding of the diagnostic and surgical management of giant hepatic hydatid cysts, given their rarity and the variety of locations in the body where they occur. CASE PRESENTATION: A

Recurrent Femoral Osseous Hydatid Disease Managed With Repeated Intramedullary Curettage: A Case Report.

Hydatid disease is a parasitic infection primarily caused by Echinococcus granulosus. Osseous hydatid disease is an uncommon manifestation and may present significant diagnostic and therapeutic challenges because of its insidious progression and tumor-like appearance. Femoral involvement is particularly rare. We report the case of a 44-year-old hypertensive woman who presented with chronic mechanical pain of the right hip evolving since 2019 and initially treated as lumbosciatica without improvement. MRI revealed an extensive intramedullary osteolytic lesion involving the right femoral diaphys

Chronic Tonsillitis as a Focal Infection: A Decade-Long Case Involving Severe Systemic Symptoms.

Unlike acute tonsillitis, which is readily recognized as infectious, chronic tonsillitis, tonsilloliths, and tonsillar detritus are often considered non-infectious and benign, despite their potential to act as focal infections causing systemic inflammatory symptoms that are frequently overlooked when tonsillar compression is not performed. We report the case of a 29-year-old female with a decade-long history of progressive musculoskeletal pain, episodic low-grade fever, headaches, and exercise-induced inflammatory arthralgia affecting the feet, ankles, wrists, and spine. The initial otolaryngo

Evaluation of progression-free status at 24 months as a surrogate endpoint for overall survival in patients with human papillomavirus-positive oropharyngeal cancer: a retrospective cohort study.

BACKGROUND: There is minimal evidence supporting the use of early clinical endpoints to evaluate the activity of initial treatment in human papillomavirus (HPV)-positive oropharyngeal cancer in the context of contemporary therapeutic approaches. The aim of this study was to assess progression-free status at 24 months as a potential surrogate endpoint for overall survival at the patient level, within a large, retrospective cohort of patients with HPV-positive oropharyngeal cancer. METHODS: In this retrospective cohort study, we included all consecutive patients aged 18 years or older with newl

Definitive Radiotherapy for Unresectable Unicentric Mediastinal Castleman Disease: A Case Report.

Castleman disease is a rare nonclonal lymphoproliferative disorder with heterogeneous clinical presentations and therapeutic approaches. Surgery remains the standard treatment for unicentric Castleman disease, while radiotherapy may represent an alternative option in unresectable cases. We report the case of a 17-year-old female who presented with exertional dyspnea related to a right hilar mediastinal mass. Chest computed tomography revealed a well-circumscribed lesion closely related to adjacent vascular and bronchial structures. Histopathological examination demonstrated abnormal lymphoid f

Multidisciplinary Treatment of Post-radiation Sarcoma in the Left Axilla Following Radiotherapy for Breast Cancer: A Case Report and Review of the Literature.

Post-radiation sarcoma is a rare clinical entity, but it is considered an aggressive late complication of radiotherapy administered as adjuvant therapy for breast cancer treatment. Although radiotherapy significantly improves local disease control and survival, radiation-induced malignancies may occur years after treatment completion. Surgical resection with negative margins remains the cornerstone of management. However, treatment is often challenging because of prior irradiation, local tissue fibrosis, and advanced local disease. We report the case of a 65-year-old woman previously treated f

Radiation-Induced Second Primary Gingival Squamous Cell Carcinoma Developing 26 Years After Radiotherapy for Nasopharyngeal Carcinoma: A Case Report.

Radiation-induced second primary oral squamous cell carcinoma is a rare but recognized late complication of radiotherapy for nasopharyngeal carcinoma, typically presenting after a prolonged latency period within previously irradiated tissues. A 79-year-old woman developed a moderately differentiated squamous cell carcinoma involving the left edentulous mandibular gingiva and alveolar ridge 26 years after radiotherapy for nasopharyngeal carcinoma, with no history of tobacco or alcohol use. The patient had severe xerostomia and other chronic post-radiotherapy oral complications. Clinical examina

A typical tuberculosis of the wrist joint and flexor-extensor tendon sheaths: A case report.

RATIONALE: Tuberculous tenosynovitis and arthritis of the wrist are a rare condition that are frequently misdiagnosed due to their nonspecific clinical, serological, and radiological presentations, which often mimics other diseases such as pigmented villonodular synovitis (PVNS). This case highlights the critical diagnostic challenges and the importance of maintaining a high index of suspicion for tuberculosis (TB) in atypical musculoskeletal presentations. PATIENT CONCERNS: One patient presented with chronic wrist pain, swelling, and significantly restricted mobility of the wrist and fingers

Adapting the Daily Inventory of Stressful Events for SMS Delivery: Recommendations for Research.

Vernal keratoconjunctivitis (VKC) is a chronic and potentially sight-threatening disease. Topical corticosteroids (Cs) seem to be the only effective treatment for this condition, although severe side effects may occur owing to their prolonged use. More recently, cyclosporine (Cyc) eye drops have been reported as a valid alternative, but so far such treatment has only been successfully experimented for a short time and in small numbers of patients. The aim of our study is to evaluate the long term safety and efficacy of topical cyclosporine eye drops in children suffering from VKC. Over a perio
